Abstract
Maternal-fetal ortamın birçok faktör tarafından etkilenmesi sonucu düşük doğum ağırlıklı bebek gelişimi olmakla birlikte, bu durumu açıklayabilecek fetal, maternal, çevresel veya uterin faktörler ve mekanizmalar tam olarak açık değildir. Doğum ağırlığı, vücut ve organların büyüme kapasitelerinin değerlendirilmesinde neonatal karakteristik parametreler arasında en önemli olandır. DDA, olumsuz intrauterin ortamın bir göstergesidir. Erişkin hastalıkların temellerinin fetal hayatta atıldığı ve metabolik-endokrin bir programlanmanın anne karnında gerçekleştiği hipotezi (Fetal orijin hipotezi) ortaya atılmıştır. Bu hipoteze göre; düşük doğum ağırlıklı bebekler, hem erken neonatal dönemde hem de erişkin hayatta onları normal ağırlıkta doğan bebeklere göre dezavantajlı duruma getirecek birçok sorunla (kardiyovasküler hastalıklar, hipertansiyon, tip II DM vb.) karşılaşma riski altındadır. Bu programlanmada oksidatif stresin anahtar bir rolü olabileceği de savunulmaktadır. DDA ve erişkin hayatta gelişen metabolik sendrom, tip II DM ve kardiyovasküler hastalıklar arasındaki artmış ilişki oksidatif stres hipotezi ile açıklanabilecektir. Bu durumun açıklığa kavuşması için yapılan çalışmalarda, DDA bebeklerin genotoksisite açısından değerlendirilmemiş olmaları nedeniyle, hem DDA bebeklerin genotoksisite açısından varolan durumlarını hem de DNA tamir kapasitelerini Comet yöntemini kullanarak ölçmeyi düşünerek bu çalışmayı planladık. Çalışmamıza, Mart-2004 ve Ocak-2006 tarihleri arasında, Ondokuz Mayıs Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Anabilim Dalı Yenidoğan Servisine yatışı yapılmış 24 SGA ve kontrol grubu olarak ise gestasyonel yaş ve doğum ağırlığı bakımından SGA grubu ile eşleştirilmiş 22 SGA olmayan bebek dahil edildi. SGA ve kontrol grubundan alman kanlardan histopaque-1077 ile gradient farkına göre lenfosit izolasyonu yapıldı. Her infant için üç lam hazırlandı (background, H2O2 ve rejenerasyon için). Birinci lam, infantın var olan genotoksik değerlendirmesi VIIIiçin ayrıldı. Diğer iki lam, 5 dakika 75 uM H2O2 ile inkübe edildikten sonra, 5 dakika % l'lik DMSO ile muamele edilerek reaksiyon durduruldu. İkinci lam, F^CVnin yarattığı hasan değerlendirmek için ayrıldı. Üçüncü lam, DNA tamir işleminin gerçekleştirilebilmesi için RPMI-1640 solüsyonu içinde 20 dakika beklemeye bırakıldı. Sonunda, bu üç lamdaki hücreler Comet yöntemine tabi tutuldu. Elde edilen comet parametreleri genotoksisite ve DNA tamir kapasitesi yönünden istatistiksel açıdan değerlendirildi. SGA ve kontrol grubunda background ve rejenerasyonu yansıtan genotoksisite değerleri açısından fark bulunamadı (p>0.05). H2O2 ile inkübasyon sonrası genotoksisite değerleri diğer iki (background ve rejenerasyon) gruptan farklı idi (p<0.05). H2O2 grubu açısından SGA ve kontrol grubu arasında anlamlı fark olduğunu (p<0.05), kontrol grubunun F^CVden daha çok etkilendiğini tespit ettik. DNA tamir kapasitesi açısından SGA ve kontrol grupları `tail DNA %` ve `tail length` parametreleri karşılaştırılarak değerlendirildi. SGA ve kontrol grupları arasında DRC yönünden anlamlı farklılık bulunamadı (p>0.05). Bu çalışmada SGA ve kontrol grupları arasında genotoksisite ve DNA tamir kapasitesi açısından Comet yöntemi ile fark bulunamasa da, bu hipotezin açıklanmasına katkısı olacağım düşündüğümüz genotoksisite çalışmalarının sürdürülmesi gerekmektedir. Anahtar kelimeler: Düşük Doğum Ağırlığı, Genotoksisite, Comet, DNA Tamir Kapasitesi IX SUMMARY The development of low birth weight infant occurs as a result of being affected of maternal-fetal enviroment by many factors; but fetal, maternal, uterine or environmental factors which can explain this event are not clear. Birth weight is the most important factor among neonatal characteristic parameters on evaluating of growth capacity of body and organs. Low birth weight is an indicator of unfavourable intrauterine enviroment. Fetal origin hypothesis, which suggests that the basis of adult diseases appear in fetal life and a metabolic-endocrine programming occurs in intrauterine period, come out. According to this hypothesis low birth weight infants are under the risk of many problems (cardiovascular diseases, hypertension, type II DM) both in early neonatal period and adult life, if compared with the infants born in normal weight. It is suggested also that oxidative stress may have a key role in this programming. The increased relation between low birth weight and such diseases occured in adult life including metabolic syndrome, type II DM, cardiovascular diseases could be explained by means of oxidative stress hypothesis. In the studies until now, low birth weight infants were not evaluated according to genotoxicity for the explanation of this process. Because of this, we planed this study for the quantitation of background genotoxicity and DNA repair capacity of low birth weight infants using Comet assay. 24 SGA infant and 22 infant as a control group who were not SGA followed up in Newborn Intensive Care Unit of Pediatri Department of Ondokuz Mayıs University Faculty of Medicine were accepted to this study. Control group was matched with SGA group according to gestational age and birth weight. The lymphocytes were isolated from whole blood obtained from infants with histopaque-1077. Three slides were prepared for each infant. First slide was for the background genotoxicity of infant. Other slides were incubated with 75 uM H2O2 for 5 Xminutes, then the reaction was stopped with the treatment of DMSO %1 for 5 minutes. Second slide was for the evaluating the damage formed through H2O2. Third slide was waited in RPMI-1640 for 20 minutes for the process of DNA repair. Finally, the cells on the slides were subjected to Comet assay. Comet parameters were statistically evaluated for genotoxicity and DNA repair capacity at the end of the study. Genotoxicity values reflecting background and regeneration were not statistically different between SGA and control group (p>0.05). Genotoxicity values after H2O2 incubation were statistically different from other two (background and regeneration) groups (p<0.05). we found a significant difference between SGA and control group for the genotoxicity after H2O2 (p<0.05) and control group was more affected from H2O2. SGA and control groups were evaluated by comparison of `tail DNA %`and `tail length` for DNA repair capacity. There was not any significant difference between SGA and control groups for DNA repair capacity (p>0.05). In this study, we did not found any significant difference between SGA and control groups for genotoxicity and DNA repair capacity with Comet assay, but genotoxicity studies must be continued.
