Abstract
ÖZET Ejakülasyon vas deferens düz kasının ritmik olarak kasılmasıyla teslislerde üretilen spermlerin üretraya doğru atılması sonucu meydana gelir. Vas deferens kasılması noradrenalin (NA) ve adenozin-5-trifosfat (ATP) gibi bazı lokal endojen faktörler tarafından düzenlenir. NA ve ATP adrenerjik sinir uçlarında beraber depolanır ve beraber salınırlar. Sildenafil sitrat erektil disfonksiyon tedavisinde ağızdan kullanılabilen, siklik guanozin monofosfat'a (cGMP) özgül fosfodiesteraz tip 5 enziminin seçici inhibitörüdür. Erektil disfonksiyona sıklıkla ejakülasyon bozuklukları da eşlik etmektedir. Bu çalışmanın amacı erektil disfonksiyon tedavisinde kullanılan sildenafil sitratın ejakülasyondaki etkinliğini araştırmak ve purinerjik sistemle olan ilişkisini ortaya koymaktır. Çalışmada sıçan izole vas deferenslerinin epididimal ve prostatik segmentleri kullanıldı. NA, seçici olmayan purinerjik reseptör agonisti ATP, seçici P2X purinerjik reseptör agonisti alfa,beta metilen adenozin 5 trifosfat (a,P meATP) ve elektriksel stimülasyona alman yanıtlar üzerine sildenafil sitratm etkisi araştırıldı. Karşılaştırmalar için seçici olmayan purinerjik reseptör antagonisti suramin ve seçici P2X purinerjik reseptör antagonisti Evans mavisi kullanıldı. Vas deferensin her iki segmentinde de NA, ATP, a,P meATP ve elektriksel stimülasyon kasılmaya yol açtı. Sildenafil sitrat ve suramin NA'e bağlı kasılmaları değiştirmezken Evans mavisi epididimal kısımda NA'e bağlı yanıtları potansiyalize etti. Her iki segmentte de sildenafil sitrat ve suramin ATP' ye bağlı kasılmaları non-kompetitif tarzda inhibe etti. Evans mavisi ise ATP yanıtlarını potansiyalize etti. Sildenafil sitrat a,p meATP'ye bağlı kasılmaları değiştirmezken suramin ve Evans mavisi her iki segmentte de kasılmaları inhibe etti. Sildenafil sitrat ve suramin elektriksel stimülasyona bağlı kasılmaları önlerken Evans mavisi kasılmaları potansiyalize etti. VIIISildenafil sitrat vas deferensin her iki segmentinde de kasılmaları suramin'e benzer şekilde inhibe etti. Bu etkinin reseptör düzeyindeki etkiden ziyade ATP nin ikincil etkilerine ya da artmış cGMP'nin hücre dışına çıkışında ATP'ye bağımlı mekanizmalara bağlı olduğu ileri sürülebilir. Sonuç olarak vas deferens kasılmasının sildenafil tarafından önlenmesi sildenafilin prematür ejakülasyon tedavisinde kullanımını gündeme getirecektir. Anahtar sözcükler: Sildenafil sitrat, vas deferens, purinerjik sistem, ejakülasyon, sıçan IX ABSTRACT Ejaculation occurs by rhythmical contractions of the smooth muscles of vas deferens to push the sperms generated in testis into the urethra. Contraction of vas deferens is controlled by some endogenous factors including noradrenaline (NA) and adenosine-5-triphosphate (ATP). NA and ATP are co-stored and released from the adrenergic nevre terminals together. Sildenafil is a selective inhibitor of cGMP-selective type 5 phosphodiesterase which can be used orally in the treatment of erectile dysfunction. Erectile dysfunctions are generally accompanied by ejeculation disorders. The aim of the present study is to investigate the effectiveness of sildenafil citrate which is used in erectile dysfunction, on ejeculation and to establish its connection with purinergic system. In the present study, epididymal and prostatic segments of isolated vas deferens were used. Effects of sildenafil citrate on the responses evoked by NA, ATP (a non-selective purinergic receptor agonist), alpha,beta methylene adenosine-5-triphosphate (a,p meATP) (a selective P2x purinergic receptor agonist) and electrical stimulation were investigated. A non-selective purinergic receptor antagonist suramine and a selective P2x purinergic receptor antagonist Evans blue were also used for comparisons. NA, ATP, a,p meATP and electrical stimulation caused contractions in both segments of vas deferens. While sildenafil citrate and suramine did not alter the contractions caused by NA, Evans blue potentiated the responses evoked by NA. Sildenafil citrate and suramine non-competitively inhibited the ATP- dependent contractions in both segments. Evans blue potentiated the ATP- dependent responses. While Sildenafil citrate did not alter the a,p meATP- dependent contractions, suramine and Evans blue inhibited these contractions in both segments. Sildenafil citrate and suramine inhibited the contractions induced by electrical stimulation but Evans blue potentiated these responses. XSildenafil citrate inhibited the contractions in both segments of vas deferens in the same way as suramin did. This finding suggest that this effect may be related to the secondary effects of ATP or the ATP dependent processes used to decrease the increased intracellular cGMP content, rather than direct effect on receptor site. As a result, inhibition of vas deferens contractions by sildenafil may lead to its possible use in the treatment of premature ejaculation.
