Akut lösemi hastalarında kemoterapi sonrası gelişen oral mukozit ile prognozun ilişkisi

Abstract
Akut lösemi tedavisi sırasında kemoterapötik ajanların yan etkisi olarak sıklıkla oral mukozit (OM) gelişir. Oral mukoziti olan hastalar ağız içi enfeksiyonlara daha yatkın olurlar. Ayrıca OM hastanın beslenmesini, yutmasını, yaşam kalitesini etkiler. Bu çalışmada kendi kliniğimizdeki kemoterapi alan akut lösemi hastalarında OM gelişme sıklığını, OM gelişen hastaların kan ve mukozit yaralarında gelişen mikroorganizmaların tipini, mukozit gelişen ve gelişmeyen hastalardaki klinik ve laboratuar farlılıklarını ve risk faktörlerini analiz etmeyi amaçladık. Araştımaya alınan 72 erişkin yeni tanı akut lösemi hastası, kemoterapi sonrası OM gelişen ve gelişmeyen olarak iki gruba ayrıldı. 72 hastanın 27 (%37)' sinde OM geliştiği, OM gelişen hastalarda ise mukozitin tedaviye başlandıktan ortalama 6.3±3.4 gün sonra geliştiği ve ortalama 9.7±5.8 gün boyunca devam ettiği saptandı. İki grup arasında sosyodemografik özellikler, tanı, tedavi protokolü ve sigara içme durumları açısından anlamlı fark bulunmadı. OM gelişen hastaların ağız mukozası değerlendirilmesinde en sık %40.7 ile evre 2 mukozit saptandı. OM gelişen hastaların ağız sürüntü kültürlerinde en sık %14.8 (n:4) ile Pseudomonas Aeroginosa ve kan kültüründe ise en sık %7.4 (n:2) ile Staphylococcus Hominis üremesi saptandı. OM gelişen hastaların nötropeni süresi ortalama 32 gün iken, OM gelişmeyen hastalarda ise ortalama 22 gün saptanmıştır (p=0,02). OM gelişen hastalarda, tedavi öncesi serum albumin değerinin ise daha düşük, mukozit sırasında CRP ve LDH değerleri ise daha yüksek bulundu, p değerleri sırasıyla (p<0,001), (p<0,001),(p= 0,002). Ayrıca OM gelişen grupta, mukozit gelişmeyene göre ölen hasta sayısının daha fazla olduğu saptandı (p=0,04). OM gelişen grupta ölen hastaların 6'sında ağır mukozit (evre 3 ve 4), 3'ünde evre 1 mukozit saptanmıştır. Ağır mukozit (evre 3 ve 4) gelişen 12 hastanın ise 6 'sı ölmüştür. OM gelişen hastalarda ateş 7.1±4.4 gün çıkarken OM gelişmeyenlerde 4.1±4.1 gün ateş yüksekliği olmuştur (p= 0,002).Sonuç olarak; OM gelişen hastalarda nötropeninin daha uzun sürdüğü, LDH, CRP değerlerinin daha yüksek olduğu ve mortalite oranının da daha fazla olduğu bulgularına bakarak OM gelişiminin kemoterapi alan hastalarda prognostik bir önemi olduğunu söyleyebiliriz. Ayrıca kemoterapi öncesi albümin değerleri düşük olan hastaların OM açısından daha sıkı takip edilmesi gerekliliğini vurgulamak isteriz Chemotherapeutic agents frequently cause oral mucositis (OM) as a side effect during the treatment of acute leukemia. Patients with oral mucositis are generally more predisposed to intraoral infections. Additionally, OM affects the nutrition, swallowing ability, and quality of life of the patient. The present study aimed to analyze the frequency of OM in acute leukemia patients receiving chemotherapy in our clinic, the type of the microorganisms in the blood, mucositis wounds of the patients that developed OM, and the clinical and laboratory differences and the risk factors in the patients who with mucositis or not. Seventy-two adult patients with newly-diagnosed acute leukemia were included in the study and divided into two groups as patients with and without post-chemotherapy OM. Mucositis events occurred in 27 (37%) of 72 patients. Appearance time of the mucositis after the initiation day of the treatment was 6.3 ± 3.4 days and OM events were continued for 9.7 ± 5.8 days. There were no significant differences in sociodemographic data, diagnosis, treatment protocol, and smoking status between the two groups. The most common finding from the evaluation of oral mucosa of the patients with OM was stage 2 mucositis at a rate of 40.7%. The most common microorganism in the oral mucositis swab cultures was pseudomonas aeruginosa with a rate of 14.8% (n=4) and Staphylococcus hominis with a rate of 7.4% (n=2) in the blood cultures. The mean duration of neutropenia in the patients with OM was 32 days compared to a mean of 22 days in the patients without OM (p=0.02). The pre-treatment level of serum albumin was lower, and the levels of CRP and LDH during mucositis were higher in the patients with OM (p values, respectively: p<0.001, p<0.001and p=0.002). Additionally, it was established that the number of patients that died was higher in the group with OM compared to the group without mucositis (p=0.04). Severe mucositis (stages 3 and 4) was identified in six of the patients that died among the group with OM and stage 1 mucositis was identified in three patients. Six of 12 patients who developed severe mucositis (stages 3 and 4) died. The fever appeared on days 7.1±4.4 of the chemotherapy in the patients with OM, whereas fever was observed on days 4.1± 4.1 in the patients without OM (p=0.002).In conclusion, the authors may suggest that the development of OM has a prognostic significance in the patients receiving chemotherapy considering the extended duration of neutropenia, higher levels of LDP and CRP, and a higher rate of mortality in patients with OM. Furthermore, we would like to highlight that the patients with low levels of albumin prior to chemotherapy should be more closely followed for OM.
