Üveitli hastalarda katarakt cerrahisi sonrasında arka kapsül kesafeti gelişimi

Abstract
11 ÖZET Amaç : Üveitli hastalarda katarakt ekstraksiyonu sonrası AKK gelişimini etkileyen faktörleri araştırmak. Hastalar ve Yöntem : Ondokuz Mayıs Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalında Temmuz 1991 - Ekim 2003 tarihleri arasında ekstrakapsüler katarakt ekstraksiyonu ve GİL implantasyonu uygulanan ve en az 6 ay takip edilen 40 üveitli hastanın 60 gözü retrospektif olarak incelendi. İmplante edilen GİL'nin tipi, cerrahi yöntem, ön kapsülektomi ve GİL'nin implantasyon yeri AKK gelişiminde rol oynayan faktörler araştırıldı. AKK'nın derecesi biomikroskopik muayenede sübjektif olarak değerlendirildi. Bulgular : Hastaların 23'ü erkek ve 17'si kadın olup ortalama yaş 37.6 + 9.97 yıl (22-59 yıl), ortalama preoperatif takip süresi 103.82 +74 ay ve ortalama postoperatif takıp süresi 47.25 ±33.73 aydı. En az AKK gelişimi hidrofobik akrilik lens grubunda tespit edilirken en şiddetli AKK gelişimi PMMA lens grubunda tespit edildi. Bu iki grup arasındaki fark istatistiksel olarak anlamlı bulundu (P=0.04). Diğer lens grupları arasında anlamlı fark bulunmadı (P>0.05). Toplam 21 gözde (%35) YAG arka kapsülotomi uygalanırken en düşük oran hidrofobik akrilik lens grubunda tespit edildi. PEKKE, FAKO ve kombine FAKO+PPV cerrahi yöntem arasında AKK gelişimi açısından istatistik olarak anlamlı bir fark bulunmadı (P>0.05). CCC yönteminde, can opener ve envelope yöntemlerine göre daha az AKK gelişimi tespit edilmesine rağmen istatistik olarak anlamlı bir fark bulunmadı (P>0.05). Kapsül içi implante edilen GİL grubunda sulkusa implante edilen GİL grubuna göre daha az AKK gelişti. Fakat iki grup arasındaki fark istatistik olarak anlamlı bulunmadı (P>0.05). Sonuç: Üveitli hastalarda AKK gelişimini etkileyen birden fazla faktör olmasına rağmen, en etkili faktör implante edilen GİL'nin tipi olarak tespit edildi. Üveitli hastalarda AKK gelişiminde diğer faktörlerin etkisini değerlendirebilmek için kontrollü ve daha geniş çalışmalara ihtiyaç vardır. Ill ABSTRACT Purpose : To investigate factors affecting the development of PCO following to cataract extraction in patients with uveitis. Patients and methods : 40 patients with uveitis (60 eyes) who had extracapsular cataract extraction and intraocular lens implantation at Ondokuz Mayıs University Faculty Of Medicine Department Of Ophthalmology between the period of July 1991- October 2003 were retrospectively evaluated. All patients were followed postoperatively for aperiod of 6 months at least. The type of IOL, surgical technique, anterior capsulotomy and IOL placement site were evaluated as factors affecting the development of PCO. The degree of PCO was subjectively determined during the biomicroscopical examination. Results : 23 of the patients were males and 17 were females. The average age of the patients was 37.6 + 9.97 years (22 - 59 years), the average preoperative following period was 103 + 74 months and the average postoperative following period was 47.25 ±33.73 months. The lowest scores of PCO was found in hydrophobic acrylic lens group, whereas the most severe PCO development occured in PMMA lens group. There was significant difference between these two groups ( P=0.04). There was no significant difference between other lens groups (P> 0.05). Nd:YAG posterior capsulotomy was applied to 21 eyes (%35) of the whole series, the lowest rate was recorded in hydrophobic acrylic lens group. There was no significant diference between PECCE, phacoemulsification and combined phacoemulsification + PPV surgical techniques regarding to PCO development (P> 0.05). Although of lower PCO incidence rate in CCC group comparing to can opener and envelope anterior capsulotomy groups, there was no significant difference between the three groups (P> 0.05). Eyes with IOL implantation in the capsular bag had less PCO incidence rate than eyes with ciliary sulcus IOL implantation. The diference between the two groups was not significantly important (P> 0.05). Conclusion: İn patients with uveitis, there are many factors affecting the development of PCO, the most affective factor was the type of IOL implanted. To evaluate the effect of other factors, there is a need for more studies with bigger number of patients.
