Liqui preptm sıvı bazlı sitoloji yönteminin tekrarlanabilirliğinin değerlendirilmesi ve pap smear ile karşılatırılması

Abstract
ÖZET Amaç : Bu prospektif çalışmanın amacı konvansiyonel Pap smear ile Liqui PrepT sıvı bazlı sitoloji yönteminin servikal epitelyal hücre anomalilerini saptamadaki performanslarım karşılaştırmak ve Liqui PrepT yönteminin tekrarlanabilirliğini değerlendirmektir. Gereç ve yöntem : Ondokuz Mayıs Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalında 2006-2007 yıllan arasında prospektif olarak toplanan 508 hastaya ait servikovajinal yayma çalışmaya dahil edildi. Konvansiyonel Pap smear preparatlan hazırlandıktan sonra bu yaymaların alındığı fırçalar etiketli Liqui PrepT numune toplama kabı içine konularak Patoloji Anabilim Dalma gönderildi. Liqui PrepT yönteminin belirttiği protokole uygun olarak preparatlar hazırlandı. Konvansiyonel PAP smear preparatlan ve Liqui PrepT preparatlan 2001 Bethesda Sistemine göre kör olarak değerlendirilip sonuçlar karşılaştınldı. Liqui PrepT yönteminin tekrarlanabilirliğinin değerlendirilmesi amacı ile numune kabındaki materyalden aynı lama iki ayrı inceleme alanı hazırlandı ve bu iki alanın sonuçları da karşılaştırıldı. Bethesda 2001 sınıflamasına göre tam verilen, konvansiyonel Pap smear ve Liqui PrepT yöntemi ile aynı preparat üzerinde oluşturulan iki alanın değerlendirildiği 424 olgunun sitolojik tanılan karşılaştınldı. Sitolojik tanıda 7 temel Bethesda sistemi kategorisi kullanıldı. Bulgular: 508 olgudan 483 tanesi (%95.1) konvansiyonel Pap smear yöntemi ile değerlendirme için yeterli iken, Liqui PrepT yöntemi ile oluşturulan birinci inceleme alanında 434 olgu (%85.4), aynı yöntem ile oluşturulan ikinci inceleme alanında 428 olgu (%84.3) olgu değerlendirme için yeterli idi. Konvansiyonel Pap smear yöntemi ve Liqui PrepT yöntemi ile oluşturulan her iki inceleme alanının en az birinde değerlendirme için yeterli hücre içermeyen 84 olgu istatistiksel değerlendirmeye alınmadı. Olgulardan 14' ü (% 3.3) Konvansiyonel Pap smear ile önemi bilinmeyen atipik skuamöz hücreler tanısı tanısı alırken, Liqui PrepT yöntemi ile 6 olgu ( % 1.4) önemi bilinmeyen atipik skuamöz hücreler tanısı aldı. Olgulardan 1' i (%0.2) Pap smear yöntemi ile düşük dereceli skuamöz intraepitelyal lezyon tanısı alırken, 3 tanesi (% 0.7) VILiqui PrepT yöntemi ile düşük dereceli skuamöz intraepitelyal lezyon tanısı aldı. 2 olgu Liqui PrepT yöntemi ile yüksek dereceli skuamöz intraepitelyal lezyon tanısı alırken (% 0.5), Pap smear ile 1 olgu yüksek dereceli skuamöz intraepitelyal lezyon tanısı aldı (% 0.2). Liqui PrepT sıvı bazlı ince tabaka sitolojisi yöntemi ile aynı preparatta oluşturulan iki ayrı halkanın sitolojik tanıları arasında tam bir uyum gözlendi. Bu iki inceleme alanının sitolojik tanıları arasında istatistiksel olarak anlamlı bir fark gözlenmedi (p>0.05). Sonuçlar: Liqui PrepT sıvı bazlı ince tabaka sitolojisi yöntemi ile konvansiyonel Pap smear yöntemlerinin sitolojik tanılan açısından karşılaştırılmasında istatistiksel olarak anlamlı bir fark bulunmuştur. Konvansiyonel Pap smear yöntemine karşılık Liqui PrepT yöntemi ile verilen önemi bilinmeyen atipik skuamöz hücreler tanısında azalma olduğu bulunmuştur. Liqui PrepT yöntemi ile konvansiyonel Pap smear yöntemine göre düşük dereceli skuamöz intraepitelyal lezyon ve yüksek dereceli skuamöz intraepitelyal lezyon tanılarında artış gözlenmiştir. Liqui PrepT yöntemi ile aynı preparat üzerinde hazırlanan iki inceleme alanının sitolojik tanılarının istatistiksel değerlendirilmesinde tam bir uyum bulunmuştur. VII ABSTRACT Objective: The aim of this prospective study are to compare the performance of Liqui PrepT is a next generation of liquid based cytology preparation technique and conventional Pap smear in detecting cervical epithelial cell abnormalities and to evaluate repeatability of Liqui PrepT method. Materials and Methods: In this prospective study 508 cervicovaginal smear were collected from who have defining criterions 508 patients between 2006-2007 in Department of Obstetrics and Gynecology, Faculty of Medicine, Ondokuz Mayıs University. After making the conventional Pap smear slides, clinicians placed the collection device with residual cells in Liqui PrepT preservative fluids and sent to Department of Pathology. The slides were prepared according to the protocol of Liqui PrepT. The conventional Pap test and Liqui PrepT slides were evaluated blindly and were compared their results according to 2001 Bethesda System. To purpose evaluating repeatability of Liqui PrepT method were prepared two examination fields on the same slide and were compared results of these fields. Diagnostic results of 424 slides which belongs to conventional Pap test and two examination fields of Liqui PrepT were compared according to 2001 Bethesda System. Seven basic Bethesda System Categories were used for cytologic diagnosis. Results: 483 of 508 (%95.1) cases were satisfactory for evaluation on conventional Pap smear. 434 cases (%85.4) on the first examination field of Liqui PrepT were satisfactory for evaluation and 428 cases (%84.3) on the second examination field of Liqui PrepT were satisfactory for evaluation. 84 cases which are unsatisfactory for evaluation with conventional Pap smear and Liqui PrepT were not evaluate statistically. 14 of 424 cases (%3.3) were interpreted as atypical squamous cells of undetermined significance on conventional Pap smear but 6 cases (%1.4) were interpreted as atypical squamous cells of undetermined significance on Liqui PrepT slides. İn spite of one of the cases (%0.2) was interpreted as low grade squamous intraepithelial lesions on conventional Pap smear, 3 cases (% 0.7) were interpreted as low grade squamous intraepithelial lesions on Liqui PrepT slides. While 2 of the cases (% 0.5) were interpreted as high grade squamous intraepithelial lesions on Liqui PrepT, 1 case (%0.2) was interpreted as high grade squamous intraepithelial VIIIlesions on conventional Pap smear. There were no statistically differences between two examination fields on the same slides of Liqui PrepT is a liquid based cytology preparation technique. Conclusions: There were statistically meaningful differences comparisons of the cytologic diagnosis the conventional Pap smear and Liqui PrepT slides. Compared with conventional Pap smear, cytologic diagnosis of the atypical squamous cells of undetermined significance were decreased on Liqui PrepT slides. When compared to the conventional Pap smear the Liqui PrepT increased detection of high grade squamous intraepithelial lesions and low grade squamous intraepithelial lesions. Statistically, there were perfect consistency compared with cytologic diagnosis of two examination fields on the same slides of Liqui PrepT.
