Rejyonel anestezi yapılan hastalarda preoperatif verilen oral karbonhidrat solusyonlarının anksiyete, hemodinami ve derlenmeye etkisi

Abstract
Anesteziye bağlı solunum yetmezliğine yol açan pulmoner aspirasyonu azaltmak için acil ya da elektif cerrahiden önce uygulanan katı açlık kuralları hastalarda huzursuzluk, susuzluk, ağız kuruluğu, nefes kokması, halsizlik, bulantı ve kusma yakınmalarında artışa yol açmaktadır. Genel anestezi altında elektif cerrahi geçirecek hastalara girişimden en az 2 saat öncesine kadar verilen oral karbonhidratlı içecekler ile preoperatif susuzluk, açlık hissi ve anksiyetenin azaldığı, insülin direncinin düştüğü, postoperatif bulantı ve kusmanın azaldığı belirtilmiştir 5,6. Ancak rejyonal anestezi altında operasyonu planlanan hastalarda preoperatif dönemde verilen oral karbonhidratlı içeceklerin hemodinami ve anksiyete üzerine etkilerini gösteren bir çalışma yoktur. Biz de çalışmamızda elektif ürolojik cerrahi planlanan hastalarda preoperatif oral karbonhidrat solüsyonlarının preoperatif ve postoperatif anksiyete düzeylerine ve hemodinamiye etkisini araştırmayı amaçladık.Bu çalışma Ondokuz Mayıs Üniversitesi Tıp Fakültesi Hastanesi'nde Samsun Klinik Araştırmalar Etik Kurulu ve hastaların onamı alınarak 15.01.2010-15.08.2010 tarihleri arasında yapılmıştır. Çalışmamıza elektif ürolojik cerrahi planlanan 18-65 yaşları arasında ASA I- II, rejyonel anestezi altında Grade II cerrahi geçirecek 60 hasta dahil edildi. Hastalar randomize olarak iki gruba ayrıldı. Tüm hastalar gece yarısından sonra aç bırakıldı. Grup S'deki hastalara cerrahi öncesi 2 saate kadar içebildiği kadar su verildi. Grup K'daki hastalara ise cerrahiden önceki gece saat 24:00'e kadar 800 ml ve operasyondan 2 saat önce saat 06:00'da 400 ml % 12,5 glukoz içeren oral glikoz solüsyonu (Nutricia Preop® (Numici, Netherlands) içirildi. Hastalara ilk anksiyete testleri (STAI-SA ve STAI-TA) operasyondan önceki gün preoperatif vizit sırasında uygulandı. Rejyonel anestezi uygulama odasına alınan hastalara sıvı yüklemesi yapıldıktan sonra spinal anestezi uygulandı. Duyusal blok seviyesi T10 olan hastalar operasyon odasına gönderildi. Hastalar operasyon odasına alındıktan sonra rutin EKG, noninvaziv kan basıncı (NIBP) ve periferik oksijen saturasyonu (SpO2) monitorizasyonu yapıldı. Hemodinamik ve solunumsal parametreleri 5 dakika aralıklarla ölçüldü. Hastaların kan glikozu cerrahi başlamadan önce ve takiben hasta derlenme odasına gelinceye kadar saatte bir ölçüldü. Hastaya verilen toplam sıvı miktarı kaydedildi. Rejyonel anestezi odasında, cerrahi başlamadan önce ve operasyon odasında 30 dk arayla ve derlenme odasında hastaların açlık, susuzluk hissi, ağız kuruluğu, halsizlik ve operasyon korkuları VAS ile değerlendirdi. Hastanın postoperatif mide bulantısı ve kusma ve hasta memnuniyeti değerlendirildi. Spinal anestezinin iki segment gerileme zamanı kaydedildi. Duyusal blok seviyesi T 8- 9 olan hastalar servislerine gönderildiler.Hastaların demografik özellikleri, anestezi ve cerrahi süreleri, operasyona alınıncaya kadar olan açlık süreleri ve aldıkları toplam sıvı miktarı arasında anlamlı fark yoktu (p>0,05). Hemodinamik parametre değerlendirmesinde; karbonhidrat ve su grubunda kalp atım hızları operasyon başlangıç değerine göre anlamlı oranda düştü, ancak bu düşüş klinik olarak anlamlı değildi (p<0,05). Kan basınçlarının gruplar arası karşılaştırmasında; karbonhidrat grubunda sistolik kan basıncının operasyonun ilk 35 dakikasında; diyastolik kan basıncının operasyonun 5, 20 ve 35. dk'larında; ortalama kan basıncının spinal anesteziden önce ve spinal anestezi yapıldıktan sonraki dönemlerinde ve operasyon boyunca su grubuna göre istatistiksel olarak anlamlı olmak üzere daha yüksek seyretti (p<0,05).Hastaların açlık, ağız kuruluğu, susuzluk, operasyon korkusu, postoperatif bulantı ve kusma insidansı ve ağrı hissini VAS ile değerlendirdiğimizde iki grup arasında anlamlı fark bulamadık (p>0,05). Ancak halsizlik hissini karbonhidrattan zengin içecek alan grupta operasyon öncesi dönemde ve 30. dakikada su grubundan anlamlı olarak daha düşük olduğunu tespit ettik (p<0,05).Hastaların kan glikozu seviyeleri bakımından peroperatif dönemde ve derlenme odasında her iki grup arasında fark bulmadık (p>0,05).Her iki grupta da preoperatif ve derlenme odasındaki STAI-SA değerleri düşüktü (p<0,05). Çalışmamızda karbonhidrattan zengin içecek grubunda su grubuna göre hastaneden çıkış süresini anlamlı düzeyde düşük bulduk (p<0,05).Sonuç olarak rejyonel anestezi altında opere olacak hastalarda preoperatif dönemde verilen karbonhidrattan zengin içeceklerin kan glikoz düzeyini etkilemeden halsizlik hissini azalttığını, anksiyete üzerine olan etkisinin su alımı ile benzer olduğu ve kan basıncının intraoperatif dönemde daha yüksek olmasını sağladığı sonucuna vardık.ANAHTAR SÖZCÜKLER: Rejyonel anestezi, karbonhidrattan zengin içecek, anksiyete Rigid fasting rules applied before urgent or elective surgery to decrease pulmonary aspiration that causes respiratory failure related to anesthesia causes increase in the quantity of the complaints of the patients like thirstiness, dryness of mouth, smelling of breath, nausea, vomiting, weakness and anxiety. It was reported that carbonhydrate rich drinks given at least two hours before general anesthesia decreased preoperative thirstiness, hunger, anxiety, insulin resistance, nausea and vomiting. But there is no study about effects of preoperative carbonhydrate rich drinks on hemodinamia and anxiety before regional anesthesia . In our study our goal is to evaluate the effects of preoperative carbonhydrate rich drinks on hemodinamia and anxiety on patients who will undergo urologic surgery under regionel anesthesia.An approval has taken from Samsun Clinical Research Etic Committee and informed consent from patients.This study was performed between 15.01.2010-15.08.2010. 60 patients whose ages are between 18-65, ASA I-II (whose elective urologic surgery had been planned) and and who would have Grade II surgery under regional anesthesia, were included to our study. Patients were divided into two groups randomly. They were fasted after the midnight. Water was given to the patients in group S as much as they could drink until two hours before the surgery. 800 ml oral glucose solution (Nutricia Preop (Numici, Netherlands) was given to the patients in Group K in the night before the surgery until 24.00 and 400 ml oral glucose solution which includes %12,5 glucose was given two hours before the surgery at 06.00. The first anxiety tests (STAI-SA and STAI-TA) were applied to the patients in the preoperative visit the day before the surgery. After intravenous liquid charge, spinal anesthesia was performed.The patients whose block level reached T10 were sent to operation room. After the patients had been taken to the operation room, monitorisation of routine ECG, non-invasive blood pressure (NIBP) and peripheric oxygen saturation (SPO2) were done. The hemodynamic and respiratory parameteres were measured at every five minutes.The blood glucose levels of the patients were measured hourly before the surgery until they came to post anesthesic care unit. The total liquid amount which had given to the patient were recorded. The hunger, thirstiness feeling, mouth dryness, weakness and the operation fears of the patients were assesed with Visual Analog Scale (VAS) in the regional anesthesia room before the surgery and in the operation room and at postoperative care unit every 30 minutes. The postoperative nausea, vomiting and patient satisfaction were recorded. The two segment regression of spinal anesthesia was recorded. Patients whose block level had been T 8-9, were sent to their primary services..There was no significiant difference (p> 0,05) among demographic specialities, anesthesia and surgery lengths, fasting times of the patients until they had been taken to the operation and total liquid amount which they had taken.Heart rate decreased statistically significant in the carbonhydrate and water group in the hemodynamic parameter assesment but this decrease was not significant clinically(p<0,05). In the comparation of blood pressures between groups; it was observed that systolic blood pressure in the first 35 minutes of the operation in the carbonhydrate group; dialostic blood pressure in the 5., 20., and 35. minutes of the operation, mean blood pressure before and after the spinal anesthesia and along the operation were higher than water group significantly in terms of statistics (p<0,05).We couldn?t find a significant difference between two groups when we observed the hunger, mouth dryness, thirstiness, operation anxiety, postoperative nausea and vomiting incidence and VAS scores (p>0,05). We observed that weakness feeling in the group which had taken rich carbonhydrate drinks, was significantly lower than the water group in the preoperative period and in the 30. minute . (p<0,05)We couldn?t find a significant difference between two groups in the preoperative period and in postoperative care unit in terms of blood pressure levels of patients(p>0,05).STAI-SA values were both low in the both group in the preoperative period and in the postoperative care unit.(p<0,05). We observed that period of discharging from hospital in the rich carbonhydrate drink group was significantly lower than the water group(p<0,05).We conclude that carbonhydrate rich drinks decreased preoperative weakness feeling without effecting blood glucose level of the patients who will undergo urologic surgery under regional anesthesia.We also conclude that effects of these drinks were similar to water and they made blood pressure higher in the operative period.Key Words: Regional anesthesia, rich carbonhydrate drink, anxiety
