Abstract
ÖZET Şizofreni, 'gerçek' algısının bozulduğu, halüsinasyon ve hezeyanlarla karakterize kronik bir hastalıktır. Klinik tablo içinde sanrıların, hezeyanların, garip davranışların egemen olduğu form pozitif belirtili, affektif küntleşme, aloji, anhedoni, apati ve insiyatif kaybının egemen olduğu form ise negatif belirtili şizofreni olarak adlandırılmaktadır. Son yıllarda şizofreni hastalarında yapılan nöro-görüntüleme çalışmalarında, hastalığın belirleyici belirti ve bulguları ile ilintili olabilecek anormallikler saptanmıştır. Şizofreni hastalarında psikotik ve davranışsal belirtilere kognitif belirtilerin de eşlik ettiği çok sayıdaki çalışmada gösterilmiştir. Nörokognitif testlerde normallere ve diğer psikiyatrik hastalıklara oranla şizofreni hastalarının daha kötü performans gösterdikleri bulunmuştur Şizofreni hastalarında limbik (hipokampus ve amigdala) ve paralimbik (anterior singulat girus, temporal kutup, insula ve orbito frontal korteks) yapıların MRG hacim ölçümleri, nöropsikolojik test performansları ile pozitif ve negatif belirtiler arasındaki ilişkiyi araştırmak amacı ile bu çalışma kurgulanmıştır. Çalışmaya DSM IV tanı kriterlerine göre şizofreni tanısı konulmuş 15 hasta (8 erkek, 7 kadın) ve eğitim düzeyleri, el tercihleri benzer 15 sağlıklı kontrol olgusu alınmıştır. Hasta ve kontrol grubuna nörokognitif test ile kranial magnetik rezonans görüntüleme yapılarak limbik (hipokampus ve amigdala) ve paralimbik (anterior singulat girus, temporal kutup, insula ve orbitofrontal korteks) yapıların MRG hacimleri ölçülmüştür. nŞizofreni hastalarında dikkat, yürütücü işlevler, vizuospasyal işlevler normallere göre kötü olarak bulunmuştur. Hasta grubunda pozitif semptomlarla MRG hacimleri ve nörokognitif testler arasında anlamlı bir ilişki saptanmamıştır. Negatif belirtiler ile vizuospasyal testler, bellek testleri, dikkat ve yürütücü işlev testlerinde anlamlı bozuklukların bulunduğu görülmüştür. Limbik ve paralimbik alanlarda hacim azalması saptanmıştır. Sonuç olarak, hacim kaybının nöral dokuda hasan yansıttığı düşünülürse, limbik ve paralimbik patolojinin şizofrenide görülen belirti ve bulguların nöral alt yapısını oluşturduğu ileri sürülebilir. Anahtar sözcükler: Şizofreni, nörokognitif test, MRI volümetri in ABSTRACT Schizophrenia is a chronic disease that is characterized with impairment of reality perception, hallucination and delirium. In the clinic profile, if the hallucinations, delirium and strange behaviors are dominant, then named as positive sypmtom, if affective flattening, alogy, anhedoni, apatia and loss of iniative is dominant, then named as negative sypmtom shizophrenia. Recently, in neuroimaging studies, some abnormalities that may be related to the signs and symptoms of the disease have been shown. Most of the studies have shown that cognitive signs accompany the psychotic and behavioral signs in schizophrenia patients. Schizophrenia patients have poor neurocognitive test performance than the normals and other psychiatric patients. This study was aimed to design a correlation between the MRI volumetric measurments of limbic (hippocampus and amygdala), paralimbic (anterior cyngulat gyrus, temporal pole, insula and orbitofrontal gyrus ) structures, neurocognitive test performance and positive and negative signs. We included 15 patients (8 men, 7 women) that are diagnosed according to the DSM IV criteria and 15 healthy control volunteer that have similar education and hand preference. We applied neurocognitive test bataria and measured the volumes of limbic (hippocampus and amygdala) and paralimbic (anterior cyngulat gyrus, temporal pole, insula and orbitofrontal gyrus) structures using MRI. in schizophrenia patients, attention, executive functions and visospatial functions were impaired according to the normals. In the patient group, there was not a positive correlation between the positive symptoms and, neurocognitive tests and MRI volumetry. We found a positive correlation between the visuospatial teste, memory tests, attention, executive functions and negative signs. IVMRI reveoled volume loss in limbic and paralimbic areas. Taken into account that volume loss indicates neural tissue damage, we may suggest that limbic and paralimbic pathology may be responsible for the neural basis of the signs and symptoms in schizophrenia.
