Abstract
IV ÖZET Bilgisayar teknolojisindeki gelişmeler ve günümüzde bilgisayarlı tomografi (BT) sistemlerinde son gelinen nokta olan multidedektör bilgisayarlı tomografi (MDBT), ince ve spiral taranmış toraks kesitlerinden trakeobronşial yapıların sanal olarak fiberoptik bronkoskopi (FOB) dekine benzer endobronşial görüntülenmesine olanak sağlamıştır. Çalışmamızın amacı sanal bronkoskopinin teknik yöntemleri, avantajları, dezavantajları, sanal bronkoskopinin FOB ile karşılaştırmalı değerlendirilmesi, fiberoptik bronkoskopiye yapacağı katkıları ve trakeobronşial sistem hastalıklanndaki tanı değerini belirlemektir. Bu amaçla Atatürk Üniversitesi Tıp Fakültesi Radyodiagnostik Anabilim Dalı Multidedektör Bilgisayarlı Tomografi Ünitesinde Mart 2004 ve Kasım 2004 tarihleri arasında trekaobronşial sistem patoloji şüphesi bulunan 40 hastaya MDBT ile sanal bronkoskopik görüntüleme yapılmış ve elde edilen sonuçlar FOB bulguları ve histopatolojik inceleme sonuçları ile karşılaştırılmıştır. FOB incelemesi bulguları referans standart alınarak yapılan istatistiki analizde sanal bronkoskopinin sensitivitesi %92.5 ve spesifitesi %89.7 olarak tespit edilmiş olup hava yollarında distale gidildikçe sensitivite ve spesifite değerlerinin düştüğü gözlenmiştir. Trakeobronşial sistem patolojisi düşünülen olgularda konvansiyonel kesitler, multiplanar reformat (MPR) görüntüler ve sanal bronkoskopi görüntülerinin birlikte kullanılması radyologun daha doğru tam koymasını sağlar. Endobronşial lezyonlarm veya hava yolu stenozlanmn değerlendirilmesi, fiberoptik bronkoskopi ve beraberinde yapılan biyopsiye rehberlik, yabancı cisim aspirasyonlanmn ilk değerlendirilmesi, obstrüktif lezyonlann distalini göstermek ve postoperatif takip sanal bronkoskopinin başlıca uygulama alanlarını oluşturmaktadır. Biyopsi ve dinamik görüntüleme yapılamaması, mukozal detayı vermemesi gibi önemli dezavantajları nedeniyle bizde sanal bronkoskopinin, FOB'un hiçbir zaman tam bir alternatifi olamayacağım düşünmekteyiz. Anahtar Kelimeler; Trakeobronşial patoloji, Multidedektör Bilgisayarlı Tomografi, Sanal Bronkoskopi, Fiberoptik Bronkoskopi SUMMARY (Diagnostic value of multidetector computed tomography in tracheobronchial system diseases vitual bronchoscopy versus fiberoptic bronchoscopy) It is now possible to have sanal endobronchial imaging from tracheobronchial structures similar to that of fiberoptic bronchoscopy via thin or helically scanned thorax sections with the developments in computer and multidetector computed tomography technologies that are last reached points in computed tomography systems. The purpose of our study is to compare the technical methods, advantages and disadvantages of virtual bronchoscopy in comparison with fiberoptic bronchoscopy and to determine its contribution over fiberoptic bronchoscopy and diagnostic value in tracheobronchial system diseases. For this purpose, a total of 40 patients were evaluated who had the suspicions of tracheobronchial system pathology by virtual bronchoscopy at the multidedector computed tomography Unit, in the Department of Radiology, Faculty of Medicine, Atatürk University during the period of March 2004 and November 2004, The results were compared with histopathologic examination and fiberoptic bronchoscopy findings. In statistical analysis based on the findings of fiberoptic bronchoscopy detected 92.5% sensitivity and 89.7% specifity of virtual bronchoscopy and relatively decreas in the values of sensitivity and specifity through the more distal airways. The combined used of conventional sections, multpilanar reformatted images and virtual bronchoscopy in the tracheobronchial pathologies provides the radiologist to get a more precise diagnosis. Virtual bronchoscopy has an important role in the evaluation of endobrochial lesions and airway stenosis, for the guidance of biopsies by fiberoptic bronchoscopy, in the first evaluation of foreign body aspiration, to show the more distal part of the obstruction and in the postoperative follow. We thought that virtual bronchoscopy is never completely an alternative method to fiberoptic bronchoscopy with the disadvantages of not to perform dynamic imaging, biopsy and insufficiency in the detection of mucosal detail.
