Abstract
ÖZET Amaç: Hormon replasman tedavisi (HRT) kullanan ve kullanmayan postmenopozal dönemdeki kadınlarda psikiyatrik semptomları incelemek, HRT kullanım süresinin, çeşidinin ve menopoz şeklinin psikiyatrik semptomlar üzerine etkisini araştırmaktır. Gereç ve yöntemler: Çalışmaya 135 postmenopozal dönemdeki kadın alındı. Bu kadınlardan 80 tanesi HRT kullanıyorken, 55 tanesi HRT kullanmıyordu. Çalışmaya katılanlara, sosyodemografık verileri içeren bilgi formu, SCL-90 R, Hamilton ve Beck depresyon değerlendirme ölçeği, Hamilton anksiyete değerlendirme ölçeği, durumluk- sürekli kaygı ölçeği, mini mental durum muayenesi (MMSE), yaşam kalitesini değerlendirmek için kısa form-36 (SF-36) ve seksüel fonksiyonları değerlendirmek cinsel istek formu uygulandı. Bulgular: HRT'nin postmenopozal dönemdeki kadınlarda özellikle anksiyete ve cinsel fonksiyonlara olumlu etkilerinin olduğunu bulduk. HRT kullanan kadınlarda kullanmayanlara oranla SCL-90 R testinde somatizasyon, depresyon, anksiyete ve toplam boyutların skorları daha düşüktü. HRT kullanım süresi ile depresyon, anksiyete ve cinsel fonksiyonlar arasında anlamlı ilişki olduğunu saptadık. HRT çeşidinin kognitif fonksiyonlar üzerine etkili bir faktör olduğunu tespit ettik. Fizyolojik menopozlu kadınlarda anksiyete skorları, cerrahi menopozlulara göre anlamlı derecede daha düşüktü. Sonuçlar: Somatik yakınmaları, depresyonu, anksiyetesi ve cinsel fonksiyon bozukluğu olan postmenopozal kadınlarda HRT'nin olumlu etkileri olabilir, özellikle tibolon veya östrojen/progesteron kombinasyonu kullanan kadınlarda kognitif fonksiyonlar daha olumlu olarak etkilenmektedir. Cerrahi menopozu olan kadınlar ise anksiyete yönünden izlenmelidir. Anahtar sözcükler: Hormon replasman tedavisi, postmenopozal kadın, psikiyatrik semptomlar. IV ABSTRACT Objective: The aim of this study is to investigate psychiatric symptoms in women using and not using hormone replacement therapy (HRT) during the postmenopausal period and to compare the symptoms of physiologically or surgically menopausal women and the duration and kind of HRT use. Materials and methods: 135 postmenopausal women were included in the study. 80 of the women were using HRT and 55 of the women were not using HRT. A form including sociodemographic data, SCL-90 R, Hamilton depression rating scale (HAM-D), Beck depression rating scale, Hamilton anxiety rating scale (HAM-A), state- trait anxiety inventory (STAI), mini mental state examination (MMSE), short form 36 (SF-36) for the evaluation of life quality and sexual desire form for the evaluation of sexual functions were applied to all participations in the study. Findings: We found that HRT in women in the postmenopausal period had positive effects especially on anxiety and sexual functions. In women using HRT compared to non-users, the scores of somatization, depression, anxiety and total dimensions in the SCL-90 R test were low. We found that there was meaningful relationship between HRT period of use and depression, anxiety and sexual functions. We determined that the kind of HRT was an effective factor on cognitive functions. The anxiety scores in physiologically menopausal women were significantly lower than surgically menopausal women. Results: HRT may have positive effects on postmenopausal women who had somatic complaints, depression, anxiety and sexual dysfunction. Cognitive functions have been more positively affected in women using tibolon and oestrogen/progesteron combination. Surgically menopausal women should be followed up in terms of anxiety.
