Abstract
GİRİŞ VE AMAÇKanserli hastalarda tromboza, sağlıklı kişilere göre daha sık rastlanır. Her kanser türünde eşit sıklıkla bulunmaz. Pankreas, over, beyin, böbrek tümörlerinde daha sıktır. Kanserin evresi, tedavisi ve tümör cerrahisi riski artıran faktörlerdir. Bu çalışmanın amacı kanser hastalarında tromboz sıklığını, ne zaman ortaya çıktığını ve hastanın aldığı tedavilerin tromboz gelişimi üzerine olan etkilerini araştırmaktır.GEREÇ VE YÖNTEMHaziran 2007 ile Mayıs 2008 tarihleri arasında Tıbbı Onkoloji polikliniğine başvuran kanser tanısı almış toplam 1003 hasta çalışmaya alındı. Dosyalar retrospektif olarak incelendi. Hastalar yaş, cinsiyet, tanı, evre, tanı anında uzak organ metastazı, takiplerinde tromboz gelişip gelişmediği, cerrahi öyküsü, radyoterapi ve kemoterapi açısından değerlendirildi. Hem semptomatik, hem de asemptomatik trombozu olan hastalar çalışmaya alındı. Tespit edilen trombozlar görüntüleme yöntemleri ile gösterilmişti. İstatiksel analizler Ki Kare testi ile yapıldı. İstatistik anlamlılık değeri p<0.05 olarak kabul edildi.SONUÇLAR1003 hastanın 29'unda (%2,9) tanıdan sonraki ilk 6 aylık dönemde tromboz saptandı. En sık insidans; böbrek, pankreas, endometrium, baş-boyun ve mide kanserlerinde idi. Tanı anında uzak organ metastazının olması, riski artıran en önemli etken olarak bulundu (p<0.001). Kemoterapi, radyoterapi almayan ve tümör cerrahisi geçirmeyen hastalarda tromboz daha sıktı. Bu bulgumuz mevcut çalışmalarla uyuşmuyordu. Tromboz, tanıdan sonraki ilk aylarda daha sık olarak görüldü (p<0.05). Hastalarımızda en sık alt ekstremite derin ven trombozu, ikinci sıklıkla pulmoner emboli vardı. BACKGROUND AND PURPOSEPatients with cancer have an increased risk of thrombosis compared with healthy people. This increased risk varies between different types of cancer. Tumors of the pancreas, ovarian, brain and kidney have the strongest association with thrombosis. It is also associated with a number of risk factors, stage of cancer, surgery and systemic therapies. The aim of this study is to determine the incidence, time-course and risk factors associated with the development of thrombosis in cancer patients.MATERİALS AND METHODSOne thousand three cancer patients who were referred to Medical Oncology outpatient clinic between June 2007 and May 2008 were included in the study. The files were examined retrospectively. Patients were evaluated according to age, sex, stage and type of cancer, distant organ metastasis during the diagnosis, thrombosis development during follow-up, history of surgery and treatment modalities (chemotherapy-radiotherapy). Symptomatic or asymptomatic patients with thrombosis were included in the study. The development of thrombosis was demonstrated with imaging methods. For statistical analyses Ki-square test was used. Statistical significance value accepted was p<0.05.RESULTSAmong 1003 patients; thrombosis was determined in 29 (2,9%) patients in the first six month after the diagnosis. Commonly; it was detected in renal, pancreas, endometrium, head/neck and gastric cancers, respectively. The presence of distant organ metastasis during the diagnosis was found as the most important etiologic factor for thrombosis (p<0.001). It was more frequent in the patients who didn?t have chemotherapy and radiotherapy and also who didn?t undergo tumor surgery. This finding was not correlated with the literature. We determined that thrombosis was commonly found in the first couple of months just after the diagnosis (p<0.05). In our patients; the most frequent type was deep venous thrombosis in the lower part of the extremity and secondly pulmonary embolism.
