Abstract
Boğmaca, üst üste gelen inatçı ve spazmodik öksürük nöbetleriyle karakterize bir akut solunum yolu enfeksiyonudur. Etkin ve güvenli bir aşısı olduğu halde büyük çocuk ve erişkinlerde hala boğmaca görülmektedir. Türkiye'de rutin aşılama programında son boğmaca aşısı 18-24. ayda yapılmaktadır. Ne doğal bağışıklık ne de aşılama ömür boyu koruyuculuk sağlamaz. Aşılamadan 3-5 yıl sonra koruyuculuk azalmaya başlar ve yaklaşık 12 yıl sonra tamamen biter. Son yıllarda ergenler ve genç erişkinlerde boğmaca olgularında artış olduğu bilinmektedir. Erişkinlerde hastalık önemli bir klinik sorun ve yaşamsal risk getirmemektedir, ancak ergen ve erişkinler hastalığı aşıları tamamlanmamış küçük çocuk ve bebeklere bulaştırabilirler. Boğmaca küçük bebeklerde ağır ve ölümcül seyredebilir. Son yıllarda ergenlerde de uygulanabilen aselüler boğmaca aşısının keşfi bebeklik çağı dışında da aşılama olanağını gündeme getirmiştir. Büyük yaşlarda aşılama programlarının uygulamaya konulabilmesi için o toplumda hastalığın seroepidemiyolojisi hakkında bilgilere gerek vardır.Bu çalışmanın amacı bölgemizdeki çocuklarda boğmaca seroepidemiyolojisini belirlemek, elde edilecek sonuçlara dayanarak boğmaca aşısının ek rapel dozunun gerekip gerekmediği ve gerekiyorsa ne zaman yapılması gerektiği konusunda yorum yapmaktır.Ağustos 2008 ile ağustos 2009 tarihleri arasında serumları toplanan, yaşları 1,5 ile 18 yaş arasında değişen sağlıklı 385 (194 erkek, 191 kız) kişide antipertussis IgG antikor düzeyleri ölçüldü. Yaş grupları 1,5-3 yaş, 4-5 yaş, 6-8 yaş, 10-12 yaş, 13-15 yaş ve 16-18 yaş olarak belirlendi.Araştırma grubunun % 48,3'ünde antipertussis IgG antikor titreleri pozitif bulundu. Yaş gruplarına göre Antipertussis IgG antikor pozitiflik oranları 1,5-3 yaş grubunda %52,7 (55), 4-5 yaş grubunda %28,1 (64), 6-8 yaş grubunda %30,2 (63), 10-12 yaş grubunda %57,3 (75), 13-15 yaş grubunda %55,7 (61) ve 16-18 yaş grubunda %64,2 (67) saptandı. Serumların % 9,1'inde 125 IU/ml ve daha yüksek pozitif titreler saptandı. Bunların % 91.4'ünü 4-5 ve 6-8 yaş grubu dışındaki gruplar oluşturuyordu.Ülkemizde uygulanan aşı takvimine göre son boğmaca aşısı sonrası yaş grubu olan 1,5-3 yaş grubunda ortalama antipertussis antikor titreleri 45,42 IU/ml, 4-5 yaş grubunda 23,29 IU/ml (en düşük), 6-8 yaş grubunda 24,51 IU/ml, 10-12 yaş grubunda 48,87 IU/ml, 13-15 yaş grubunda 40,03 IU/ml, 16-18 yaş grubunda 51,38 IU/ml (en yüksek) bulundu. 1,5-3, 10-12, 13-15 ve 16-18 yaş gruplarının ortalama antipertussis antikor titreleri arasında istatistiksel olarak anlamlı fark yoktu (p>0,05). Ancak 4-5 ve 6-8 yaş grubu 1,5-3, 10-12, 13-15 ve 16-18 yaş gruplarıyla karşılaştırıldığında aradaki fark istatistiksel olarak anlamlıydı (p<0.05).Bu çalışmada 4-5 yaş ve 6-8 yaşlarda boğmaca antikorlarının seroprevalansı çok düşük bulunmuştur. Bu son doz boğmaca aşısından sonra aşı sonrası gelişen antikor düzeyinin 4-6 yıl içinde düştüğünü ve bu çocukların boğmacaya karşı duyarlı hale geldiğini göstermektedir. Okul yılları boyunca boğmaca seropozitivitesindeki artış bu yıllarda aşı yapılmadığına göre doğal enfeksiyon yoluyla olmaktadır. Doğal enfeksiyon prevalansının en yüksek olduğu 11-18 yaşlarda antikor titresi doruk düzeye çıkmaktadır.Bu verilere göre Türkiye'de bebeklikte uygulanan dört doz boğmaca aşılaması uzun süreli koruyuculuk için yeterli olmamaktadır. Boğmaca enfeksiyonu riskini azaltmak, ergen ve erişkinleri dolayısıyla da bebekleri boğmacadan korumak için, okul öncesi çocuklar veya ilköğretim 1. sınıfa ve ergenlere rapel dozlarının uygulanması gereği vardır. Okul öncesi yaşlarda pekiştirme doz aşıların uygulanmasının güçlüğü dikkate alındığında okula girişin ilk yılında yapılan dT aşısını, aselüler boğmaca aşısı da içeren karma Tdab biçiminde yapmak, ergen ve genç erişkinleri korumak açısından da ilköğretim son sınıfta bir rapel dozu daha yapmak uygun olacaktır. Ancak son kararı vermeden önce değişik bölgelerde benzer çalışmaların ve gider ? yarar analizlerinin yapılması gereklidir.Anahtar kelimeler: Pertussis, seroprevalans, ELISA, aşılama. Pertussis is an acute respiratory system infection which is characterised by pertistent overlapping and spasmodic coughing fits. Although safe and effective vaccination exists, pertussis is still seen in older children and adults. The last pertussis vaccination is performed at 18-24 months-old according to the Routine immunization program in Turkey. Neither natural immunity nor vaccination provides protection throughout whole life. After 3?5 years of vaccination protection starts to decrease and lasts totally approximately after 12 years. In recent years it is known that there is an increase in pertussis cases amongst young adults and adolescents. In adults, the disease does not cause an important clinical problem and a vital risk, however adolescents and adults may spread the disease to babies and young children whose vaccinations are incomplete. Pertussis can be severe and fatal in infants. In recent years, the discovery of acellular pertussis vaccine that can be applied to adolescents as well, has revived the possibility of vaccination beyond babyhood. In order to implement the vaccination programs at older ages, information about seroepidemiology of the disease in that community is neededThe purpose of this study was to determine- the seroepidemiology of pertussis in our region and according to our results it was analysed that either there is a need for rapel dose for pertussis vaccine or not. If there is a need then make comments on about the time of vaccinationAntipertussis IgG antibody levels of 385 healthy (194 male, 191 female) children between 1.5 and 18 years and whose serum were collected between August 2008 and August 2009 were measured. Age groups were defined as: 1,5-3 years, 4-5 years, 6-8 years, 10-12 years, 13-15 years and 16-18 yearsAntipertussis IgG antibody titers were found to be positive at 48.3% of the research group. IgG antibody positivity rates of 1,5-3, 4-5, 6-8, 10?2, 13-15, 16-18 age groups were determined as 52.7% (55), 28.1% (64), 30.2% (63), 57.3% (75), 55.7% (61), and 64.2% (67), respectively. 125 IU/ml and higher positive titres were determined at 9.1 % of the serums. 91.4% of these were composed of the groups except 4-5 and 6-8 age groupsThe average antipertussis antibody titers at 1,5-3 which is after the last dose according to the vaccination schedule applied in our country 4-5, 6-8, 10-12, 13-15 and 16-18 age groups were found as 45.42 IU/ml, 23.29 IU/ml (the lowest), 24.51 IU/ml, 48.87 IU/ml, 40.03 IU/ml and 51.38 IU/ml (the highest), respectively. There was no statistically significant difference between the average antipertussis antibody titers of 1.5-3, 10-12, 13-15 and 16-18 age groups (p> 0.05). However there was a statistically significant difference when the 4-5 and 6-8 age groups were compared with the others (p <0.05In this study, the seroprevalence of pertussis antibodies were found to be very low at 4-5 and 6-8 years. This shows that the antibody levels which are developed after the last dose of pertussis vaccine decreases in 4-6 years and these children become sensitive to pertussis. The increase of pertussis seropositivity during the school years is due to natural infection as there is no vaccination in these years. The antibody titer level makes peak in 11-18 age group at which the natural infection prevalence is the highestAccording to these data, four doses of pertussis vaccination applied in Turkey in infancy is not sufficient for long-term protection. In order to reduce the risk of pertussis infection, and protect the adolescents, adults and therefore the babies from pertussis, there is a need for a rapel dose application to pre-school or primary school class 1 children and adolescents. When the difficulty of implementing the rapel doses in pre-school ages is considered; applying the dT vaccine made in the first year of primary school as a mixed Tdab vaccine form including acellular pertussis vaccine and in order to protect the adolescents and young adults applying one more rapel dose in the last year of primary education will be more convenient.
