Abstract
Amaç: Panik bozukluğu olan bir grup hastada panik atağı belirtilerinin dağılımını araştırmak, panik bozukluğunun olası alt tiplerini ve bu alt tiplerle ilişkili klinik özellikleri saptamak, alt tiplemeyi belirleyen etkenleri araştırmak amaçlanmıştırGereç ve yöntemler: Çalışmaya Ondokuz Mayıs Üniversitesi Tıp Fakültesi Psikiyatri Polikliniğine başvuran ve DSM-IV-TR panik bozukluğu ölçütlerini karşılayan 105 hasta dahil edilmiştir. Eş tanılar, yapılandırılmış görüşme formu SCID-I kullanılarak konmuştur. Tüm hastalara PABL, SCID-II, BAÖ, ADİ, PBŞÖ, PAÖ uygulanmıştır. Çalışmamızda PABL kullanılarak yapılan faktör analizi sonucunda saptanan üç faktörün kullandığımız klinik ölçekler ile korelâsyonuna bakılmıştır. Elde edilen faktörler için öngörücü olabilecek klinik özellikleri saptamak için bu faktörler PAÖ'nin alt başlıklarıyla hiyerarşik çoklu regresyon analizine tabi tutulmuştur.Bulgular: Hastaların %39'unda Eksen I, %42.9'unda Eksen II tanısı saptanmıştır. En sık eşlik eden tanılar; anksiyete bozuklukları ve duygudurum bozukluklarıdır. Panik ataklar sırasında görülen ilk üç belirti sıklık açısından çarpıntı, nefes darlığı, boğulma hissi, şiddet açısından ise çarpıntı, göğüs ağrısı ve nefes darlığı olarak saptanmıştır. DSM-IV-TR dışı belirtilerden ise ilk üç sırayı sıklık açısından huzursuzluk, ağız kuruluğu, görme bulanıklığı, şiddet açısından huzursuzluk, ağız kuruluğu, boğazda düğümlenme hissi almıştır. Yapılan faktör analizi sonucunda ?solunum-dolaşım?, ?bilişsel?, ?otonomik? şeklinde isimlendirdiğimiz üç faktör elde edilmiştir. Faktörlerin klinik özelliklerle olan ilişkisi incelendiğinde panik atak şiddeti, panik atak sıklığı, agorafobi derecesi birinci faktör olan solunum-dolaşım alt tipi için güçlü öngörücüler olarak bulunmuştur. Beklenti anksiyetesi ikinci faktör olan bilişsel alt tip için güçlü bir öngörücü olarak bulunmuştur. Sağlık konusunda endişe duyma derecesi solunum-dolaşım alt tipi için daha güçlü öngörücü, otonomik tip için öngörücü, bilişsel tip içinse ters öngörücü olarak bulunmuştur.Sonuç: Çalışmamızda panik bozukluğunun olası alt tipleri ve bunlarla ilişkili klinik özellikler belirlenmiştir. Panik bozukluğu hastaları değerlendirilirken DSM-IV-TR'de yer alan panik atağı belirtilerinin alt tipleri belirlemek için yetersiz olduğu göz önüne alınmalıdır. Alt tiplerin belirlenmesi hastalığın gidişi ve tedavi yöntemleri konusundaki çalışmalara katkı sağlayabilir. Objective: To investigate dispersion of panic attack symptoms, to determine the possible subtypes of panic disorder and the clinical features related to these subtypes and to investigate the factors which define subtyping in a group of patients with panic disorderMaterials and Method: A hundred and five patients who applied to psychiatry policlinics of Ondokuz Mayıs University Medicine Faculty and met the DSM-IV-TR panic disorder criterions were included in the study. Comorbidities were diagnosed by using Structured Clinical Interview for SCID-I. PASC, SCID-II, BAI, ASI, PDSS and PAS were applied to all patients. In our study, we examined correlation between clinical scales we use and three factors which was determined as a result of factor analysis that was performed by using PASC. To determine the clinical features which can be predictive for obtained factors, these factors were subjected to hierarchical multiple regression analysis with the subtitles of PAS.Results: Axis I diagnosis was determined in 39 % of patients, whereas axis II diagnosis was determined in 42.9 % . The most frequently observed comorbidities were anxiety disorders and mood disorders. The first three symptoms that observed during panic attacks were palpitation, sensations of shortness of breath, feeling of choking in terms of frequency; palpitation, chest pain and sensations of shortness of breath in terms of severity. The first three non-DSM-IV-TR symptoms were feeling of discomfort, dryness of mouth, blurred vision in terms of frequency; feeling of discomfort, dryness of mouth and feeling of knot in throat in terms of severity. As a result of the performed analysis, three factors were defined which were named as ?respiratory-circulatory?, ?cognitive? and ?autonomic?. When the relation between the factors and clinical features was investigated, panic attack severity, panic attack frequency and grade of agoraphobia were found to be strong predictors for the first factor, ?respiratory-circulatory? subtype. Anticipatory anxiety was found to be a strong predictor for the second factor, cognitive subtype. Grade of being anxious for health was stronger predictor for respiratory-circulatory subtype, predictor for ?autonomic? subtype, and a reverse predictor for the cognitive subtypeConclusion: In our study, possible subtypes of panic disorder and the clinical features related to them were determined. While evaluating the patients with panic disorder, it should be considered that panic attack symptoms in DSM-IV-TR are insufficient to determine the subtypes. The determination of subtypes could contribute in prognosis of disorder and studies about treatment methods.
