Noninvaziv mekanik ventilasyon uygulanan hastalarda tam yüz ve oronasal maske etkinliğinin karşılaştırılması

Abstract
AMAÇ:Hasta uyumu ve maske seçimi NIMV uygulamasında en önemli etkenlerdir. NIMV tedavisinde hastaların tam yüz ve oronasalmaskeleritolaribilitesi ve tedavinin etkinliği değerlendirildi.GEREÇ VE YÖNTEM:Tip 2 solunum yetmezliği olan 60 hasta prospektifrandomize, tam yüz maske grubu (n=30) ve oronasal maske grubu(n=30) olarak iki gruba ayrıldı.Her iki grup yoğun bakımdan çıkış süresi, evde oksijen konsantratörü, BİPAP/CPAP, nebülizatör cihazı kullanımı ve süresi bakımından karşılaştırıldı. Tedavi öncesi ve tedavinin 1. , 6. , 24. , 72. saatinde arter kan gazı ve solunum sayısına bakıldı.Hastaların tedaviye uyumu hasta uyum skalası (HUS) ile tedavinin 1., 6., 24. saatinde değerlendirildi.BULGULAR: Tam yüz maske grubundan 8 hasta maske uyumsuzluğu ve klostrofobi gelişmesi, oronasal maske grubundan 2 hasta persistanhiperkapni nedeni ile çalışmadan çıkarıldı. Her iki grubun yoğun bakımda kalış süreleri benzerdi. Tam yüz maske grubunda tedavinin 1. ve 24. saatinde pH'da düzelme (p=0,042, p=0,033), tedavinin 72. saatinde PCO2' de azalma daha fazla (p=0,024) idi. Her iki grupta hasta uyumu ve solunum sayısı açısından farklılık yoktu. Tam yüz maske grubunda gözlerde yanma ve basınç şikayeti(p=0,025), oronasal maske grubundaise bası yarası daha fazlaydı (p=0,025).SONUÇ: Çalışmamızda vaka sayısı az olmasına rağmen tam yüz maske ile PCO2 'de düşüş ve PH' da düzelme istatiksel olarak anlamlı şekilde daha fazla bulunmuştur.Çalışmamızda her iki grupta hasta uyumu açısından fark tespit edilmemiş olup, hasta uyumunu daha ayrıntılı bir ölçekle değerlendirmek daha uygun olacaktır. Hasta uyumu olan olgularda tam yüz maskenin seçilmesinin, hiperkapnik solunum yetmezliği tedavisinde başarıyı artıracağı unutulmamalıdır.Anahtar Kelimeler:Hiperkapnik solunum yetmezliği, NIMV, hasta uyumu Aim: Patient compliance and mask selection are the most important factors in the application of NIMV. In the NIMV treatment, the tolerability of full face masks and oronasal masks and the effectiveness of the treatment have been evaluated.Materials and Method: Sixty patients who have respiratory insufficiency were separated into two groups as full face mask group (n=30) and oronasal mask group (n=30) in a prospective randomized way. Both groups were compared for the stay in intensive care unit.Before the treatment and in the 1st, 6th, 24th and 72nd hours of the treatment, arterial blood gas and respiratory rate were measured. The compliance of the patients to the treatment was evaluated in the 1st, 6th and 24th hours of the treatment with Patient Compliance Scale (PCS).Results: Eight patients of full face mask group were excluded group because of face-mask mismatch and claustrophobia, two patients of oronasal mask group were excluded from the group because of persistant hypercapni. The duration of stay groups in intensive care unit was similar in both groups. In full face group, recovery in pH was in the 1st and 24th hours of the treatment (p=0,0042; p=0,033), and the decrease in PCO2 was greater in the 72nd hour of the treatment (p=0,024). There was no significant difference in both groups in terms of patient compliance and respiratory rate. In full face group, there were more complaints about eye burning and pressure (p=0,025) while in oronasal mask group, there were more complaints about pressure sore (p=0,025).Conclusion: Although the number of cases in our study is very low, decrease in PCO2 and improvement in pH with full face masks were statistically significant. In our study, no difference was found in terms of patient compliance between the groups and it will be more effective to evaluate the patient compliance with a more detailed scale. It should not be forgotten that in cases which there is a patient compliance, selecting full face mask will increase the rate of success in hypercapnic respiratory insufficiency.Key words: Hypercapnic respiratory insufficiency, NIMV, patient compliance.
