Abstract
AMAÇ: Bu çalışmada, T1DM'li çocuk ve adolesanların uzun dönem takiplerinde elde edilen sonuçlar değerlendirerek, glisemik kontrol durumlarını, tedavi şemalarını, antropometrik özelliklerini, puberte durumlarını, eşlik eden diğer otoimmün hastalıkları, gelişen subakut ve kronik komplikasyonları ve bu komplikasyonların gelişmesinde rol oynayan faktörleri ortaya koymayı amaçladık.GEREÇ VE YÖNTEM: Ondokuz Mayıs Üniversitesi Tıp Fakültesi Çocuk Endokrinolojisi Bilim Dalında Ekim 1991 ve Ekim 2009 tarihleri arasında takip ve tedavi edilen, diyabet süresi 5 yıl ve daha fazla olan 69 T1DM'li çocuk ve adolesan hastanın dosyaları incelendi.BULGULAR: Çalışmaya alınan 69 hastanın 43'ü (% 62,3) erkek, 26'sı (%37,7) kızdı. Hastaların takvim yaşları ortalaması 13,4±3,2 (7,3?17,9) yıl; tanı yaşları 0,7 ile 11,8 ortalaması 6,1±3,1 ( 0,7?11,8) yıl olarak saptandı. Diyabet süresi ortalamaları 7,3±1,9 (5,1?12,9) yıldı. Son muayene VKİ'lerine göre 69 hastanın 3'ü (%4,3) obez, 5'i (%7,2) fazla kilolu kiloluydu. Fazla kilolu hastaların 1'i erkek iken; 4'ü (%80) kız; obez hastaların 2'si (%66,7) erkek iken, 1'i (%33,3) kızdı. Final boya ulaşan hastaların final boy SDS'si hedef boy SDS'sine göre daha fazla olarak saptandı. Hastalarda 19'unda (%27,5) oranında lipodistrofi, 22'sinde (%31,9) dislipidemi geliştiği saptandı. Dokuz hastada (%13,0) KEH tespit edildi. L1-L4 kemik mineral dansitometre Z skoru -4,5 ile 4,9 arasında olup ortalama -0,01 ± 1,58 olarak bulundu. Hastaların 4'ünde (%5,8) osteopeni; 3'ünde (%4,3) ise osteoporoz tespit edildi. Yirmiüç hastada (%33,3) mikrovasküler komplikasyon geliştiği görüldü. Ondört hastada (%20,3) sadece diyabetik nefropati, 6'sında (%8,7) sadece diyabetik nöropati, 2'sinde (%2,9) sadece diyabetik nefropati ve nöropati, 1'inde (%1,4) diyabetik nefropati, nöropati ve retinopati geliştiği görüldü. Mikrovasküler komplikasyon gelişme riski, tanı yaşı ve diyabet süresi fazla olan hastalarda daha yüksek olduğu saptandı. KEH ile diyabetik nöropati birlikteliği saptanmazken; KEH ile diyabetik nefropatisi olan 2 hasta (%22,2) olduğu görüldü. Hastaların diğer otoimmün hastalıklarla birlikteliği değerlendirildiğinde; 14'ünde (%20,3) hashimoto tiroiditinin; 8'inde (%11,6) gluten enteropatisinin eşlik ettiği saptandı.SONUÇ: Diyabetin geç dönem komplikasyonlarının erken bulguları çocukluk döneminde başlar. Bu nedenle diyabetin geç dönem komplikasyonların erken tanınması ve gerekli önlemlerin alınması diyabet hastalarının yaşam sürelerinin uzatılması ve yaşam kalitelerinin iyileştirilmesi açısından oldukça önemlidir.Anahtar Kelimeler: Tip 1 Diyabetes Mellitus (T1DM), çocuk, komplikasyonlar OBJECTIVE: In the present study, by evaluating the outcomes which were obtained in long-term following of the children and adolescents with Type 1 DM, we aimed to reveal their glisemic control conditions, treatment diagrams, the anthropometric features, puberty conditions, other concomitant autoimmune diseases, developing subacute and chronic complications and the factors which play role in developing of the complications.MATERIAL AND METHOD: The files of 69 children and adolescent patients with T1DM who were treated and followed in Ondokuz Mayıs University Pediatric Endocronology Department between October 1991 and October 2009 and whose diabetes duration was 5 years and more were examined.RESULTS: Fortythree (% 62,3) of 69 patients who were included in the study were male, 26 (%37,7) were female. The mean chronological age of patients was 13,4±3,2 (7,3?17,9) years; the mean diagnosis age was determined as 6,1±3,1 ( 0,7?11,8) years. The mean diabetes duration was 7,3±1,9 (5,1?12,9) years. According to the last examination BMI; 3 (%4,3) of 69 patients were obese, 5 (%7,2) were fat; whereas 1 of fat patients was male; 4 were female; whereas 2 of obese patients were male, 1 was female. The final height SDS of the patients who reached to final height was determined as more than target height SDS. In 19 (%27,5) patients lipodystrophy was determined and in 22 patients (%31,9) dyslipidemia was determined. In 9 (%13,0) patients LJM was determined. The mean L1-L4 bone mineral densitometer Z score was -0,01 ± 1,58 (-4,5- +4,9). In 4 (%5,8) patients osteopenia was determined; in 3 (%4,3) patients osteoporozis was determined. In 23 (%33,3) patients, it was seen that microvascular complication has developed. In 14 (%20,3) patients; only diabetic nephropathy, in 6 (%8,7) patients; only diabetic neuropathy, in 2 (%2,9) patients; only diabetic nephropathy and neuropathy, in 1 (%1,4) patient; diabetic nephropathy, neuropathy and retinopathy developed. Microvascular complication developing risk was determined as higher in the patients whose diagnosis age and diabetes duration is much more. Whereas no association of LJM with diabetic neuropathy was determined, 2 (%22,2) patients were seen who had KEH and diabetic neuropathy. When the concomitans of patients with the other autoimmun diseases were evaluated; it was determined that in 14 (%20,3) patients hashimoto thyroiditis was determined and in 8 patients (%11,6) gluten enteropathy was determined.CONCLUSION: The early results of diabet?s late-term complications begin in childhood. Thus, early realizing of diabet?s last-term complications and providing the necessary precautions is so important in terms of prolonging the survival and improving the life qualities of diabetes patients.Keywords: Tip 1 Diabetes Mellitus (T1DM), children, complications
