Abstract
ÖZET Giriş: İyonizan radyasyon bilinen en güçlü kataraktojenik ajanlardan biridir. Lens opasifikasyonunu açıklamak için önerilen mekanizmalardan bir tanesi ya radyasyon ile yada reaktif oksijen bileşikleri ile kristalinlerin oksidasyonudur. Lens diğer dokular gibi oksidayonun yıkıcı etkilerine karşı korunabilmek için koruyucu mekanizmalara sahiptir. Bunlar süperoksit dismutaz (SOD), katalaz (CAT) ve glutatyon peroksidazdır (GSH-Px). Oksidatif hasarın belirtilerinden biri lipid peroksidasyonun son ürünü olan malondialdehittir(MDA). Antioksidan koruyucu sistemde azalma artmış lens oksidasyonundan ve katarakt gelişmesinden sorumludur. En yüksek kan seviyesi gece ölçülen melatonin sirkadien ritme bağlı olarak pineal bezden salgılanan nöro- hormondur. Melatoninin bir çok dokuda antiperoksidaz aktivitesine sahip olduğu bulunmuştur. Melatoninin serbest oksijen radikalleri temizleyici etkisi olduğu gösterilmiştir. Amaç: Total kraniuma tek doz 5 Gy radyoterapi sonrası lenste radyasyonun oluşturduğu katarakta karşı 5 mg/kg/gün dozundaki Melatoninin antioksidan rolünü belirlemek. Materyal ve Metod: Bu deneyde Sprague-Dawley ratlar kullanıldı ve dört gruba ayrıldı. Radyoterapi ve Radyoterapi+Melatonin grubundaki ratlara total kraniuma tek doz 5 Gy kobalt 60 cihazı kullanılarak radyoterapi uygulandı. Melatonin ve Melatonin grubundaki ratlara günde 5 mg/kg/gün dozunda 10 gün süre ile intraperitoneal melatoninin verildi. Kontrol grubundaki ratlara 10 gün süre intraperitoneal serum fizyolojik uygulandı. Bu çalışmada Chylacke'ın katarakt sınıflaması kullanıldı. 10. günün sonunda ratlar öldürüldü ve lipid peroksidasyon seviyesi MDA ve SOD, GSH-Px gibi antioksidan enzimlerin aktivitesini ölçmek için gözleri enükle edildi. Sonuçlar: Grade 1 katarakt gelişimi RT grubundaki 8 ratda tespit edilirken, RT+Melatonin grubunda yalnızca 2 ratda tespit edildi. Bununla beraber hem kontrol grubunda hem de Melatoninin grubundaki rafların hiç birinin lensinde biomikroskopik olarak her hangi bir değişiklik tespit edilmedi. RT grubunda grade 2 katarakt bir ratda tespit edildi. RT grubundaki rat lenslerindeki MDA seviyesi Kontrol, Melatonin ve RT+Melatonin gruplarından anlamlı olarak daha yüksekti (p<0.003, p<0.000, p<0.011). RT grubundaki GSH-Px aktivitesinin Kontrol, Melatonin ve RT+Melatonin gruplarındanVI daha düşük olduğu tespit edildi (p<0.007, p< 0.000, p<0.000 ). Radyoterapi grubundaki SOD aktivitesi Kontrol, Melatonin ve RT+Melatonin gruplarından daha düşüktü (p< 0.007, p<0.000,p<0.001). Sonuç: Melatonin oksidatif stres oluşumunu önleyici etkisinden dolayı radyasyonun oluşturduğu katarakta karşı koruyucu bir etkiye sahiptir. Bu etkisi ile melatonin radyoprotektör bir ajan olarak kullanılabilir. One of the mechanisms proposed to explain lens opacification is the oxidation of crystallins, either by radiation or reactive oxygen species (ROS). The lens, like other tissues, contains a series of defense mechanisms that may protect it from the injurious effects of oxidation. These include superoxide dismutase (SOD), catalase (CAT) and glutathione peroxidase (GSH-Px). One of the indices of oxidative damage is the malondialdehyde (MDA) formation as an end product of lipid peroxidation. A decrease in the antioxidant defenses could be responsible for increased lens oxidation and cataract development. Melatonin is neurohormone essentially produced in pineal gland according to a circadian rhythm, the highest blood level being measured at night. Melatonin has been found to have an antiperoxidative effect on several tissues. It has been shown that melatonin has a scavenger effect on ROS. Purpose: To determine the antioxidant role of Melatonin (5 mg/kg/day) against radiation-induced cataract in lens after total-cranium irradiation of rats with a single dose of 5 Gy. Materials and Methods: Sprague-Dawley rats were used in this experiment and were divided into four groups. Rats in the radiation and the radiation+ melatonin groups were exposed to whole cranial irradiation of 5 Gy in a single dose delivered by Co-60 teletherapy unit. The rats in the radiation-melatonin and the melatonin groups were received 5mg/kg melatonin daily intraperitoneal injection starting from the first day of radiation for ten days. Group 1 was received an intraperitoneal injections of saline for ten days. Chylacke's cataract classification was used in this study. At the end of 10 days, the rats were killed and their eyes were enucleated to measure the antioxidant enzymes (SOD), (GSH-Px ) and lipid peroxidation level(MDA). Results: While grade 1 cataract development was detectable in 8 rats in the RT group, it was detectable only in 2 rats in the RT+Melatonin group, whereas none of the rats both in the control group and in the RT+Melatonin group exhibited any bio-microscopic change in their lenses. In the RT group, grade 2 cataract was detectable in 1 rat. MDA level in the rat lens in the RT group was significantly higher than in the Control, theVIII Melatonin, and the RT+Melatonin groups (p<0,003, p<0,000, p<0,01 1 respectively) In the RT group, GSH-Px activity was significantly lower than in the Control, the Melatonin and the RT+Melatonin groups. (p<0,007, p< 0,000, p<0.000 respectively) SOD activity in the RT group was lower than in the Control, the Melatonin, and the RT+Melatonin groups (p< 0.007, p<0.000, p<0.001 respectively). Conclusion: Melatonin has a protective effect on radiation-induced cataract by its preventing effect on oxidative stress. It may use as a radioprotector agent.
