İntravenöz deksmedetomidinin, entübasyon ve ekstübasyona hemodinamik yanıt ve postoperatif analjezik tüketimi üzerine etkisinin araştırılması

Abstract
ÖZET Amaç: İntravenöz deksmedetomidinin entübasyon ve ekstübasyona hemodinamik yanıt ve Hasta Kontrollü Analjezi (HKA) izlemi ile postoperatif 24 saatte tüketilen toplam analjezik miktarına etkisini araştırmaktır. Gereç ve Yöntem: Genel anestezi altında elektif abdominal cerrahi planlanan (bir saatten kısa olmayan, histerektomi ve miyomektomi) 20-60 yaşlan arasında, ASA I-II grubu 50 kadm hasta çalışmaya dahil edildi. Hastalar rastgele iki gruba ayrıldı ve deksmedetomidin grubu (grup D, n=25) ve kontrol grubu (grup P, n=25) olarak adlandırıldı. Deksmedetomidin grubuna (grup D) indüksiyondan önce yükleme dozu olarak 0.5 ug/kg deksmedetomidin intravenöz 10 dakikada verildi. İndüksiyonda iv tiyopental verilirken, 0.5 ug/kg/h deksmedetomidin infüzyonu başlanarak cerrahi cilt sütürasyonu bitimine kadar verildi. Kontrol gubuna (grup P) grup D'deki ilaç hacmine eşit %0.9 serum fizyolojik infüzyonu cerrahi cilt sütürasyonu bitimine kadar verildi. Anestezi imdüksiyonu kirpik refleksi kaybolana kadar tiyopental, 1 ug/kg fentanil ve nöromusküler blokaj 0.1mg/kg vekuronyum ile sağlandı. Anestezi idamesine %66 azot protoksit ve %33 oksijen karışımı içinde %2 sevofluran ve 0.02 mg/ kg vekuronyum ile devam edildi. Sıvı idamesi 15ml/kg %0.9 NaCl ile, kan kayıpları kaybedilen miktarın üç katı %0.9 NaCl ile karşılşandı. Entübasyondan sonra ve intraoperatif 5., 10., 20., 30., 40., 60., 90. ve 120. dakikalarda ve ekstübasyondan 10 dakika önce, ekstübasyonda, ekstübasyondan 10 dakika sonra kalp hızı, ortalama arter basmcı, periferik oksijen satürasyonu kaydedildi. Cerrahi sonunda kas gevşetici etkisini antagonize etmek için neostigmin (0.03mg/kg) ve atropin (0.015mg/kg) yapıldı. Ekstübasyondan önce 20mg petidin intravenöz uygulandı. Ekstübasyon süresi anestezik ilaçların kesilmesinden ekstübasyona kadar olan süre, uyanma süresi ise anestezik ilaçların kesilmesinden sözlü uyanlara verbal ve motor yanıt alınıncaya kadar geçen süre olarak tanımlandı ve kaydedildi. Hastalar ayılma odasmda ilk bir saat 10., 30. ve 60. dakikalarda ve serviste 2., 6., 12. ve 24. saatlerde kalp hızı, sistolik ve diastolik kan basmcı, ortalama kan basmcı, bulantı-kusma, kaşmtı, titreme ve ağrı düzeyleri kaydedildi. Postoperatif tüm olgulara petidin 2mg/ml ile hazırlanmış Hasta Kontrollü Analjezi (HKA) ile i.v analjezi uygulandı. Hastalara uygulanan toplam analjezik miktan, ilk analjezik istek zamanı ve analjezik istek sayılan HKA cihazının hafıza bilgilerinden kaydedildi. rvBulgular: Gruplar arasında demografik özellikler, anestezi, cerrahi, ekstübasyon ve uyanma süreleri yönünden anlamlı fark yoktu(p>0.05). Her iki grup arasında intraoperatif ve post operatif kalp atım hızlan karşılaştınldığnda kontrol grubunda daha yüksekti (p<0.05). Kontrol grubunun entübasyon ve 90. dakikadaki ortalama arter basmcı (OAB) değerleri deksmedetomidin grubundan daha yüksekti (p<0.05). Postoperatif OAB iki grup arasında anlamlı fark yoktu (p>0.05). Gruplar arasında tüketilen petidin, tiopental miktarı, ilk analjezik istek zamanı ve toplam analjezik istek sayıları deksmedetomidin grubunda anlamlı olarak düşük bulundu(p<0.05). Sonuç: Anestezi indüksiyonundan 10 dakika önce verilen (0.5 ug/kg) deksmedetomidin bolus dozu takiben cerrahi süresince 0.5 [ig/kg/h intravenöz infüzyonu ile yan etki görülmeksizin entübasyon, İntraoperatif ve ekstübasyonda anestezi ve cerrahiye karşı gelişen hemodinamik yanıt baskılanmaktadır. îndüksiyonda kullanılan tiyopental rniktarınıda azaltmaktadır. Postoperatif tüketilen petidin miktarında azalma olması, deksmedetomidinin analjezik etkinliğini düşündürmektedir. Ağrılı major cerrahi hastalarda deksmedetomidinin intravenöz premedikasyon amacıyla kullanıldığında ekstübasyon ve uyanma zamanını etkilemeksizin kullanılabileceğini düşünmekteyiz. Abstract Objective: In this study we aimed to investigate the hemodynamic answer to iv dexmedetomidine, premedication during entubation and extubation and to the amount of total analgesic agents consumed in the patient's body postoperatively, in 24 hours by using PCA(Patient Control Analgesia) system. Materials and MethodsrThe study were executed with the approval and with the consent of 50 subjects who were scheduled for an elective abdominal surgery which lasts less than 1 hour, under genereal anesthesia. They were between 20 and 60 years old age and participated in groups I - II of American Society of Anesthesiology. Subjects were then reorganized randomly to form two groups, each with 25 persons and named as: Dexsmedetomidine group (group D, n=25) and control group (group P, n=25). Before induction; in Group D (dexmedetomidine group) 25 patients received 0,5 (ig/kg dexmedetomidine hydrochloride lasting for 10 minutes.Following induction, 0,5 p.g/kg/min dexmedetomidine was applied until the end of surgery till the end of the suturation of the skin.In Group P (control group) 25 patients received % 0,9 physiological serum as the same volume of medication used for group D and for the same treatment durations as in Group D. Anesthesia was induced using fentanyl 1 mcg/kg and thyopental till the tear reflex is lost. Vecuronium was given at the amounts of 0.1 mg/kg during induction to achieve neuromuscular blockage and for maintenance 0.2mg/kg was given respectively. For maintenance of anesthesia; sevofloran at the concentration 2% in a mixture of 66% nitrogen protoxide and 33% oxygen. Liquid maintenance was supplied with 15 ml/kg NaCl %0.9 and the amount of lost blood was maintaned with the phisological serum, with a threefold of the lost amount After entubation,and intraoperatively at the 5th, 10th, 20th, 30th, 40th, 60th, 90th, 120th minutes and 10 minutes before,during and 10 minutes after extubation, heart rate, avarage arterial pressure and peripheric oxygen saturations were recorded. At the end of the surgery; neostigmine 0.03 mg/kg and atropine 0.015 mg/kg were used to antagonize the neuromuscular blockage. Before extubation 20mg petidyn was given iv. The extubation time was defined and measured as duration between at the VItime of stopping the anesthesic drugs till extubation, and awakening time defined and recorded as duration from stopping the anesthesic drugs till the patient's verbal and motor answer achieved to`open your eyes` remote. In the recovery room in the first hour, at the Oth, 10th, 30th, 60th minutes and at the department at the 2nd, 6th, 12th, 24th hours, the patients were monitored for determining their heart rates, systolic and diastolic blood pressures,avarage blood pressures,nausea and vomittmg,itching,shivering and pain levels were recorded. PCA device prepared at 2 mg/ml petidine concentration was used for i.v analgesia to all cases postoperatively. The total amount of analgesia applied to patients,the first time they needed analgesia and the number of analgesia they used, was assased by using the recorded memory information of the PCA device. There were no significant difference between groups according to demographic features, anesthesia, surgery, extubation and awakening times (p>0.05). When the two groups are compared for intraoperative and postoperative heart rates; heart rates were higher in the control group(p<0.05). In the control group; average arterial blood pressure values at the time of entubation and at the 90th minute intraoperatively were higher than the dexmedetomidine group(p<0.05). Postoperatively there were no significant difference between groups according to average arterial blood pressure values(p>0.05). The amount of used petidine and thiopental,the first time analgesia needed and the total number of analgesia used was found significantly lower in the dexmedetomidine group(p<0.05). Result: When dexmedetomidine was given 10 minutes before anesthesia induction with a 0.5 ug/kg bolus dose and following this dose when it was also given with a dose of 0,5 ug/kg/h during surgery;without any side effects;it was proved that hemodynamic answer to both anesthesia and surgery was declined.The amount of used thiopental during induction is also decreased.Due to decriment of postoperatively used petidine amount, it was also thought that dexmedetomidine has some analgesic effects.In patients being done a major surgical operation and suffering from pain when dexmedetomidine was applied for iv premedication; we are thinking of that it may be used without any effect on extubation and awekening time periods.
