Çocukluk çağı akut lenfoblastik lösemileri`nde BCR/ABL ve TEL/AML1 gen düzensizliklerinin klinik ve hematolojik önemi

Abstract
Lösemiler, genetik değişikliklerin sonucu olarak hematopoetik hücrelerin klonal proliferasyon gösterdiği, en yaygın çocukluk çağı kanseridir. Çocukluk çağı lösemilerinde, tedavi öncesi yapılan genetik analizler, tanısal ve prognostik önem taşır.Bu çalışmada, ALL tanısı alan pediatrik hasta popülasyonunda, sitogenetik analizler ve floresan in situ hibridizasyon (FISH) çalışmaları ile saptanan akut lenfoblastik lösemiye özgün BCR/ABL ve TEL/AML1 ilişkili genetik belirteçlerin klinik, immünolojik, hematolojik ve morfolojik parametrelerle ilişkisinin ve prognostik önemlerinin belirlenmesi amaçlanmıştır.Çalışmamızda, Ocak 2005 ve Kasım 2013 tarihleri arasında, Ondokuz Mayıs Üniversitesi Çocuk Sağlığı ve Hastalıkları Anabilim Dalı Çocuk Hematoloji Bilim Dalında ALL tanısı ile izlenen ve genetik analiz için Pediatrik Genetik Tanı Laboratuarlarına başvuran 115 hasta retrospektif olarak incelenmiştir.Çalışmamızda, metafaz elde edilen 98 hastanın 61 (%62,2)' inde sitogenetik analiz sonucu normaldi, 37 (%37,7)' sinde ise sitogenetik aberasyon olduğu saptandı. Sadece metafaz sayısı optimal olan hastalar incelendiğinde sitogenetik aberasyon oranı %54.4 olarak saptandı. Hiperdiploidi ( >46 kromozom) %12 oranında saptandı. Bu grupta en yaygın kromozom kazancının 4, 14 ve 21. kromozomlar olduğu gözlendi. Hipodiploidi (<46 kromozom) ise %1,7 oranında saptandı. Sitogenetik aberasyonu olan hastaların relaps-excitus oranları sitogenetik sonucu normal olan hastalara göre belirgin yüksek bulundu.FISH analizi ile hastaların 60 (%52,2)' ında FISH aberasyonu olduğu saptandı. En yüksek anomali oranı %25,2 ile Polizomi 21 (amplifikasyon 21) olarak belirlendi. Hastaların %13,9' unda TEL/AML1 füzyonu, %3,5'inde BCR/ABL füzyonu saptandı. Polizomi 21 saptanan hastalarda TEL/AML1 füzyonu daha az oranda görüldü.Literatür verileri temelinde iyi prognostik faktör olarak kabul edilen TEL/AML1 füzyonuna sahip olan hastalarda hiç relaps saptanmadı. Sadece bir hasta sepsis nedeni ile excitus olmuştu. TEL/AML1 füzyon pozitif olan hastaların genel sağkalım sürelerinin uzun olduğu görüldü (en uzun süre takip edilen hasta için 96 ay). Kötü prognostik faktör kabul edilen BRC/ABL füzyon pozitif olan hastalarda ise excitus oranının yüksek olduğu ve sağkalım sürelerinin füzyon negatif olanlara göre belirgin daha kısa olduğu görüldü. Relaps ve excitus oranları yüksek ve sağkalım süreleri kısa olan bir grubu tanımlayan amplifikasyon 21' in, 'orta risk grubu' nu tanımlayıcı olabildiğini destekleyen veriler elde edildi.Çalışmanın sonucunda lösemi hastalarında genetik incelemelerin, diagnostik katkıları yanında prognostik değer taşıdığı; genel sağkalım/olaysız sağkalım ve yüksek/düşük risk belirlemelerinde etkin parametreler oldukları ve istatiksel anlamlılığı olan veriler sağladıkları görüldü. Akut lenfoblastik lösemide sitogenetik analizlerin yetersiz kaldığı füzyon gen tespitinde, kriptik düzensizliklerin tespitinde ve de özellikle mitotik indeksin düşük olduğu durumlarda, FISH analizi ile anlamlı ve tamamlayıcı veriler elde edildiği gözlendi. Özellikle prognozu belirlemede önemli olan füzyon dışı TEL ve AML 1 delesyon, duplikasyon ve amplifikasyonlarının tesbiti için ya da BCR ve ABL delesyon ve duplikasyonlarının tesbitinde, etkin genetik analiz yönteminin FISH yöntemi olduğu belirlendi. Leukemia is the most common form of childhood cancer which develops as a result of clonal proliferation in hematopoietic cells, triggered by genetic changes. Genetic analyses done before treatment holds a diagnostic and prognostic value for childhood leukemia.The aim of this study was to evaluate the prognostic significance of genetic markers detected by cytogenetic analysis and fluorescent in situ hybridization [BCR, ABL and TEL, AML1] which are specific for acute lymphoblastic leukemia (ALL) and to correlate the results with the clinical, hematological and morphological parameters in a population of children who were diagnosed with ALL.In this study, a sum of 115 patients who were diagnosed with ALL at Ondokuz Mayıs University Pediatric Hematology Department in between January 2005 and November 2013, and who were applied to Laboratories of Pediatric Genetic Department for genetic analysis were observed retrospectively.As a result, 37.7% of the patients had cytogenetic aberrations. When only the patients with optimal metaphase number were considered, cytogenetic aberration rate was 54.4%. Hyperdiploidy rate was 12%. Chromosome gains were mostly for chromosomes 4, 14 and 21. Hypodiploidy (<46 chromosome) rate was 1.7%. The relapse and mortality ratio were higher for the patients who had cytogenetic aberration.FISH aberration was found in 60 (52.2%) patients. Polisomy 21 (amplification 21) was the highest anomaly rate with 25.2%. TEL/AML1 fusion rate was found in 13.9% of the patients whereas BCR/ABL fusion rate was 3.5%. TEL/AML1 fusion rate was less in the patient group presenting with polisomy 21.No relapse was found in patients with TEL AML 1 fusion, which is accepted as a good prognostic factor according to the literature data. Only one patient was deceased because of sepsis. The general survival rate of the patients with a positive TEL/AML1 fusion was high ( 96 months for the longest observed patient). The mortality rate of the patients who had a BRC/ABL fusion, which is accepted as a poor prognostic factor, was higher and the survival rate was shorter when compared to the ones who did not have the fusion. The patient group with amplification 21 described a group with a higher rate of relapse and mortality and a shorter survival rate; thus showed that it could point to a group of a medium risk ALL patients.As a result of this study it was concluded that genetic analysis of patients with leukemia holds a prognostic value as well as a diagnostic aid; moreover, it is an efficient parameter in determining the general survival/event free survival and high/low risk evaluation and enables us to gather data with statistical value. It was observed that in situations in which the cytogenetic analysis was not efficienct enough to have optimal results or was totally inefficient such as the presence of a fusion gene, submicroscopic aberrations and a low mitotic index, FISH analysis supplied complementary and significant results. It was concluded that FISH was the effective type of genetic analysis, especially for the situations such as the presence of deletions, duplications or amplifications in a TEL/AML 1 or BCR/ABL fusion negative patient.
