Abstract
Amaç : Glukozamin sülfatın radyasyona bağlı mesane hasarı üzerine etkisinin olup olmadığını saptamakGereç ve yöntemler : Çalışmamıza %80 güç, 10 birim fark ve 8 birimlik standart sapma ile grupları tanımlayacak sekizer rat olması gerektiği hesaplanarak 128 Spraque Dawley tipi dişi ratla başlandı. Radyoterapi öncesi kontrol grubu başlangıç değerlerini elde etmek için sakrifiye edildi. Daha sonra Glukozamin sülfat (GA), Radyoterapi (RT) ve Radyoterapi+Glukozamin Sülfat (RT+GA) olmak üzere üç kol oluşturuldu. Her bir kolda sekizer rat on günde bir sakrifiye edilerek gruplar tanımlandı. Bu şekilde 40. güne kadar sakrifikasyona devam edildi. Radyoterapiden on gün önce GA ve RT+GA kollarına glukozamin sülfat 600mg/kg/gün, glukozamin sülfat uygulanmayan kola ise aynı şekilde serum fizyolojik 9cc/gün oral gavajla verildi. Supin pozisyonda 50mg/kg ketamin HCL ile anestezi yapılarak pleksiglas üzerine sabitlenen bir rata mesaneye kontrast madde verilerek planlama BT çekildi. Eclips planlama sisteminde tüm organlar konturlenerek planlama yapıldı. Işınlama, RT ve RT+GA gruplarına, diafragma altı bölgeye 10x10 cm. alan boyutuna LINAC ile 6 MVX foton kullanarak SSD 100 cm., referans izodoz %92, tek doz 10 Gy olacak şekilde uygulandı. Radyoterapi uygulanan gün 0. gün olarak kabul edildi. Bir gün sonrasında sakrifikasyona başlandı. Sakrifiye edilmeyen GA ve RT+GA kollarına glukozamin sülfat, RT koluna ise serum fizyolojik verilmeye devam edildi. Sakrifikasyon servikal dislokasyonla ile yapıldı. Mesane eksize edilerek histopatolojik olarak mesane hasarının göstergeleri olan ürotelde dökülme, epitel kalınlığı, umbrella hücre sayısı, interstisiyel hücre sayısı ve bazal hücre sayısı, immünohistokimyasal olarak UPIII kalınlığı, UPIII boyanma kaybı, interstisiyel katın yüzeyel kısmında boyanma yoğunluğu incelendi. Normal dağılım sağlayan sürekli değişkenler için Anova ve Post Hoc Tukey testi, katogorik sınıflanan veriler için Kruskal Wallis ve Mann-Whitney U testi kullanıldı. Tüm hipotezler çift yönlüydü ve p< 0,05 anlamlılık değeri seçildi.Bulgular : Sadece GA uygulaması ile başlangıç değerine göre anlamlı fark gözlenmedi. Yine RT ile RT+GA grupları arasında da istatiksel farklılık yoktu. Dört grubun karşılaştırılmasında, 10. ve 20 günlerde RT ve RT+GA gruplarında ürotel tabakasındaki dökülmeler istatiksel olarak anlamlıydı (p=0,01 ve p=0,000).Epitel kalınlığı, RT grubunda hem GA hem de kontrol grubuna göre 20.günde azaldı (p=0,02 ve p=0,000), 30. günde düzeldi. RT+GA grubunda düşüş 10. günde başladı (p=0,01 ve p=0,003), 20. günde GA grubuna göre anlamlı farklılık yoktu, kontrol grubuna göre düşüş devam etti (p=0,005), 30. günde düzeldi.UPIII kalınlığı, RT grubunda hem GA hem de kontrol grubuna göre radyoterapinin 1. günde düşüş gösterirken (p=0,011 ve p=0,007), 30. günde düzeldi. RT+GA grubunda bu düşüş 10. günde başladı (p=0,002 ve P=0,001), 20.günde düzelme izlendi.UPIII boyanma kaybı 1. günde RT grubunda hem kontrol grubuna hem de GA grubuna göre daha fazla oranda izlendi (p=0,004). 10. günde RT ve RT+GA grubu kontrol grubuna göre, yine RT grubunun GA grubuna göre UPIII boyanma kaybı istatiksel olarak anlamlıydı (p=0,001). RT grubunda RT+GA grubuna göre UPIII boyanma kaybı daha fazlaydı (%85,7). 20. günde RT ve RT+GA grupları hem kontrol hem de GA gruplarına göre UPIII boyanma kaybı istatiksel olarak anlamlıydı (p=0,000). RT+GA grubunun tümünde UPIII boyanma kaybı izlendi ve kaybın %42,9'luk kısmı +++ düzeyindeydi. RT grubunda ise %83,3 lük bir boyanma kaybı söz konusuydu 30. günde RT+GA grubunda UPIII boyanma kaybı yokken RT grubunda %37,5 oranında bir boyanma kaybı vardı (p=0,037).Yüzeyel interstisiyel boyanma yoğunluğu RT grubu kontrol grubuna göre 10. günde istatiksel olarak anlamlı arttı (p=0,04). 20. günde RT+GA grubu hem kontrol hem de GA gruplarına göre yüzeyel interstisiyel boyanma yoğunluğu arttı (p=0,006 ve p=0,005).RT ve RT+GA grupları GA grubu ile karşılaştırıldığında; 10. günde RT ve RT+GA grubunda umbrella hücre sayısı azaldı (p=0,018 ve p=0,011). 20. günde RT grubunda umbrella hücre sayısında azalma devam etti (p=0,024). İnterstisiyel hücre sayısında, gruplar arasında istatiksel olarak anlamlı farklılık yoktu. RT grubu 20. günde kontrol grubuna göre anlamlı bazal hücre sayısında artış izlendi (p=0,02).Sonuç : Radyoterapi uygulamasında oral olarak verilen glukozamin sülfat, mesanede UPIII ekspresyonunu ve hücre proliferasyonunu arttırarak radyoterapi hasarını istatiksel olarak anlamlı düşürmesi söz konusu olabilmektedir. Bu çalışmada glukozamin sülfatın radyasyon uygulanan mesanede, ürotelde dökülme dereceleri, epitel kalınlığı, UPIII kalınlığı UPIII boyanma yoğunluk dereceleri ve hücre sayıları tespit edilerek epitelin korunmasına katkıda bulunabileceği gözlendi. Sonuçta, glukozamin sülfatın ürotelde radyoterapinin olumsuz etkilerini daha erken dönemde azaltabileceği düşünülmektedirAnahtar kelime : Glukozamin sülfat, radyasyon, mesane hasarı Objective : The objective of this study is to determine whether glucosamine sulfate is effective for preventing the irradiation induced bladder damage.Materials and Methods: 128 Spraque Dawley rats (female) were firstly examined by considering the requirements of 80% strength, 10 units of difference and 8 units of standard deviation and that every group include eight rats. Pre-radiotherapy control group was sacrificed in order to obtain the baseline values. The following three groups were established, afterwards: 1st group was glucosamine sulfate (GA), 2nd group was Radiotherapy (RT) and 3rd group was Radiotherapy + Glucosamine (RT + GA). Ten days prior to the radiotherapy, glucosamine sulfate of 600 mg / kg / day was applied to GA and RT + GA groups through oral gavage. Non ? glucosamine group on the other hand was administrated with serum physiologic of 9 cc / day through oral gavage, as well. The rat in the supination was subject to ketamine HCI anesthesia of 50 mg / kg and then this rat was fixed on a flexi glass thereby administrating the contrast agent for CT. Planning was carried out by contouring all the organs in the eclipse system. Irradiation was applied to RT and RT + GA groups to the sub-diaphragmatic region 10x10 cm with LINAC by 6 MVX photon, and SSD 100 cm, reference isodose 92%, and single dose of 10 Gy. The day of radiotherapy onset was considered to be day 0 (zero). The day following the radiotherapy eight rats from GA, RT and RT + GA groups were sacrificed. In the following days, glucosamine sulfate of 600 mg / kg / days was applied to the GA and RT + GA groups and serum physiologic was applied to RT group through gavage and up to the 40th day eight rats from GA, RT and RT + GA groups were sacrificed for every ten days. Sacrifice was carried out with cervical dislocation. Bladder was excised and the epithelial thickness, UP III thickness, UP III staining thickness, staining density in the surface site of interstitial layer, urothelial loss, numbers of umbrella cells, numbers of interstitial cells and numbers of basal cells ? histopathological and immunohistochemical indicators of bladder damage ? were examined. ANOVA and Post Hoc Tukey test were applied for the permanent variables enabling the normal distribution, and Kruskal Wallis and Mann ? Whitney U tests were adopted for the categorized results. All the hypothesis were two ways and the significance value was determined to be p<0.05.Findings: No significant difference was detected in the GA application compared to the initial value. Epithelium thickness in RT group was reduced in the 20th day compared to both GA and control group (p=0.02 and p=0.000) and improved in the 30th day. The reduction in the RT + GA group was initiated in the 10th day (p=0,01 and p=0,003), and there were no significant differences in the 20th day compared to GA group and the reduction continued compared to the control group (p=0.005) and this level was improved in the 30th day.UP III thickness reduced in the RT group in the 1st day of radiotherapy compared to both GA and control group (p=0.011 and p=0.007) and improved in the 30th day. This reduction in the RT + GA group initiated in the 10th day (p=0.002 and p=0.001) and improved in the 20th day.UPIII staining loss was higher in the RT group in the 1st day compared to control and GA group (p=0.004). UPIII staining loss rates of RT and RT + GA groups were statistically more significant in the 10th day than control group and those of RT group than GA group (p=0.001). UP III staining loss was higher in the RT group compared to RT + GA group (85.7%). UP III staining loss rates of RT and RT + GA were statistically more significant in the 20th day compared to control groups and GA groups (p=0.000). UP III staining loss was observed in all RT + GA groups and 42.9% of this loss rate was in the level of +++. On the other hand, 83.3% staining loss was detected in the RT group and there was no UP III staining loss in RT + GA group in the 30th day, however 37.5% staining loss was detected in RT group (p=0.037).Surface interstitial staining density increased statistically in the RT group compared to control group in the 10th day (p=0.04). Surface interstitial staining density increased in the RT + GA group in the 20th day compared to both control and GA groups (p=0.006 and p=0.005).The epithelial loss of RT and RT + GA groups were statistically significant in the 10th and 20th days (p=0.01 and p=0.000).The numbers of umbrella cells were reduced in the RT and RT + GA groups in the 10th day when RT and RT + GA groups were compared to GA group (p=0.018 and p=0.011). The numbers of umbrella cells continued to reduce in the RT group in the 20th day (p=0.024). The numbers of interstitial cells were not significantly different among the groups. The numbers of basal cells were significantly higher in the RT group in the 20th day (p=0.02).Results: Oral administrated glucosamine sulfate in the radiotherapy application may increase the UP III expression and cell proliferation in the bladder thereby statistically reducing the radiotherapy damage. This study supported that epithelial integrity was preserved by detecting the epithelial thickness of glucosamine sulfate in the irradiated bladder, UP III thickness, UP III staining loss levels, and urothelial loss levels, as well. In conclusion, we consider that the glucosamine sulfate may reduce the negative effects of radiotherapy in the urotelium in the early stage.Keywords: Glucosamine sulfate, radiation, bladder damage
