Acil tıp kliniğine başvuran non-benzodiyazepin antiepileptik ilaç zehirlenmelerinin incelenmesi

Abstract
AMAÇ Çalışmamızın amacı, 2008?2012 yılları arasında 19 Mayıs Üniversitesi Tıp Fakültesi Hastanesi Acil Servisi?ne (OMÜTF) başvuran non-benzodiyazepin antiepileptik ilaçlara (NBDAEİ) maruz kalımların demografik özelliklerini, klinik bulgularını, tedavi yöntemlerini, klinik sonuçlarını ortaya koymaktır. YÖNTEMKesitsel ve tanımlayıcı nitelikteki araştırmamızda, acil servise zehirlenme başvurusuyla kabul edilen adli takip dosyası açılan benzodiyazepinler dışındaki antikonvülzan ilaçlara maruz kalan olguların yaş, cinsiyet, alınan antikonvülzan tipleri, zehirlenme tipi, zehirlenme nedeni, klinik belirti ve bulgular, önerilen ve uygulanan tedavi yöntemleri ve sonuç verileri değerlendirildi. Hastaların meslekleri, psikiyatri konsultasyonu sonrası aldığı tedaviler, hasta özgeçmişlerine yönelik dosya verileri tarandı. Tüm veriler çalışma için hazırlanmış standart bilgi formlarına, daha sonra da SPSS version 15 veri tabanı programına kaydedildi. Olguların klinik belirti ve bulguları EAPCCT/IPCS Poisoning Severity Score?a göre derecelendirildi. Değişkenlerin normal dağılıma sahip olup olmadıkları Kolmogorov Smirnov testi ile incelendi. Değişkenlere ilişkin tanımlayıcı istatistikler frekans, yüzde, aritmetik ortalama ± standart sapma (SD), ortanca (min.-mak.) şeklinde sunuldu. Sürekli değişkenler için iki grup karşılaştırmaları Mann Whitney U testi ile 3 ve üzeri grup karşılaştırmaları Kruskal Wallis testi ile incelendi. Kategorik değişkenlerin incelenmesinde ise Ki-Kare testi kullanıldı. P<0.05 istatistiksel olarak anlamlı kabul edildi. BULGULAR Çalışmaya alınan dönem içinde başvuran 1987 hastanın 64?ü NBDAE ilaçlarla zehirlenmeye maruz kalmıştı. NBDAE ilaçlarla zehirlenme oranı tüm zehirlenmelerin %3.2?sini oluşturduğu belirlendi. Hastaların %64.1?i kadın (n=41), %36?sı erkek (n=23) hastalardan oluşmaktaydı. Genel yaş ortalaması 28±10 olarak bulundu. Başvuralar en çok kış(n=19,%32.9) ve yaz (n=17,28.9) mevsimlerinde yapılmıştı. En sık başvuru yapılan ay ekim(n=9,%15.3) en sık başvuru saati 18:00?23:59 (n=27,%42.2) arası yapılan başvurulardan oluşmaktaydı. Hastaların %92.2 si istemli olarak ilaç almıştı. Başvuruların %67.1?i (n=43) 18?29 yaş aralığında yer alıyordu. Zehirlenen hastaların %59.3?ünün gelir getirici işi yoktu. Zehirlenmelerin en çok ilaç alımını takiben 2 saat içinde (n=25,%39.1) bir sağlık kuruluşuna başvurduğu belirlendi. Hastalar en sık eski kuşak (n=52, %81.3) antiepileptiklerle zehirlenmişti. En sık zehirlenilen ilaçlar tek başına karbamazepin (n=22, %34.4) ve tek başına valproik asid (n=19, %29.7) ile maruziyet olarak belirlendi. Eski ve yeni kuşak VIII ilaçlarla zehirlenmeler klinik bulgu ve belirti gelişimi, hastanede kalış süreleri açısından karşılaştırıldığında aralarında farklılık saptanmadı. Karbamazepin alan olgularda daha çok klinik bulgu ve belirti geliştiği ancak buna rağmen hastanede kalım süresinin daha kısa olduğu saptandı. NBDAE ilaçlarla zehirlenmede birlikte başka bir grup ilaç alım oranı %45.3 (n=29) olarak saptandı. Birlikte alınan ilaçlar en çok %12.5 alım ile antideprasan ve antipsikotik ilaçlardı. Birlikte alkol alımı erkeklerde kadınlara oranla daha sık gözükmektedir.Hastaların %72?si (n=46) önceden NBDAE ilaç kullanıyordu. Daha önceden kullandığı ilaçlarla zehirlenen hastalar en çok bu ilaçları nörolojik hastalıklar nedeniyle kullanıyordu(n=27, %42.2). Toksik dozda ilaç alan hastalarda,toksik dozda ilaç almayan hastalara göre daha çok klinik bulgu ve belirti gelişmiş ve ortalama hastanede kalış süresi toksik dozda ilaç almayanlara göre daha uzun bulunmuştur. Hastaların büyük çoğunluğu (n=64,%75) acil gözlem ünitesinde takip edildikten sonra taburcu edilmiştir. Psikiyatri konsültasyonu sonrası hastaların %66?sı duygu durum bozukluğu tanısı almış %51.6?sına (n=33) SSRI grubu bir antidepresan ilaç önerilmiştir. Çalışmaya alınan hastaların, kendi isteği ile acil servisi terk eden 6 hasta dışında tamamı sekelsiz iyileşmiş, ölüm bildirilmemiştir. SONUÇ NBDAE ilaç alımlarında istemli ilaç alımı yüksek oranda gözükmektedir. En sık maruz kalınan ilaçlar eski kuşak antiepileptik ilaçlar olup bunlar içinde en sık görülen ilaçlar karbamazepin ve valproik asidtir. Kadınlarda antiepileptik ilaçlara maruziyet oranı erkeklerden fazladır.En sık zehirlenmeye maruz kalınan yaş grubu genç yetişkin hastalardır. Karbamazepin zehirlenmesinde valproik asid ve diğer antiepileptik ilaçlara oranla daha sık klinik bulgu ve belirti görülmekte, ancak karbamazepin zehirlenmeleri daha kısa süre acil serviste takip edilmektedir.NBDAEİ maruziyeti ile başvuran hastalarda en çok gözlenen psikiyatrik hastalık duygu durum bozukluklarıdır. Anahtar Kelimeler: Antikepileptik ilaç, zehirlenme, acil servis Objective:In this study, we aimed to reveal the demographic properties, clinical findings, treatment methods and clinical results of the exposures to non-benzodiazepine antiepileptic drugs (NBDAED) of the patients who applied to Ondokuz Mayıs University Faculty of Medicine Emergency Department (OMUFM) between 2006-2012.Materials and Methods:In this cross-sectional and descriptive study, we evaluated the age, sex, anticonvulsant types taken, poisoning types, cause of poisoning, clinical symptoms and findings, suggested and applied treatment methods, and results of the cases in which patients were exposed to the anticonvulsant drugs other than benzodiazepines and were admitted to the emergency department due to their poisoning complaints. The patient files were scanned to identify the occupation, treatment received after psychiatric consultation and the background of the patients. Data was filled in the standard information forms designed for this study and then installed to Statical Package for Social Science (SPSS) 15.0. The clinical symptoms and findings of the cases were scaled according to EAPCCT/IPCS Poisoning Severity Score. We checked whether the variables had normal distribution using the Kolmogorov Smirnov test. The descriptive statistics about the variables were presented as frequency, percentage, arithmetic mean ± standard deviation (SD), median (min-max). The comparisions of two groups were analyzed using the Mann Whitney U test and the comparisons of three or more groups were analyzed using the Kruskal Wallis test. Chi-square test was used to analyze to categorical variables.p<0.05 was considered as significant. Results: 64 patients out of 1987 patients analyzed in this study were exposed to poisoning with NBDAED. It was found that the rate of poisoning with NBDAED was 3.2 % of the total poisoning. 64.1 % of the patients were female (n=41), and 36 % were male (n=23). The average age of the patients was 28±10. The most admission to the hospital was in winter (n=19,32.9 %) and summer (n=17, 28.9 %). The most frequent admission was in October (n=9, 15.3 %) and the most frequent admission time was 18:00?23:59 (n=27, 42.2 %). 92.2 % of the patients took these drugs deliberately. 67.1 % of the admissions (n=43) were between the ages 18?29. 59.3 % of the poisoned patients were unemployed. It was found that most of the poisoned patients applied to a health care organization in 2 hours (n=25, 39.1 %) after the drug intake. Most of the patients were poisoned with old generation antiepileptics (n=52,81.3 %). In the poisonings, the most frequently taken drugs were carbamazepine on its own (n=22,34.4 %) and valproicasid (n=19, 29.7 %) on its own. When poisoning with old and X new generation drugs were compared according to their clinical symptom and findings, and the duration of hospital stay, no difference was identified. It was found out that more clinical symptoms and findings developed in cases in which carbamazepine was taken, but the duration of hospital stay was shorter. In poisioning with NBDAED, the rate of dug intake together with other drug groups was 45.3 % (n=29). The drugs taken together were mostly antidepressants and antipsychotics with the rate of 12.5 %. Alcohol drinking together with the drug intake was more frequent in males than females. 72 % of the patients (n=46) had used NBDAED beforehands. Those patients, who poisoned with the drugs which they had used beforehands, used these drugs because of norological diseases (n=27,42.2 %). More clinical symptoms and findings were developed in patients with toxic dose drug intake than those with non-toxic dose drug intake. Also the duration of hospital stay of patients with toxic dose drug intake was longer than those with non-toxic dose drug intake. Most of the patients (n=64, 75 %) were discharged from the hospital after their follow-up in the emergency department observation unit. After the psychiatric consultation, 66 % of the patients were diagnosed as mood disorder and 51.6 % of the patients (n=33) were advised SSRI group antidepressant. All patients in this study got well without any sequela except from 6 patients who left the emergency department voluntarily, and no death was informed. Conclusion:Deliberate drug intake is seen with a high rate in NBDAED intake. The most frequent drug exposure is to the old generation antiepileptic drugs and the most used drugs are carbamazepine andvalproic acid. The rate of exposure to the antiepileptic drugs is higher in females than males. The most frequent poisoning age group is the young adult patients. More frequent clinical symptoms and findings are seen in carbamazepine poisonings than those of valproic acid and other antiepileptic drugs, however the follow-up of the poisonings from carbamazepine in the emergency department is shorter. The most frequent psychiatric diseases seen in patients with exposure to NBDAED are the mood disorders. Key Words: Antiepileptic drug, poisoning, emergency department
