Abstract
ÖZET Çalışmanın Amacı: Bu çalışmanın amacı, nedeni açıklanamayan yineleyen gebelik kayıplarında protein S, protein C, antitrombin III, APCR, faktör VIII, homosistein düzeylerini araştırmak ve habitüel abortus grubunda bu faktörlerin etkisini değerlendirmektir. Uygulanan Yöntemler: Çalışma Mart 2004-Haziran 2005 tarihleri arasında, Ondokuz Mayıs Üniversitesi Kadın Hastalıkları ve Doğum Kliniği'nde, nabitüel abortus tanısı almış 40 vaka ve en az bir sağlıklı gebelik geçirmiş 40 kontrol grubu ile yapıldı. Vaka ve kontrol grubu protein S, protein C, APCR, faktör VIII düzeyleri koagülometrik yöntemle, antitrombin ni düzeyleri Berichrom AT III yöntemi ile ve homosistein düzeyleri FPIA ile ölçüldü, istatistiksel değerlendirmede Mann Whitney U, Fischer-Exact testi kullanıldı. Çalışmanın Bulguları: Protein S aktivitesi vaka grubunda ortalama %82,20±23,66 (%40,50-120,30) iken, kontrol grubunda ortalama %90,62±24,14 (%25,90-150,50) idi. İki grup arasında istatistiksel açıdan anlamlı farklılık saptanmadı (p>0,05). Konjenital protein S eksikliği vaka grubunda saptanmaz iken (%0), kontrol grubunda l hastada saptandı (%2,5). Vaka grubu protein C aktivitesi ortalama %118,00±26.03 (%38,50-150,50) iken, kontrol grubunda %135,69±22,77 (%44,80-155,50) idi. İki grup arasında protein C aktiviteleri açısından anlamlı farklılık saptandı (p<0,05). Heterozigot protein C eksikliği vaka grubunda 2 hastada (%5), kontrol grubunda l hastada saptandı (%2,5). İki grup antitrombin III aktiviteleri karşılaştırıldığında, vaka grubunda %106,22±15,02 (%90,90-150,00) iken, kontrol grubunda %115,61±10,77 (%85,10-150,00) idi. İki grup arasında istatistiksel olarak anlamlı farklılık bulundu (p=0,001). Her iki grupta da konjenital AT ni eksikliği saptanmadı. APCR, vaka grubunda 2 kişide saptanırken (%5), kontrol grubunda l kişide saptandı (%2,5). İki grup arasında APCR Vaçısından anlamlı farklılık yoktu (p>0,05). Vaka grubunda faktör VIII aktivitesi ortalama %135,67±35,21 (%80,90-210,20) iken, kontrol grubunda ortalama %129,07±33,46 (%80,50- 203,90) idi. İki grup arasında faktör VIII aktivitesi açısından anlamlı farklılık saptanmadı (p>0,05). Vaka grubunda 14 kişide (%35), kontrol grubunda 12 kişide (%30) normalin üzerinde değer saptandı. Plazma homosistein düzeyleri karşılaştırıldığında, vaka grubunda ortalama 13,60±10,47 umol/L iken, kontrol grubunda 9,48±4,71 umol/L idi. İki grup arasında istatistiksel olarak anlamlı farklılık mevcuttu (p<0,05). Vaka grubunda 6 kişide hafif (%15), 4 kişide orta şiddette (%10) hiperhomosisteinemi saptanırken, kontrol grubunda 6 kişide hafif hiperhomosisteinemi saptandı (%15). Sonuç: Nedeni açıklanamayan habitüel abortus olguları trombofili açısuıdan değerlendirihnelidir. Çalışmamızda, habitüel abortuslu hasta grubunda protein C, AT III ve homosistein düzeylerinde kontrol grubuna göre anlamlı farklılık bulunması, tekrarlayan abortuslarda bu faktörlerin önemli rol oynayabileceği fikrini desteklemektedir. Özellikle ailesel trombofili öyküsü olanlarda antikoagülan tedavi başlanması açısuıdan, bu faktörleri değerlendirmek önemli olabilir. Bu konu ile ilgili tüm parametrelerin değerlendirildiği geniş serili çalışmalara ihtiyaç vardır. Anahtar Kelimeler: Yineleyen gebelik kayıpları, protein S-C, APCR, AT ni, homosistein. ABSTRACT Objective: The aim of this study is to investigate the protein S, protein C, AT III, APCR, F VIII, homocystein levels in unexplained recurrent pregnancy losses and assess the effect of these factors in the habitual abortion group. Study Design: This study was made in Ondokuz Mayıs University, Obstetrics and Gynecology Clinic between March 2004- June 2005 with 40 patients who diagnosed with habituel abortion and a control group of 40 patients who at least once had a healthy pegnancy. In both groups protein S, protein C, APCR, F VIII (with coagulometric procedure), AT III (with AT III Berichrom procedure) and homocysteine (with FPIA procedure) levels measured. In statistical analysis Mann Whitney U and Fischer-Exact test were used. Results: The mean protein S activity was 82,20±23,66% (40,50-120,30%) in the case group, while 90,62±24,14% (25,90-150,50%) in the conrol group. No significant difference was found statistically between two groups (p>0,05). Congenital protein S deficiency was diagnosed in none of the study (0%) group, while 1 of the control group (2,5%). The protein C activity of the case group was 118,00±26,03% (38,50-150,50%), while it was 135,69±22,77% (44,80-150,50%) in the control group. Statistically significant difference was found between the two groups (p<0,05). Heterozigot protein C deficiency was found in 2 of the study (5%) and 1 of the control group (2%,5). In the terms of AT III activity, it was 106,22±15,025% (90,90-150,00%) in the case group while it was 115,61±10,77% (85,10- 150,00%) in the control group. Statistically significant difference was found between the two groups (p=0,001). No cases of congenital antithrombin III deficiency were found in any case and control groups. There were 2 (5%) APCR in the study group and was 1 (2,5%) in the control group. The difference between the two groups was not statistically significant vn(p>0,05). The mean level of F VIII activity was 135,67±35,21% (80,90-210,20%) in the case group while it was 129,07±33,46% (80,50-203,90%) in the control group. The difference between the two groups was not statistically significant (p>0,05). The higher levels of F VIII activity were found in 14 (35%) of the study group and 12 (30%) of the control group. When homocysteine levels were compared, the mean value was 13,60±10,47 umol/L in the study group while it was 9,48±4,71 umol/L in the control group. The difference was statistically significant between the two groups (p<0,05). Mild hyperhomocysteinemia was found in 6 of the study (15%) and 6 of the control group (15%). Intermediate hyperhomocysteinemia was found 4 of the study group (10%). Conclusion: Unexplained recurrent pregnancy losses should be evaluated in the field of thrombophilia. Finding of significant differences in the levels of protein C, AT III and homocysteine levels while compared to the control group in our study, supports the idea of the possibility of the role of these factors in the recurrent pregnancy losses. From the point of starting anticoagulant therapy, in especially the patients with family history of thrombophilia, it can be significant to evaluate these factors. Studies with larger series are needed to clarify all aspects of these thrombophilia parameters on unexplained recurrent pregnancy losses.
