Abstract
AMAÇ: Güvenilir bir aksiller evreleme yapabilmek için yüksek seviyede kesinlik gerekmektedir. Geleneksel olarak doğru bir aksiller evreleme için en az 10 lenf nodu çıkarılması önerilir. Bu çalışmanın amacı, meme kanserinde neoadjuvan kemoterapi alıp sonrasında opere edilen ve kemoterapi almadan opere edilen hastalar arasında aksiller diseksiyonla çıkarılan lenf nodu sayılarını karşılaştırmak ve lenf nodu pozitifliğine etki eden faktörleri araştırmaktır.METOD: 01.01.2006-31.10.2012 tarihleri arasında Ondokuz Mayıs Üniversitesi Tıp Fakültesi Genel Cerrahi Kliniği?nde lokal ileri meme kanseri nedeniyle neoadjuvan kemoterapi alıp sonrasında opere edilen 49 hasta ile kemoterapi almadan primer cerrahi yapılan 144 hasta çalışmaya alındı. Aksiller diseksiyonla çıkarılan toplam lenf nodu sayısı, pozitif lenf nodu sayısı ve <10 ile ?10 lenf nodu sayısı olarak kategorize edildi. Karşılaştırılacak değişkenler SPSS 15.0 programından yararlanılarak kaydedildi. P<0.05 anlamlılık düzeyi olarak kabul edildi.BULGULAR: Neoadjuvan kemoterapi alan grupta çıkarılan ortanca lenf nodu sayısı 16(6-33) almayan grupta ise 20(5-55) idi. Ortanca pozitif lenf nodu sayısı neoadjuvan kemoterapi grubunda 5(0-19), primer cerrahi grubunda 10(0-51) idi. Neoadjuvan kemoterapi grubundaki 49 hastanın 8?inde (%16.3) 10?un altında lenf nodu çıkarılırken, primer cerrahi grubundaki 144 hastanın 17?sinde (%11.8) 10?un altındaydı. İki grup arasındaki çıkarılan toplam ve pozitif lenf nodu sayılarının anlamlı olarak farklı olduğu ancak <10 lenf nodu sayısı farkının anlamlılığa ulaşmadığı saptandı.SONUÇ: Neoadjuvan kemoterapi alan meme kanserli hastalarda çıkarılan lenf nodu sayısı, almayanlara göre daha az olabilir. Bu durum her zaman yetersiz bir aksiller diseksiyona bağlanmamalıdır. Neoadjuvan kemoterapi alan meme kanseri hastalarında evrelemenin güvenilirliğini artırmak için lenf nodu sayısının değerlendirildiği daha fazla çalışmaya ihtiyaç vardır. AIM: A high level of accuracy is required to make a reliable axillary staging. It is recomended to take out at least 10 lymph nodes for a traditionally accurate axillary staging. The aim of this study is to compare the numbers of lymph nodes taken out by dissection between the patients with breast cancer operated on after having neoadjuvant chemotherapy treatment and the ones operated on without having chemoteraphy, and to investigate the factors affecting the lypmh node positivity.METHOD: 49 patients operated due to advanced breast cancer after neoadjuvant chemotherapy and 144 patients with the similar stage of the cancer having primary surgical treatment without chemotherapy at the general surgery clinic of Ondokuz Mayıs University Medicine Faculty between the dates 01.01.2006 and 31.10.2012 are included in the study. The total number of lymph nodes taken out by axillary dissection is categorized as the number of positive lymph nodes and the number of <10 and ?10 lymph nodes. The variables to be compared are recorded using the program SPSS 15.0. P<0.05 is accepted as significance level.FINDINGS: Median number of dissected lymph nodes from the patient group having neoadjuvant chemotheraphy is 16 (16-33) and 20 (5-55) for the patient group without having chemotheraphy. The number of median positive lymph nodes is 5 ( 0-19) in the chemotheraphy group and 10 (0-51) in the primary surgical group of patients. In 8 patients out of 49 neoadjuvant chemotheraphy patients (16.3%), the number of dissected lymph nodes was below 10, and it was below 10 in 17 patients out of 144 primary surgical patients. It was detected that numbers of dissected total and positive lymp nodes between two groups were significant, but the differences between the numbers of <10 lymph nodes didn?t reach significance.CONCLUSION: The number of dissected lymph nodes from the patients with breast cancer having neoadjuvant chemotheraphy may be less than the ones dissected from the patients without having chemotheraphy. This may not always be attributed to an inadequate axillary dissection. More researches to evaluate the number of lymph nodes are required in order to increase the reliability of staging in the patients with breast cancer having neoadjuvant chemotheraphy.
