Abstract
AMAÇ: Çocukluk çağı akut lösemilerinde tedavi sırasında enfeksiyon kökenli pnömoniler başta olmak üzere pulmoner komplikasyonlar sık görülmektedir. Bu hastalarda tanı güçlüğü olabilmektedir. Pulmoner komplikasyonlar immun sistemi baskılanmış olan lösemili hastaların bir bölümünde antimikrobiyal ve destek tedavisindeki gelişmelere rağmen mortaliteye neden olabilmektedir. Çalışmamızda bölümümüzde akut lösemi tanısı alan ve takibi sırasında pulmoner komplikasyon gelişen hastaların sıklığını, klinik, laboratuvar, radyolojik, mikrobiyolojik özellikleri ile hastalığın tedavisi, izlemi, prognozu ve akut lösemi ile ilgili özelliklerini değerlendirilerek bulgularımızı literatür ile karşılaştırmak tanı ve tedavi konusunda eksiklerimiz varsa bunlardan mümkün olunları gidermenin yollarını aramayı amaçladık.HASTALAR VE YÖNTEM: Çalışmaya 1 Haziran 2004 ?1 Temmuz 2009 tarihleri arasında Ondokuz Mayıs Üniversitesi Tıp Fakültesi Çocuk Hematoloji Bilim Dalı'nda akut lösemi tanısı alan ve tedavi gören, tedavi sırasında veya tedavi sonrasında pulmoner komplikasyon gelişen hastalar alındı. Hastaların akut lösemi tedavi sırasında ve sonrasında gelişen pulmoner komplikasyonlara ait özellikleri, klinik, laboratuvar ve radyolojik bulguları değerlendirildi. Tanı, tedavi, izlem ve prognoz ile ilgili özellikler araştırıldı.BULGULAR: Çalışmaya alınan 60?ı (%51,7) kız, 56?sı (%48,3) erkek, yaş ortancası 58 ay (6 ay-194 ay) olan toplam 116 akut lösemili hastadan 46 olguda 68 adet (%58) pulmoner komplikasyonun geliştiği gözlendi. Pulmoner komplikasyonların 58 (%85.3)'i enfeksiyon kökenli idi ve en sık tedavinin konsolidasyon & güçlendirme aşamasında (%54.4) geliştikleri saptandı. Enfeksiyon dışı pulmoner komplikasyonlar ise %11.8 sıklıkta olup daha çok tedavi başlamadan veya tedavinin hemen başında ortaya çıkmıştı. İki olguda pulmoner komplikasyonun etyolojisi sınıflandırılamamıştı. Komplikasyonların % 47.1'ı ağır nötropenik (ANS<500/µL) dönemde ortaya çıkmıştı. Ağır nötropenik hastalarda ateş, hipoksemi, oksijen ve mekanik ventilasyon gereksinimi, ek sistem tutulumu, anemi (Hb<8g/dl), ağır trombositopeni (<20000/mm³), antimikrobiyal tedavide değişiklik gereksinimi, antifungal kullanımı ve pulmoner komplikasyon nedeniyle ölüm ağır nötropenik olmayan hastalara göre anlamlı derecede yüksekti. Pulmoner komplikasyon gelişen hastaların 52'sinde (%76 ) ateş, öksürük, akciğer oskültasyonunda raller, solunum seslerinde azalma, takipne ve hipoksi gibi semptom veya fizik muayene bulgularından en az ikisi mevcuttu. Hastaların %27,9 unda takipne, %20'sinde hipoksemi ve oksijen gereksinimi vardı. Bu hastalardan 5 tanesi mekanik ventilatöre gereksinim göstermiş ve ancak birisi hayatta kalabilmişti. Pulmoner komplikasyonların 15'inde (%22) solunum sistemi dışı organ veya sistemlere ait patolojik muayene bulgularından bir veya daha fazlası mevcuttu. Çalışma grubunda oluşan 68 pulmoner komplikasyondan 10 tanesinde (%14.7) Toraks CT, 7 tanesinde de (%10.3) HRCT çekilmişti ve en sık rastlanan BT veya HRCT bulgusu konsolidasyon, buzlu cam görünümünde opasiteler ve alveoler dansite artımı idi. Mikrobiyolojik olarak etken ajan sadece 13 hastada ( %17.6) gösterilebilmişti. Bu nedenle hastaların çoğunda çoklu antibiyotik ve antifungal tedavi ampirik olarak kullanılmıştı. Çalışmada akut lösemilerde pulmoner komplikasyon gelişme sıklığı %58, pulmoner komplikasyona bağlı mortalite oranı %10 olarak bulundu. Pulmoner komplikasyon oluşan hastalarda lösemi tipinin AML olması ve hastalığın relaps olmasının yanı sıra , takipne, hipoksi ve mekanik ventilatör gereksinimi ve trombositopeni varlığının mortalite üzerine anlamlı etkisi olduğu görülmüştür. Pulmoner komplikasyon oluştuğu sırada ölen hastalarda antibiyotik tedavisinde değişiklik yapma oranı ve antifungal kullanımı da iyileşen hastalardan yüksek bulundu.SONUÇ: Çalışma grubumuzda pulmoner komplikasyon gelişme sıklığı oldukça yüksek olmasına rağmen pulmoner komplikasyona bağlı mortaliteyi literatüre göre kabul edilebilir sınırlarda bulduk. Bunun hastaların çoğuna ampirik olarak başlanan geniş spektrumlu poliantimikrobial ve antifungal tedaviyle ilgili olduğunu düşünüyoruz. Ancak bu tip tedavinin maliyeti yüksek olmaktadır. Her ne kadar mortalitede aşırı bir yükseklik olmasa da çalışma grubumuzda pnömonili hastaların çok azında etken ajanın saptanabilmesi mikrobiyolojik tanı yöntemlerimizin yetersiz olduğunu düşündürmektedir. Gerek materyal elde etme gerekse laboratuar çalışma tekniklerimizi geliştirmemizin hastalarda etken ajana yönelik doğru tedavi yaklaşımında bulunmamızı sağlayacağı ve böylece oldukça yüksek olan tedavi giderlerini de azaltacağı düşüncesindeyiz.Anahtar kelimeler: Pulmoner komplikasyonlar, çocukluk çağı, akut lösemi AIM: During the treatment of acute childhood leukaemia, pulmonary complications are frequently encountered, especially pneumonias of infectious origin. These diseases may be difficult to diagnose, and in spite of developments in antimicrobial and support treatments, pulmonary complications may cause mortality in some patients whose immune system is under pressure. In this study, our purpose was to evaluate the frequency of pulmonary complications in patients diagnosed with acute leukaemia, their clinical, laboratory, radiological, and microbiological characteristics, their treatment, follow-up, and prognosis, and the characteristics of acute leukaemia; we compared our findings with the literature and investigated ways to overcome any possible shortcomings in diagnosis and treatment.PATIENTS AND METHOD: The study was performed between 1 June 2004 and 1 July 2009 in the Child Haematology Department of Ondokuz Mayıs University Medical Faculty on patients who had been diagnosed with acute leukaemia and had undergone treatment, and who had developed pulmonary complications during or after treatment. The characteristics of these complications, and clinical, laboratory and radiological findings were assessed, and characteristics relating to their treatment, follow-up and prognosis were studied.FINDINGS: A total of 116 acute leukaemia patients were included in the study, of whom 60 (51.7%) were female and 56 (48.3%) were male. Average age was 58 months (6 months - 194 months), and 68 (58%) pulmonary complications were observed in 46 cases. Of these pulmonary complications, 58 (85.3%) were of infectious origin, and were observed to develop most frequently (54.4%) at the stage of consolidation of treatment. Non-infectious pulmonary complications were seen at a rate of 11.8%, and generally occurred before or at the very beginning of treatment. In two cases, the aetiology of pulmonary complications could not be classified. 47.1% of complications occurred in the severe neutropenic period (ANS<500/µL). In severely neutropenic patients, high temperature, hypoxaemia, the need for oxygen and mechanical ventilation, involvement of other organs, anaemia (Hb<8g/dl), severe thrombocytopenia (<20000/mm³), the need for changes in antimicrobial treatment, the use of antifungal treatment and pulmonary complications meant that mortality was high compared with non-severely neutropenic patients. In 52 (76%) of patients who developed pulmonary complications, at least two of the following were present: symptoms or findings at physical examination such as high temperature, coughing, rales in pulmonary oscultation, a reduction in respiratory sounds, tachypnea or hypoxia. Tachypnea was found in 27.9% of patients, and Hypoxia and a need for oxygen in 20%. Five of these patients showed a need for mechanical ventilation, and only one of these survived. In fifteen (22%) of those with pulmonary complications, one or more findings from pathological examination of the organs and systems other than the respiratory system were present. In ten (14.7%) of the 68 pulmonary complications which formed the study group, thoracic CT was performed and in 10.3% HRCT was taken, and the most commonly encountered finding of either CT or HRCT was consolidation, opacity with the appearance of frosted glass, and an increase in alveolar density. The microbiologically effective agents could be seen in only 13 (17.6%) of the patients, and for this reason multiple antibiotic and antifungal treatments were given empirically to most of the patients. The frequency of pulmonary complications in acute leukaemia patients on the study was 58%, and the mortality rate in connection with pulmonary complications was 10%. In patients where pulmonary complications occurred, tachypnea, hypoxia and the need for mechanical ventilation and the existence of thrombocytopenia along with leukaemia-type AML and the relapse of the patient was seen to have a significant effect on mortality. When pulmonary complications occurred, the rate of changes in antibiotic treatment and the use of antifungals in patients who died was higher than in patients who recovered.RESULTS: Although the frequency of development of pulmonary complications was very high in the study group, we found that mortality from pulmonary complications was at an acceptable level according to the literature. We feel that this was attributable to empirically initiated wide-spectrum poly-antimicrobial and antifungal treatment in most patients. However, the cost of this treatment is high. However high the mortality rate, our microbiological identification methods were sufficient to establish the effective agent in very few of the pneumonia cases in our study group. We feel that development of either material collection or laboratory working techniques will enable the right treatment to be established for the effective agent in these patients, and in this way the very high costs of treatment may be reduced.Key words: Pulmonary complications, childhood, acute leukemia
