Abstract
ÖZET Bu çalışmada amacımız, ürolityazisli hastalarda hiperoksalüri ve hipositratürinin risk faktörü olarak görülme sıklığını belirlemek ve analiz edilen taş cinsine göre, ekskrete edilen analit (oksalat ve sitrat) düzeylerini karşılaştırmaktır. Aralık 2004-Mayıs 2006 tarihleri arasında Ondokuz Mayıs Üniversitesi Tıp Fakültesi Üroloji Anabilim Dalma üriner sistem taş hastalığı nedeniyle başvuran 137 hasta çalışmaya dahil edilmiştir. Tüm hastalarda 24 saatlik idrarda oksalat ve sitrat ölçümü yapılmıştır. 61 hastanın taş analizi sonucu belirlenmiş ve hastalar taş içeriğine göre 3 gruba ayrılmıştır. I. Grup; pür kalsiyum oksalat taşı, II. Grup; kalsiyum oksalat mixt ve III. Grup kalsiyum oksalat içermeyen taş oluşturmuş hastalardan oluşmaktadır. Çalışmaya alınan hastaların 86'sı erkek, 51 'i kadın olup, yaş ortalamaları 47.255 ± 13.57 olarak bulunmuştur. Erkeklerde kadınlara göre, ortalama oksalat ekskresyonu yüksek, ortalama sitrat ekskresyonu düşük olarak bulunmuştur. Hiperoksalüri ve hipositratüri sıklığı erkeklerde daha yüksektir, ancak aradaki fark istatistiksel olarak anlamlı bulunmamıştır. Hiperoksalüri ve hipositratüri kombine edildiğinde, sıklık, erkeklerde kadınlardan anlamlı olarak daha yüksek bulunmuştur. Taş analizine göre ayrılan 3 grup hasta arasında 24 saatlik idrarda oksalat ve sitrat ekskresyonu açısından anlamlı bir fark saptanmamıştır. Hiperoksalüri ve hipositratüri ürolityazisli hastalarda, kadın erkek her iki cins için önemli risk faktörleridir. Ancak bu faktörleri kombine ettiğimizde, erkekler kadınlara göre daha fazla riske sahiptir. Üriner sistem taş hastalığı rekürrens riski yüksek olan bir hastalıktır ve birçok faktör etyolojide rol almaktadır. Bu çalışmada taş analizi ile metabolik tanı arasında anlamlı bir ilişki bulunmasa da, bu konuda daha geniş çaplı başka araştırmalara ihtiyaç vardır. VII SUMMARY The main aim in this study was to determine the frequency of hyperoxaluria and hypocitraturia as risk factors in patients with urolithiasis and to compare levels of these analytes(oxalate and citrate) according to stone analysis. 137 patients who were diagnosed urinary tract Stones between December 2004 and May 2006 in Ondokuz Mayıs Üniversitesi, Medical Faculty, Department of Urology were included in the study. Oxalate and citrate levels were accounted from 24 hours urine samples. Stone analysis of 61 patients were determined and patients were divided to 3 groups according to stone analysis. We examined 86 males and 51 females with a mean age of 47.25 ± 13.5. Mean oxalate excretions were higher and mean citrate excretions were lower in the males than in the females. The frequency of hyperroxaluria and hypocitraturia were higher in the males, but the difference was not statisticaly significant. When hyperoxaluria and hypocitraturia combined, the frequency were significantly higher in the males than in the females. 24 hours oxalate and citrate excretions were not statisticaly significant between 3 groups of patients who were divided according to stone analysis.. I. Group; pure calcium oxalate stone formers, II. group; mixed calcium oxalate stone formers and III. group; patients who were formed stones except calcium oxalate. Hyperoxaluria and hypocitraturia are important risk factors in patients with urolithiasis for both the males and the females. But when we combined these factors, the males had more risk than the females. Urinary tract stone disease has higher recurrens risk and lots of etiologic factors effect to stone formation. Although in the study, there is no significant relationship between stone analysis and medical diagnosis, there is necessary to further investigations.
