İntrabdominal yerleştirilen prostetik materyallerin barsak geçiş hızına etkisi

Abstract
ÖZET Amaç: İnsizyonel hernilerin tek tedavisi cerrahidir. Cerrahide genellikle defekt 3 cm. den büyükse yama ile onarım yapılmakta olup bu amaçla değişik materyaller kullanılmaktadır. Bu prostetik materyaller nümsün daha az olması ve daha fizyolojik olması düşünülerek hastalarda sıklıkla uygulanmaktadır. Bizim kliniğimizde de hem kapalı hem açık onarımlarda intraabdominal yamalar kullanılmaktadır. Bu onarımın uygulandığı birçok hastada, postoperatif dönemde distansiyon karın ağrısı ve subileus tablosu görülmüştür. Kullanılan yamalar ile bu klinik tablo arasındaki ilişkiyi ortaya çıkarmak için bu çalışma yapıldı. Gereç ve Yöntem: Çalışmada Wistar Albino türü ortalama ağırlıkları 200-250gr. olan toplam 52 adet rat kullanıldı. Çalışma her biri 10 rattan oluşan 4. deney grubu, 6'şar rattan kontrol ve sham grubu ile yapıldı. Çalışma gruplarına ratın tüm batınını örtecek büyüklükte Gore-tex, Sepramesh, Parietex Composite ve Parietene Composite mesh yerleştirildi. Cerrahi işlemin 7. günü bütün ratlara 2 mi % 50 sulandırılmış baryum sülfat gavajdan verildi. Kırk beş dakika sonra tüm ratlar sakrifiye edilip yapışıklık skorlaması yapıldı ve pilordan çekuma kadar ince barsaklar çıkarıldı. İnce barsakların uzunluğu ve baryum sülfatın pilordan çekuma doğru kastettiği mesafe ayrı ayrı ölçülüp oranlanarak intestinal geçiş hızı belirlendi. Bulgular: Kontrol ve sham grubunda yapışıklık skorlaması ve barsak geçiş hızları benzer bulundu. Deney gruplarından Parietex Composite grubunda kontrol ve sham grubuna yakın geçiş hızı ve yapışıklık tespit edildi. Parietene Composite, Sepramesh ve Gortex grubunda yapışıklık anlamlı oranda yüksek bulunmasına rağmen barsak geçiş hızı anlamlı oranda düşük bulundu. Tartışma ve Sonuç: Batın içi prostetik materyal kullanımı sonucu, barsak hızının belirgin bir şekilde etkilendiği görüldü. Yapışıklık miktarıyla barsak geçişi arasında ters orantı tespit edildi. Polipropilen ve PTFE meshlerinde polyestere göre daha fazla geçiş hızında gecikme olduğu tespit edildi. Anahtar kelimeler: Yama, barsak geçiş hızı, yapışıklık, fıtık. VÎI Abstract Purpose: The only treatment modality for incisional hernias is surgery. Various types of prosthetic materials are used, particularly for defects larger than three cm. Decreased risk of recurrence is one of the advantages of these materials. Prosthetic materials are commonly used both in open and laparoscopic procedures in our department. However, patients who undergo this type of operations suffer from postoperative abdominal distension, pain and subileus. The aim of this study is to investigate the relationship, if any, between the complaints and the type of material used. Materials and method: Fifty-two Wistar Albino rats, weighing 200-250 g, were used in this study. They were divided into four study groups (10 each), one control and one sham-operated group (six each). Gore-tex, Sepramesh, Parietex Composite or Parietene Composite meshes were placed all over the anterior abdominal wall of rats. In postoperative day seven, all rats were administered liquid barium sulphate. Forty-five minutes later, all rats were sacrificed, scored for abdominal adhesions and all of the small intestine have been resected. Length of small intestine was measured, as well as the length barium sulphate passed, thus the intestinal transit time was decided. Results: adhesion scores and intestinal transit times for control and sham groups were the same. In Parietex Composite group, these variables were similar with those in the control and sham groups. In Parietene Composite, Sepramesh or Gortex groups, adhesion score was significantly higher but transit time was significantly lower than the control and sham groups. Conclusion: intraabdominaily placed prosthetic materials significantly affects intestinal transit times. Amount of adhesion and transit time was inversely proportional. There was a higher delay in transit time for polypropylene and PTFE grafts when compared with polyesther. Keywords: Graft, intestinal transit time, adhesion, hernia. VIII
