Abstract
ÖZET Geri plan: Deliryum, akut olarak başlayan ve dalgalı bir seyir gösteren global mental durum değişikliği, özellikle dikkat kaybı ile karakterize olan bir nöropsikiyatrik sendromdur. Bir hastalık nedeni ile hastaneye yatırılan hastalar arasında ki deliryum sıklığı % 2 ile % 30 olarak bildirilmektedir. Bununla birlikte literatürde acil serviste yapılmış deliryum sıklığı ile ilgili çalışmaların sayısı son derece azdır. Bu çalışmalar yaşlı hasta gruplarından oluşmaktadır. Bilgilerimize göre bizim çalışmamız acil serviste yapılmış tüm erişkin yaş gruplarım içeren ilk çalışmadır. Amaçlar : 1) Deliryumu etyolojik olarak sınıflandırmak ve 2) deHryumun sıklığı ve etyolojisi bakımından 65 yaş altı ve 65 ve üstü yaş gruplarım karşılaştırmak. Metod: 1 aralık 2002 ile 1 aralık 2003 tarihleri arasında Üniversitemizin acil servisinde (Mental Bozuklukların Tanısal ve Sayımsal Elkitabı -IV ) DSM IV deliryum tanılı 43 hasta çalışmaya dahil edildi. Hastalar 65 yaş altı ve 65 ve üstü yaş olmak üzere iki alt gruba ayrıldı. Her bir hastada, ayrıntılı anamnezi takiben tam bir fizik ve nörolojik muayene yapıldı. İlave olarak mini mental durum değerlendirme skalası (MMDD) uygulandı. Direkt radyolojik ve hematolojik tetkiklerin yanısıra yapısal kranial patolojilerin varlığım düşündüren hastalardan kranial nörogörüntüleme de elde edildi. Tüm veriler özel çalışma kartlarına işlendi. Çalışmanın sonunda verilerin istatiksel analizi yapıldı ve istatiksel yöntem olarak student t ve x2 testleri kullanıldı. Bulgular: 43 deliryumlu hastanın 18 'i bayan (% 42) ve 25 'i erkek (%58) olup, kadın - erkek oram 0,72 idi. Yaş ortalaması 61,18+18 idi. Olguların % 49'u (n=21) grup l'de yer alırken %51'i (n=22) grup 2'de yer aldı. Deliryum etyolojisinde en sık metabolik nedenler saptandı. Deliryum ve etyolojik faktörlerin sıklığı bakımından grup 1 ve 2 arasında istatiksel olarak anlamli fark yoktu. Sonuç: Acil serviste deliryum tanısı alan tüm hastalar, başta metabolik nedenler olmak üzere, etyolojik yönden mutlaka araştırılmahdır. Çünkü, deliryumun tedavisinde esas olanın altta yatan nedenin tedavisi olduğu bildirilmektedir. Anahtar kelimeler : acil servis, deliryum, etyoloji, yaş grupları IV ABSTRACT Background: Delirium is a neuropsychiatric syndrome characterized by the acute onset and fluctuating course of globally altered a mental status. It has been reported that the frequency of delirium among patients hospitalized with any disorder ranges from 2 to 30 %. However, in the literatüre, the number of such studies is infrequency. These studies have included patients who are 65 age and older. According to our knowledge, our study is first study including adult patients in all age groups. Objectives: 1) to classify delirium according to its etiologic causes and 3) to compare the age groups for the etiology and frequency of delirium. Methods: Fourty - three patients were included in this study, diagnosed as a delirium according to (Diagnostic and Statistical Manual of Mental Disorders - IV) DSM IV in emergency service (E S) from december 2002 to december 2003. The patients were subdived into two groups as 65 age (the group 1) and 65 age and older (the group 2). Following a detail history, each patient was underwent to physical and neurological examinantion. Additionaly, The Mini Mental State Examination (MMSE) was applied. Radiological and hematological examinations were obtained from all patients who had indications. In the end of study, the statistical analies of all data were made, The student t -test and x2 test were used for statistical analysis. Results: Of patients with delirium, 18 were female and 25 were male. Mean age was 61,18 ±18 (from 17 to 90). Of the pateints, 49 % (n=21) were placed in the group 1 and 51 % (n=22) were placed in the group 2. The most common causes were metabolic disorders in the patients with delirium. There was no significant difference betwen the group 1 and 2 for the frequency, and etiology of delirium Likewise, the frequency and etiology of delirium were not significantly different between the age groups. Conclusions: In emergency service, all of the patients with delirium should be evaluated for etiologic factors, especially metabolic disorder. Because it has been reported that the treatment of underling cause is essential m delirium. Key words : emergency service, delirium, etiology, age groups
