Ratlarda taurolidin ve ginkgo biloba ekstresinin kolon anastomozu patlama basıncı üzerine etkileri

Abstract
Amaç: Gastrointestinal sistem cerrahisi sonrası gelişen mortalite ve morbiditeden sorumlu faktörler içerisinde, anastomoz kaçakları halen ilk sıradaki yerini korumaktadır. Anastomoz iyileşmesi sürecinin başarılı bir şekilde tamamlanabilmesi için pek çok faktörün birlikte sağlanması gerekmektedir. Anastomozun kanlanması ve oksijenasyonu bunların en önemlileridir. Anastomoz iyileşmesi sürecinde oluşan medyatör ve sitokinlerin yara iyileşmesi üzerindeki olumsuz etkileri bir çok araştırmaya hala konu olmaktadır. Kolon temizliği ve hazırlığı yapılmadan yapılan anastomozlarda, lokal kontaminasyon durumunda inflamasyon olması ve diseksiyon sırasında iskemi oluşması ve anastomoz sonrası dolaşım ve kanlanmanın önemi, iyileşme sürecinde oluşan Platellet aktive edici faktör ve diğer zararlı medyatör ve sitokinlerin zararlı etkilerine karşı; Ginkgo Biloba Ekstresi olan EGB 761 ve Taurolidin'in iyileşme süreci üzerinde olumlu etkileri olabileceği düşünülerek bu çalışma planlanmıştır.Materyal ve metod: Barsak temizliği vermeden ve oluşacak lokal kontaminasyona aldırmadan sol kolonda diseksiyon yapıp rezeksiyon ve primer anastomoz uyguladığımız onar rattan oluşan 3 grup oluşturuldu. 1.Grup kontrol grubu olup, anastomoz öncesinde ve sonrasında intraperitoneal SF ile irrigasyon yapıldı. 2.Grup, Ginkgo Biloba Ekstresi grubu olup aynı şekilde anastomoz öncesi ve sonrası Ginkgo Biloba Ekstresi solusyonu ile irigasyon yapıldı. 3.Grup Taurolidin grubu olup aynı işlemler yapılıp taurolidin solusyonu ile irigasyon yapıldı. Bütün gruplar post-op. 7. Gün sakrifiye edilerek barsak patlama basıncı, anastomoz hattında hidroksiprolin düzeyi ve histopatolojik inceleme yapılıp ayrıca yapışıklık incelendi.Bulgular: 1.Grubun ortalama patlama basıncı 104,60 mmHg, 2.ve 3. Grubun ortalama patlama basınçları sırası ile 122,30 ve 168,10 mmHg ölçüldü. Doku hidroksiprolin değerleri ise sırası ile ortalama 0,25350-0,37620 ve 0,55410 ? g/mg. Ölçüldü. Bu durumda taurolidin grubunun her iki gruptan, ginkgo biloba grubunun da kontrol grubundan anlamlı derecede yara iyileşmesi üzerinde olumlu etkiler elde edilmiştir (p ? 0.01). Ayrıca histopatolojik inceleme ile anastomoz iyileşme skoru istatiksel olarak taurolidin grubunun diğer gruplardan anlamlı derecede yüksek bulunmuştur (p ? 0.01). Ayrıca ginkgo biloba ekstresin kontrol grubuna göre daha fazla yapışıklığa neden olduğu taurolidin grubunun ise daha az yapışıklık yaptığı gördük.Sonuç: Sol kolon anastomozu sonrası oluşan anastomoz iyileşmesini olumsuz etkileyen faktörlerin tedrici olarak taurolidin ve ginkgo biloba ile nötralize edilebildiğine dair bulgular elde edilmiştir. Objective: Among factors which develop after gastrointestinal surgery and are responsible from mortality and morbidity, anastomosis leakages are still in the first place. Many factors should be provided together to accomplish anastomosis healing process succesfully. Most important of these are circulation and oxigenisation of anastomosis. Negative effects of mediators and cytokines, which occur within the anastomosis healing process, on scar healing are still topics of many investigations. This study is planned with the thought that in anastomosis without cleaning and preparation of colon, EGB 761, which is an extract of ginkgo biloba, and taurolidine may have positive effects on healing process, on importance of circulation after anastomosis, on the presence of inflammation in case of local contamination and occurence of ischemia during dissection and may have effects against harmful effects of platelet activating factor (PAF) and other mediators or cytokines which occur in healing process.Materials and method: We made a dissection on left colon without decontaminating colons and concerning about local contamination in rats which are divided into three groups, there were ten rats in each group. 1st group was the control group, an intraperitoneal irrigation was made with SF (saline) before and after anastomosis. 2st group was Ginkgo Biloba Extract group, before and after anastomosis, irrigation was made with ginkgo biloba extract solution. 3th group was taurolidine group, intraperitoneal irrigation was made with taurolidine solution. All rats in all groups were sacrificed in 7th day after the operation; intestinal explosion pressures, hydroxiproline levels on anastomosis line as well as adhesions were examined and histopathologic examinations are made.Results : According to measurements of mean explosion pressures, mean explosion pressure was 104.60 mmHg in the first group, and 122.30 and 168.10 mmHg in the second and third group, respectively. Tissue hydroxiproline levels are 0,25350-0,37620 and 0,55410 ? g/mg in the first, second and third group, respectively. So, there were significantly more positive effects on scar healing in taurolidine group than in both ginkgo biloba and control groups, and significantly more positive effects are observed in ginkgo biloba group than in the control group (p<0.01). Furthermore, histopathologic examination and anastomosis healing score was found to be statistically higher in taurolidine group than in other groups (p<0.01). Moreover, we saw that ginkgo biloba extract causes more adhesions than control group and taurolidine causes less adhesions than other groups.Conclusions: We found that taurolidine and ginkgo biloba can interfere the negative factors which effect anastomosis healing after left colon anastomosis, taurolidine was superior to ginkgo biloba.
