Akciğer rezeksiyonu sonrası gelişen doku hacim değişikliklerinin steorolojik metodlar ile değerlendirilmesi

Abstract
ÖZET Giriş ve Amaç: Akciğer rezeksiyonunu takiben, oluşan plevral boşluk, kalan akciğer tarafından tam ya da parsiyel olarak doldurulur. Literatürde, rezeksiyon sonrası akciğerde meydana gelen hacimsel değişiklikleri, Yüksek Rezolüsyonlu Akciğer Tomografisi (YRBT) görüntüleri üzerinden değerlendiren bir çalışmaya rastlanmadı. Bu çalışmada, preoperatif ve postoperatif dönemde YRBT görüntüleri üzerinde steorolojik yöntemlerle toplam akciğer ve her bir akciğer lobunun hacimlerinde meydana gelen değişiklikleri hesaplamayı amaçladık. Materyal-Metod: Çalışma, 2002-2005 yıllan arasında Samsun Ondokuz Mayıs Üniversitesi Tıp Fakültesi Göğüs Cerrahi Kliniğinde akciğer kanseri nedeniyle ameliyat edilmiş 25 hasta üzerinde gerçekleştirildi. Bütün hastalar erkekti. Ortalama yaş 59,6±8,3 bulundu. Total hacim ve her bir lobun volüm ve değişiklik oranlan, preoperatif ve postoperatif Yüksek Rezolüsyonlu Akciğer Tomografisi (YRBT) görüntüleri üzerinde nokta sayım yöntemi kullanılarak hesaplandı. Sonuçlar: Tüm hastalar küçük hücre dışı akciğer kanseri nedeniyle ameliyat edildi. Ortalama total pulmoner volüm preoperatif periyotta 6.40±1.40 litre, postoperatif periyotta 4,92±1,21 litre olarak bulundu. Ekspanse grupta, akciğerlerin ortalama 30,79±10,64%'u rezeke edildi. Ancak, total akciğer hacmi bu grupta ortalama 18,51±15,96% oranda azaldı. Yani; ekspanse grupta, akciğer rezeksiyonu sonrasında oluşan plevral boşluk kalan akciğer tarafından %50 oranda dolduruldu. Tartışma: Çalışmamızın sonuçlarına göre; total pulmoner dokunun ve her bir lobun hacim ve hacim fraksiyonlan Steorolojik metotlardan Cavalieri prensibini kullanarak YRBT VIIIkesit görüntüleri üzerinden pratik olarak hesaplanabilir. Elde edilen hacim bilgilerinin sintigrafik olarak belirlenen tahmini hacim bilgileri ile karşılaştırmalı olarak değerlendirilmesinin, bu metodun gelecekte yapılacak çalışmalarda daha doğru kullanılması açısından uygun olacağını düşünmekteyiz. To our knowledge, no study has yet evaluated this volumetric change in thoracic content using high resolution computed tomography (HRCT) scans. We aimed to evaluate the volume changes of the lungs using HRCT scans during the preoperative and postoperative periods. Materials and Methods: In this study, we took HRCT scans of 25 patients preoperatively and three months after the resection. All patients were male and their mean age was 59.6±8,3 years. The volume and volume fraction of individual pulmonary lobes were estimated by superimposing point-counting grids on the preoperative and postoperative HRCT scans. Results: All patients were operated due to nonsmall cell lung cancer. The mean volumes of total pulmonary volumes in all patients were 6.40±1.40 liters and 4.92Ü.21 liters in the preoperative and postoperative periods, respectively. While, 30.79±10.64% of pulmonary tissue was resected in expanded patients, the pulmonary volumes was diminished by 18.51±15.96% postoperatively. Volumetric analyses showed that the remaining lung increased its volume by 12.28%. Thereby, almost 50% of the resected pulmonary tissue was recovered volumetrically by the remaining tissues. Conclusions: Present results showed that the volume and volume fraction lung tissues and individual lobes could be estimated practically on HRCT scan using the method proposed in this study. We suggest, however, that a comparative study evaluating the volume changes estimated by stereological methods and by a scintigraphic approach would provide a judgment about which method is the most suitable to apply.
