Parkinson hastalığında otonomik yetmezlik: Tremor baskın olan ve olmayan hasta gruplarını karşılaştıran prospektif bir çalışma

Abstract
Amaçlar: Parkinson hastalığı (PH)'nın ana motor belirtileri tremor, bradikinezi, rijidite ve postural insitabilitedir. Bununla birlikte PH'de, kabızlık, terleme bozukluğu, ortostatik hipotansiyon (OH), sfinkter disfonksiyonu (SD) ve erektil disfonksiyon gibi otonomik yetmezlik (OY) semptomları sıktır. PH'li hastaların yaklaşık %90'ı OY semptom ve bulgularından yakınmaktadır. OY'nin PH'nin motor alt tipleri ile ilişkisi fazla çalışılmamıştır. Bir çalışmada OH'nin, PH'nin postür ve yürüme bozukluğu olan motor alt tipi ile ilişkili olduğu, tremor alt tipi ile ilişkili olmadığı bildirilmektedir. Hipotezimiz OY'nin sıklığı ve şiddetinin, hastalığın motor alt tiplerinde farklı olabileceğini, tremor baskın (TB) PH'li hastalarda OY'nin daha az yaygın olabileceğidir. Çalışmamızın amacı, PH'deki OY sıklığı ve şiddeti bakımından TB ve tremor baskın olmayan (TBO) hasta grupları arasında fark olup olmadığını araştırmaktır.Hastalar ve Yöntemler: Bu çalışmaya yaş ve cinsiyet oranları benzer olan 30 TB, 30 TBO PH'li hastalar ve 30 sağlıklı gönüllü kişiler dahil edildi. OY'ye yönelik anamnez bilgileri alındı. Hastaların tedavi sırasında OFF döneminde, yatarak ve ayakta arteryel kan basıncı, kalp hızı ölçüldü. OY ile ilgili belirti ve bulgulardan biri veya birkaçı saptanan hastalarda OY pozitif kabul edildi. OY'nin şiddeti `The Scala for out comes in PD for autonomic symptom' (SCOPA AUT)'a göre belirlendi. Hastaların motor belirtilerinin baskınlığı, öykü ve Unified Parkinson's Disease Rating Scale (UPDRS)'in motor komponentine göre belirlendi. Buna göre hastalar TB ve TBO hasta grubu olarak ikiye ayrıldı. Gruplar demografik özellikler, OY, OH, konstipasyon, SD, hiperhidrozis sıklığı ve SCOPA AUT skorları için karşılaştırıldı.Bulgular: Çalışmamızda gruplar yaş, sayı, cinsiyet dağılımı açısından benzerdi (p=0.2). PH'li hastalarda%100, kontrol grubunda ise %53.3 oranında en az bir OY semptom veya bulgusu saptandı (p<0.0001). PH'li hastalarda SCOPA AUT skoru kontrol grubuna göre daha yüksek iken (P<0.0001), TB ve TBO gruplar arasında ise anlamlı farklılık bulunmadı (p=0.4). OH, PH'li hastalarda %25 oranında görülürken, kontrol grubunda ise hiç saptanmadı. PH'li hastalarda SD, konstipasyon, hiperhidrozis kontrol grubuna göre anlamlı oranlarda yüksek bulundu (P<0.0001). Her ne kadar OY'nin her bir belirti bakımından TB ve TBO gruplar arasında anlamlı fark yoksa da, OH ve SD, TBO grupta daha yaygındı. Konstipasyon hastalığın her evresinde yüksek oranlarda iken, SD ise hastalık evresi arttıkça görülme sıklığı artmakta idi (p=0.02).Sonuçlar: Çalışmamız, PH'li hastalarda OY'nin oldukça yaygın olduğu sonucunu desteklemektedir. Çalışmamızda hastalığın evresi ve süresi ilerledikçe, OY şiddeti ve sıklığı artmakta idi. OY'nin sıklığı ve şiddeti bakımından TB ve TBO gruplar arasında farklılık yoktu. Konstipasyon bakımından TB ve TBO gruplar arasında farklılık yok iken, OH ve SD TBO grupta daha yaygın görüldü. Böylece PH'li hastalarda OY'nin belirtileri ve bulguları her evrede dikkatlice değerlendirilmelidir.Anahtar kelimeler: Parkinson hastalığı, tremor baskın grup, tremor baskın olmayan grup, otonomik yetmezlik, ortostatik hipotansiyon, sfinkter disfonksiyonu. Objectives: The main motor symptoms of Parkinson?s Disease (PD) are tremor, bradykinesia, rigidity and postural instability. However, symptoms of autonomic failure (OF) such as constipation, sweating disorders, orthostatic hypotension (OH), sphincter disfunction (SD) and erectil disfunction are common. Approximately 90% of patients with PD suffer from OF symptoms and findings. In the motor subtypes of PD, OF had not been studied so much. In a study, it has been reported that OH is related to PD?s motor subtype that shows posture and walking instability; but not to tremor subtype. So we hypothesize that the frequency and severity of OF may be different in motor subtypes of disease and OF is less common in patients with tremor dominant (TD) PD. The aim of our study was to investigate if there is a different between TD and non-tremor dominant (NTD) patient groups in terms of the frequency and severity of OF in PD.Patients and Methods: Thirtypatients with TD PD, 30 patients with NTD PD and 30 healthy control individuals, whose age and gender rates were similar were included in the study. OF oriented anamnesis data was obtained from all subjects. The OFF period during treatment, the arterial blood pressure and heart rate of patients while standing and lying were examined, OF was accepted as positive in the present of one of the symptoms or findings of OF. The severity of OF was determined according to? The Scala for out comes in PD for autonomic symptom? (SCOPA AUT). Dominancy of patients motor symptoms was determined according to history and the motor component of Unified Parkinson?s Disease Rating Scale (UPDRS). The patients were divided in to two subgroups as TD and NTD. The groups were compared in terms of demographic features, OF, OH, constipation, SD, hyperhydrosis and SCOPA AUT scores.Results: In our study, the groups were similar in terms of age, size, gender rate (p=0.2). The frequency of OF was 100% in the PD patients and 53.3% in the control group (p<0.0001). SCOPA AUT score was higher in the PD patients compared with the control group (P<0.0001). There was no significant difference between TD and NTD groups (p=0.4). OH was observed in 25% of the PD patients. However it wasn?t determined in the control group. OH was more common in the NTD group compared with the TD group. In the PD patients, SD, constipation and hyperhydrosis were significantly higher than the control group. While constipation was high frequency in all stages, SD frequency had increased by disease stage (p=0.02).Conclusion: The present study supports that OF is so common in PD patients. In our study, the severity and frequency of OF had increased by increasing of disease?s stage and duration. There was no difference between the TD and the NTD patients in terms of frequency and severity of OF. While constipation was similar in both the groups, OH and SD was more common in the NTD group. Thus, in PD patients, symptoms and findings of OF should be examined more carefully in all stages.Keywords: Parkinson?s disease, tremor dominant group, non-tremor dominant group, autonomic failure, orthostatic hypotension, sphincter disfunction.
