Akciğer Kanserli Hastalarda Tanı ve Tedavide Gecikmeye Neden Olan Faktörler

Abstract
AMAÇ: Akciğer kanserli hastalarda tanı ve tedaviye kadar geçen süredeki gecikmeleri etkileyen faktörleri saptamak ve bunların hastalık evresi ve prognoza etkisini araştırmak.YÖNTEM: Akciğer kanser tanısı alan hastaların; yakınmaların başlangıcından; ilk doktora başvuruya kadar (`hasta başvuru süresi'), ilk doktora başvurudan tedavi süresine kadar (`doktor gecikmesi'), göğüs hastalıkları merkezine başvurana kadar (`sevk süresi'), başvurudan tanı (`tanı süresi'), tanıdan tedaviye kadar geçen süreler (`tedavi süresi') belirlendi.BULGULAR: Çalışma, akciğer kanser tanısı alan toplam 315 hasta üzerinde yapıldı. İkiyüzseksenikisi erkek, 33'ü kadın olan hastaların %80'i küçük hücreli dışı (KHDAK), % 20`si küçük hücreli akciğer kanseri (KHAK) idi. Hasta başvuru süresi ortalama 28 (0-301) gün ve bu sürenin hiç eğitim almamış, köyde yaşayan grupta daha uzun olduğu görüldü(p=0.02). Sevk süresinde en büyük zaman kaybı ortalama 71(0-678) gün ile aile hekimlerine başvuran hastalarda gözlendi (p=0.003). Tanı süresi ortalama 23(1-842), tedavi süresi 14(0-95) gün ile literatür bulguları ile uyumlu bulundu. KHDAK hastalarda doktor gecikme süresi ortalama 76(3-793) gün, KHAK hastalarda ortalama 70(9-389) gün olduğu izlendi. Hastaların tanı ve tedavi aşamasındaki gecikme süreleri ile evreleri ve sağ kalım arasında ilişki saptanmadı.SONUÇ: Birinci basamakta görevli hekimlerin eğitim düzeyini arttırarak hasta ve sevk süresi gecikmeleri önlenebilir. Doktor ve hasta gecikmesinin prognoza etkisinin olmaması, hastalığın ilerleyici olmasına bağlı olabilir.Anahtar Kelimeler: Akciğer kanseri, tanı, tedavi, gecikme OBJECTIVE: Detection of the factors that cause delay in diagnosis and treatment in patients with lung cancer and investigation of their effects on stage and prognosis of the disease.METHOD: For all the patients with lung cancer; time from the onset of the symptoms to the application to the first doctor(`patient admission time?), time from the first admission to treatment(`doctor delay?), time to the application to the chest disease department(`referral duration?), time from the second admission to diagnosis(`diagnosis duration?) and time from diagnosis to treatment(`treatment duration?) were determined.RESULTS: The study was performed with totally 315 patients who were diagnosed as lung cancer. 282 of the patients were male and 33 of patients were female with whom %80 have NSCLC, %20 have SCLC. The mean patient application time was 28 (0-301) days and this was longer in patients who weren?t educated and who live in a village(p=0.02). The maximum mean elapsed time in the referral duration which was observed in patients who applied to family medicine doctors (p=0.03) was 71(0-678) days. The mean diagnosis duration with 23(1-842) days and the mean treatment duration with 14(0-95) days were found to be consistent with literature. The mean doctor delay time was 76(3-793) days in patients with NSCLS and it was 70(9-389) days in patients with SCLC. No association was detected between the delays in the diagnosis and treatment durations and survival.CONCLUSİON: Delays seen during the referrals can be prevented with enhancing the awareness of the primer care doctors. The fact that the doctor and patient delay have no effect on the prognosis may be associated with the progressive nature of the disease.Keywords: Lung cancer, diagnosis, treatment, delay
