Tüm düzeltme ameliyatı yapılan fallot tetralojili hastalarda ritim ve iletim bozukluklarının incelenmesi

Abstract
Çalışmamızı, tüm düzeltme ameliyatı yapılmış Fallot tetraloji'li hastalarda ritim ve iletim bozuklukları sıklığının ve ilişkili olabilecek faktörlerin EKG, Holter monitörizasyon ve egzersiz testi ile belirlenmesi amacıyla planladık. Çalışmaya, Fallot tetralojisi nedeniyle ameliyat edilmiş yaş ortalaması 11,04 ± 4,54 yıl, ortalama ameliyat yaşı 4,53 ± 3,13 yıl ve izlem süresi ortalaması 6,5 ± 3,8 yıl olan, 17'si kız ve 12'si erkek toplam 29 hasta (Grup I) alındı. Kontrol grubunu ise benzer demografik özellikte 13'ü kız, 20'si erkek 33 çocuk (Grup II) oluşturdu. Ayrıca, dosya kayıtlarında ameliyat öncesi ve sonrasında EKG kaydı bulunan 41 hasta ritim bozukluğu açısından karşılaştırıldı. EKG kayıtlarında Grup I'de %79,3 komplet sağ dal bloğu (RBBB), Grup H'de %6,1 inkomplet RBBB tespit edildi. Grup I'de %3,4 VPK ve %6,9 sol anterior hemiblok ile uzun QT'nin RBBB'ye eşlik ettiği görüldü. Grup Fin QRS, PR süresi ve QTc ortalama değerleri Grup II' den anlamlı derecede uzun tespit edildi. Holter kayıtlarında Grup I 'de 18 hastada, Grup İP de 17 hastada ritim bozukluğu olduğu ancak Grup I'de çoğunlukla ventriküler prematür kontraksiyon (VPK) ve sık supraventriküler ektopik atımlar (SVE) görülürken; Grup Il'de VPK dört hastada bulunmuş, saptanan ritim bozukluğunun çoğunu seyrek SVE oluşturmuştur. Egzersiz testinde Grup I'de iki hastada sık ventriküler prematür kontraksiyonlar tespit edildi. Grup Il'de ritim bozukluğu gözlenmedi. Grup Fin egzersiz süresi anlamlı derecede kısa bulundu. Grup I'de %79,3 pulmoner yetersizlik (PY), %58,6 pulmoner stenoz (PS), %20,7 VSD ve %13,8 aort yetersizliği saptandı. PS bulunan tüm olgularda PY de mevcut idi. Hastaların PS gradiyent ortalaması 19,1 ± 9,3 mmHg, PY hız ortalaması ise 1,8 ±0,2 m/sn idi. İzlem süresi, ameliyat yaşı ve şekli ile ritim bozukluğu arasında ilişki saptanmadı. Rezidüel PS ile ritim bozukluğu arasında pozitif ilişki bulundu. PS ve PY birlikteliğinin Holter kayıtlarmdaki ritim bozukluğuna anlamlı katkısı olduğu görüldü. QRS-PR ve QTc sürelerinin Holter' de ritim bozukluğu varlığı ile ilişkisi gösterilemedi. Ameliyat öncesi ve sonrası çekilen EKG'leri karşılaştırılan hastalarda ritim bozukluğunda anlamlı artış saptandı ancak ameliyat şekli ile ilişki gösterilemedi. ıxSonuç olarak çalışmamızda EKG'de saptanan VPK sıklığı literatür verilerinden daha seyrek, Holter'de saptanan ritim bozukluğu sıklığı ise literatür ile uyumlu bulunmuştur. Ritim ve iletim bozukluğunun EKG ile saptanması her zaman mümkün olmamaktadır ve 24 saatlik Holter monitörizasyon ritim bozukluğunun saptanmasında daha etkin bir yöntemdir. Tüm düzeltme uygulanmış FT'li hastaların, fizik muayene ve ekokardiyografinin yam sıra ritim ve iletim bozuklukları yönünden de yakın izlemlerinin önemli olduğu; EKG'nin yam sıra özellikle Holter monitörizasyon ve egzersiz testinin de uygulanmasının yararlı olacağı görüşündeyiz. The aim of our study was to determine the frequency of rhythm and conduction disturbances in patients with tetralogy of Fallot who underwent total correction; and to evaluate the effect of various factors on ECG, Holter monitorization and exercise test. 17 girls and 12 boys operated for tetralogy of Fallot were included in the study (Group I). Their mean age was 11.04+-4.54, mean age at which they underwent operation was 4.53+-3.13 and mean duration of follow up was 6.5+-3.8 years. Control group comprised of 33 children (13 girls-20 boys) with similar demographic features.(Group II). In addition, ECG records of 41 patients before and after operation were compared in terms of rhythm disturbances. In group I, complete right bundle brunch block (RBBB) was detected on ECG records at the rate of 79.3% and in group II, incomplete RBBB was detected at the rate of 6.1%. In group I, RBBB was also accompanied by VPC (3.4%), left anterior hemiblock (6.9%) and long QT. QRS, PR and QTc intervals were significantly longer in group I than in group II. In Holter records, 18 patients in group I and 17 patients in group II had rhythm disturbances. However, the problem was mostly ventricular premature contraction (VPC) and frequent supraventricular ectopic beats (SVE) in group I, whereas in group II; VPC was present in four patients, SVE being the most frequent problem. In group I, frequent ventricular premature contractions were detected in two patients during exercise test, while in group II, VPC wasn't observed. In group I, exercise duration was found to be significantly shorter than the other group. In group I, pulmonary insufficiency (PI) and pulmonary stenosis (PS) were found at the rate of 79.3% and 58.6% respectively, VSD at the rate of 20.7% and aort deficiency at 13.8%. In all cases with PS; PI was also present. Mean PS gradient was 19.1 +-9.3 mmHg and mean velocity of PI was 1.8+-02 m/sec. No relation has been found between the duration of follow up, age and type of surgery and rhythm disturbances. A positive relation was found between residual PS and rhythm disturbances. It was observed that coexistence of PS and PI contributed significantly to the rhythm disturbances in Holter records. There was no relation between QRS-PR and QTc intervals and the presence of rhythm problems in Holter monitoring. In the ECGs before and after surgical correction, a significant rise in rhythm disturbances was established, but no relation with operation technique could be demonstrated. In conclusion, the frequency of VPC detected by ECG has been found to be lower than that in the literature and the frequency of rhythm disturbances detected with Holter monitoring has been found to be consistent with that in the literature. It is not always possible to detect rhythm and conduction disturbances with ECG alone and 24 hour Holter monitorization is an important method for evaluating the rhythm disturbances. It is our suggestion that in patients with tetralogy of Fallot who underwent total correction, close monitorization for rhythm and conduction disturbances is also important as well as physical examination and ECG on fallow-up. For this purpose, Holter monitorization and exercise tests should also be performed in addition to ECG on fallow-up of these patients.
