Kapama tedavisine cevap alınamayan ambliyopi hastalarında cam tedavisi uygulama sonuçları

Abstract
AMAÇ: Ambliyopi tedavisinde kapama tedavisi uygulanmış fakat takiplerinde görme artışı olmayan ve kapama tedavisine uyum sağlayamayan hastalarda CAM tedavisinin etkinliğini araştırmak. YÖNTEM: Ağustos 2003 - Nisan 2006 tarihleri arasında Ondokuz Mayıs Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı'na başvuran daha önceden ambliyopi tanısı konmuş ve kapama tedavisi uygulanmış fakat en az 4 aylık takiplerinde görme artışı olmayan ve kapama tedavisine uyum sağlayamayan 29 hastanın 29 gözü çalışmaya alındı. Hastalar yaşlarına göre, ambliyopi tiplerine göre ve CAM öncesi görme düzeylerine göre gruplara ayrıldı. Tedavide üst üste olarak 6 gün CAM tedavisi uygulandı. CAM tedavisi sonrası hastalara tekrar kapama tedavisi uygulandı ve görme düzeylerindeki değişikliklerin istatiksel analizi yapıldı. BULGULAR: Çalışmamızda CAM tedavisi ile 29 ambliyop gözün 1 1 inde ( % 37,93) en az 2 sıra, 23 tanesinde ( % 79,3 1) en az 1 sıra artış oldu. Yaş grupları arasında ve tam grupları arasında CAM tedavisi ile elde edilen görme keskinliği artışı bakımdan istatiksel fark yoktu. SONUÇ: 6 gün üst üste CAM tedavisi uygulanması ile ambliyop gözlerde görme keskinliği ile kısa sürede elde edilen artışın daha sonra yapılacak kapama tedavisine uyumu arttıncı bir faktör olabileceğini düşündürmektedir. Anahtar Kelimeler: Amliyopi, CAM tedavisi, hasta uyumu Purpose: To evaluate the effectiveness of CAM therapy for the treatment of amblyopia in the patients, who have no visual improvement with the occlusion therapy and are not compliant with patching. Material and Methods: Twenty-nine eyes of 29 patients that were referred to Ophthalmology Department of Ondokuz Mayis University Faculty of Medicine between August 2003 and April 2006 are included in the study. Although they had been previously diagnosed and treated with the occlusion therapy, during at least 4 months follow up time they had no visual improvement and were reluctant with further patching. The patients were grouped according to age, type of amblyopia and the visual acuity level before CAM therapy. CAM therapy was applied for 6 consecutive days to all of the patients. After CAM therapy, all eyes were occluded again and the differences between the initial and the post-therapic visual acuity of the patients were analyzed statistically. Results: Visual acuity improved for at least 2 lines in 1 1 of 29 amblyopic patients (37.93 %) and for at least 1 line in 23 of 29 (79.31 %). There is no statistically significant difference between age groups and types of amblyopia in terms of visual acuity improvement by using CAM therapy. Conclusion: It is thought that the improvement of visual acuity by using CAM therapy for 6 consecutive days may improve the compliance of the patients for further occlusion therapy.
