Abstract
Giriş: Bu çalışmada düşük doğum ağırlıklı (DDA) bebeklere folik asit (FA) takviyesinin gerekliliği, hangi dozun uygun olduğu ve anne sütünün folat içeriğinin belirlenmesi amaçlanmıştır.Materyal-Metod: Ondokuz Mayıs Üniversitesi Yenidoğan Yoğun Bakım Ünitesi?nde 01.01.2012-31.09.2012 tarihleri arasında izlenen 141, DDA bebek prospektif olarak değerlendirildi. Bebeklerin birinci gün serum ve eritrosit içi folat (EİF) seviyeleri alındıktan sonra izlemde 12 bebek FA alamadı, 46 bebek 25 µg, 39 bebek 50 µg, 44 bebek 75 µg FA desteği aldı. Birinci ay sonunda kontrol serum ve EİF seviyeleri bakıldı. Annelerin doğumdan sonraki 48 saat içinde serum- EİF ve üçüncü günde sütlerindeki folat seviyeleri ölçüldü. Bebeklerin ne ile beslendiği, maternal FA, sigara ve alkol kullanımı kayıt edildi.Bulgular: 141 DDA'lı bebeğin ortalama doğum ağırlığı 1788,2±478,4 g, ortalama gebelik süresi 33,5±2,9 hafta idi. Bebeklerin birinci gündeki ortalama serum ve EİF seviyeleri sırasıyla 21,2±12,2 ng/ml, 922,7±460,7 ng/ml idi. Annelerin ortalama serum ve EİF seviyeleri sırasıyla 12,6±7,5 ng/ml ve 852,3±302,5 ng/ml idi. Anne sütü folat seviyesi 31,4±34,1(ortanca: 17) ng/ml idi. Gebelikte FA kullanan annelerin anne sütü folatı kullanmayanlara göre istatistiksel olarak anlamlı yüksekti (p<0,001). Anne eritrosit içi ve serum folat düzeyi ile bebeklerin doğum ağırlığı ve preterm doğum arasında anlamlı bir ilişki bulunamadı. Anne eritrosit içi ile bebek eritrosit içi folatı ve bebek serum folatı arasında zayıf güçte bir korelasyon vardı (r2=%15,4, r2= %3,6). Anne serum folatı ile bebek serum folatı ve süt folatı arasında zayıf güçte korelasyon vardı ( r2=%17,3; r2=%3,9). Bebek eritrosit içi ve serum folatı ile doğum ağırlığı, gebelik haftası, respiratuvar distres sendromu, bronkopulmoner displazi, prematüre retinopatisi, sepsis, konjenital kalp hastalığı, intraventriküler kanama ve kan transfüzyonu için tekrar yatış arasında istatistiksel olarak anlamlı bir ilişki bulunamadı. Gruplar arasında anne, bebeklerin demografik özellikleri, anne sütü folatı açısından fark yoktu. Folik asit alanlarla almayanlar karşılaştırıldığında eritrosit içi ve serum folat düzeyleri alanlarda daha yüksek olarak bulundu fakat istatistiksel olarak anlamlı bulunmadı. Güçlendirilmiş anne sütü veya formüla ile beslenen bebeklere verilen farklı dozlardaki FA?in kontrol folat seviyesini etkilemediği görüldü.Sonuç: Sonuç olarak DDA bebeklere FA takviyesinin gerekliliğini belirlerken; bebeğin ne ile beslendiğine dikkat edilerek karar verilmelidir. Güçlendirilmiş anne sütü ile beslenen bebeklerin FA takviyesine ihtiyacı yoktur. Yalnız anne sütü ile beslenen bebeklerin FA takviyesi gerekliliğini belirlemek için daha geniş çalışmalara ihtiyaç vardır. Objective: The aim of this study was to determine if there is a need to give folic acid to low birth weight babies (LBW); what is the optimum dose for folic acid replacement for these babies and to assess human milk folate levels.Subjects and Methods: We analysed 141 LBW babies prospectively followed in Ondokuz Mayis University Neonatal Intensive Care Unit in between 01.01.2012-31.09.2012. After taking blood for plasma and red blood cell (RBC) folate levels on the first day of life, we randomly gave babies 25 µg (46 babies), 50 µg (39 babies), 75 µg (39 babies) folic acid. Twelve babies could not get folic acid supplementation. Blood for control plasma and RBC folate levels were taken at the end of the first month. Mother?s RBC and plasma folate levels within 48 hours of delivery and milk folate levels which were taken on the postpartum third day were analysed. Feeding type of babies, maternal FA, tobacco and alcohol use were also recorded. Results: The mean birth weight and gestational age of babies were 1788,2±478,4 g and 33,5±2,9 weeks, respectively. The mean plasma and RBC folate levels on the first day of life were 21,2±12,2 ng/ml, 922,7±460,7 ng/ml, respectively. The mean plasma and RBC folate levels of mothers were 12,6±7,5 ng/ml ve 852,3±302,5 ng/ml, respectively. Human milk folate level was 31,4±34,1 (median 17) ng/ml. The milk folate levels of mothers? who got FA through gestation were higher than who did not (p<0,001). There was no relation between mothers? RBC-plasma folate levels and birth weight of babies and prematurity. There was mild correlation between mothers? RBC folate levels with babies? RBC-plasma folate levels (r2=%15,4, r2= %3,6). There was mild correlation between mothers? plasma folate levels with babies? plasma and milk folate levels ( r2=%17,3; r2=%3,9). There was no relation between babies? RBC-plasma folate levels and birth weight, gestational age, respiratory distress syndrome, bronchopulmonary dysplasia, retinopathy of prematurty, septicemia, congenital heart disease, intraventricular hemorrhage and rehospitalisation for RBC transfusion, There were no difference between groups for demographic features of babies? and mothers? and milk folate levels. The RBC-plasma folate levels of babies who were given FA supplementation had higher control RBC-plasma folate levels but it was not statistically significant. We found that there was no effect of different doses of FA supplementation to control folate levels for babies either fed by fortified human milk or formula. Conclusion: While we decide to give folic acid supplementation of LBW infant we should take into account the type of feeding. We can say that there may be no need for folic acid supplementation of babies getting fortified human milk. More studies are required to determine folic acid supplementation of exclusively breastfed babies.
