Abstract
ÖZETAMAÇ: Deneysel olarak oluşturulan doku kayıplı korneal perforasyonlarda N-bütil-2-siyanoakrilat ve Ankaferd uygulamasının tedavi etkinliğini, klinik ve histopatolojik olarak değerlendirmek ve karşılaştırmak.GEREÇ VE YÖNTEM: Çalışmaya alınan 29 adet Yeni Zelanda tavşanının 34 gözü, rasgele olarak 4 gruba ayrıldı. 1. grup topikal Ankaferd ve kan uygulanan 10 tavşan, 2. grup sadece topikal otolog kan uygulanan 9 tavşan, 3. grup N-bütil-2-siyanoakrilat uygulanan 10 tavşan, 4. grup ise hiçbir uygulama yapılmayan 5 tavşandan oluşan kontrol grubu olarak belirlendi. Tavşanların bir gözünün santral korneasından `disposable punch` ile 1 mm boyutunda tam kat doku çıkarılarak perforasyon oluşturuldu. Uygulama sonrası tavşanların 1-2-3-7-14 ve 21. günlerdeki oftalmolojik muayenesinde korneal ödem, ön kamara (ÖK) derinliği, ön segmentte inflamasyon ve yara yerinden sızıntı varlığı değerlendirildi. 21. günde tüm tavşanlar sakrifiye edildi ve iyileşmiş yara dokusundaki korneal saydamlık mikroskobik olarak değerlendirildi. Histopatolojik inceleme; epitel tabakasının rejenerasyonu, fibrozisin şiddeti, yangısal hücre infiltrasyonunun varlığı ve vaskülarizasyon göz önüne alınarak yapıldı. Tüm deneklerin operasyon günü video ile postoperatif takip günleri ise fotoğraflanarak kayıt altına alındı.BULGULAR: Postoperatif ilk gün grup 1 ve 3' teki sızıntı yokluğu ve ÖK oluşumu grup 2 ve 4' e göre istatistiksel olarak anlamlıydı (grup 2'ye göre p=0.018, grup 4' e göre p<0.001). İlk bir haftada sızıntı varlığı görülen deneklerin istatistiksel karşılaştırmasında, grup 1' in diğer gruplara göre yara yeri sızıntı varlığının daha az olması anlamlı bulundu (grup 2' ye göre p=0.033, grup 3' e göre p=0.019, grup 4' e göre p=0.001). Benzer şekilde ilk bir haftadaki normal ÖK derinliği görülen deneklerin istatistiksel karşılaştırmasında, grup 1' de diğer gruplara göre ÖK sığlaşması daha az olup anlamlı bulundu (grup 2' ye göre p=0.011, grup 3' e göre p=0.003, grup 4' e göre p<0.001). Postoperatif ilk 14 gün boyunca, grup 1 ve 3' te kontrol grubuna göre daha az inflamasyon görülmesi istatistiksel açıdan anlamlıydı (ilk iki gün p=0.025, 3. gün p=0.003, 7. ve 14. gün p=0.001). Grup 2' nin kontrol grubuyla karşılaştırılmasında ise ilk üç gün anlamlı sonuç görülmezken, 7. ve 14. günlerde istatistiksel olarak daha az inflamasyon görüldü (ilk üç gün p>0.05, 7. gün p=0.016, 14. gün p=0.001). İlk gün korneal ödem açısından gruplar arasında anlamlı fark görülmedi (p=0.212). 2. gün grup 1' de grup 2 ve 4' e göre daha az korneal ödem görülürken (p=0.038), 3. gün sadece grup 1' in kontrol grubuyla karşılaştırılmasında korneal ödem açısından anlamlı fark bulundu (p=0.031). 14. gün ilk üç grupta kontrol grubuna göre korneal ödem yokluğu istatistiksel açıdan anlamlıydı (p=0.006). Ayrıca, grup 3' de 4 tavşana tekrar yapıştırıcı uygulaması yapılırken, diğer gruplarda ek uygulama yapılmadı ve bu fark istatistiksel olarak anlamlı bulundu (p=0.012). Grup 1' in diğer gruplarla karşılaştırılmasında, iyileşmiş perforasyon alanındaki korneal saydamlık grup 1 lehine anlamlı bulundu (p=0.027). Histopatolojik incelemede; grup 1' in grup 2, 3 ve 4 ile karşılaştırılmasında, fibrozis yoğunluğunun ve inflamatuvar hücre infiltrasyonun daha az görülmesi istatistiksel açıdan anlamlıydı (grup 2' ye göre; p=0.004 ve p=0.012, grup 3' e göre; p=0.002 ve p=0.007, grup 4' e göre;p=0.022 ve p=0.001).SONUÇ: Doku kayıplı korneal perforasyonlarda Ankaferd uygulaması, oküler yüzeyde ciddi bir yan etki ve tekrar uygulama gereksinimi olmaksızın hızlı bir iyileşme sağlamaktadır. Oldukça yaygın kullanılan N-bütil-2-siyanoakrilat'a göre, `Ankaferd ` daha başarılı bulunmuştur. Ayrıca, histopatolojik olarak diğer gruplara göre çok daha düşük seviyede stromal fibrozise yol açtığı görüldü. Deneysel olarak daha büyük çapta oluşturulan korneal doku defektlerinde, tedavi etkinliğinin diğer cerrahi doku yapıştırıcılarıyla karşılaştırılması uygun olacaktır.ANAHTAR KELİMELER: Ankaferd, N-bütil-2-siyanoakrilat, korneal perforasyon ABSTRACTOBJECTIVE: To assess and to compare the treatment efficiency of N-butyl-2-cyanoacrylate and Ankaferd clinically and histopathologically in experimentally formed corneal perforations with tissue loss. MATERIAL AND METHOD: 34 eyes of 29 New Zealand rabbits included in the study were randomly grouped in 4. The first group included 10 rabbits which received Ankaferd and blood, the second group included 9 rabbits which received only topical autologous blood, the third group included 10 rabbits which received N-butyl-2-cyanoacrylate and the fourth group included 5 rabbits which did not receive any application. Perforation was formed by taking out a full layer of 1 mm tissue from the central cornea of rabbits' eyes with `disposable punch`. On the post-application first, second, third, seventh, fourteenth and twenty first days ophthalmologic examination of rabbits, corneal edema, anterior chamber (AC) depth, inflammation in the anterior segment and presence of leakage from the wound were assessed. On the 21st day, all the rabbits were sacrificed and corneal transparency on the healed wound tissue was assessed with microscopy. Histopathological examination was conducted by taking into consideration the regeneration of epithelium layer, the intensity of the fibrosis, presence of inflammatory cell infiltration and vascularization. The operation days of all the subjects were videoed while their postoperative follow-up days were recorded with photos. RESULTS: Absence of leakage and formation of AC in groups 1 and 3 in the preoperative first day were statistically significant when compared with groups 2 and 4 (p=0.018 when compared with group 2, p<0.001 when compared with group 4). In the statistical comparison of the subjects which were found to have leakage in the first week, it was significant that Group 1 had less wound leakage when compared with other groups (p=0.033 compared with group 2, p=0.019 compared with group 3, p=0.001 compared with group 4). Similarly, in the statistical comparison of the subjects which were found to have normal AC depth in the first week, group 1 was found to have less AC shallowing when compared with the other groups (p=0.011 compared with group 2, p=0.003 compared with group 3, p<0.001 compared with group 4). Less inflammation in groups 1 and 3 during the first 14 days when compared with the control group was statistically significant (first two days p=0.025, third day p=0.003, 7th and 14th day p=0.001). In the comparison of the second group with the control group, no significant results were seen in the first three days, statistically less inflammation was seen on the 7th and 14th days (the first three days p>0.05, 7th day p=0.016, 14th day p=0.001). No significant difference was found between groups on the first day in terms of corneal edema (p=0.212). On the 2nd day, less corneal edema was seen in group 1 when compared with groups 2 and 4 (p=0.038), significant difference was found in the comparison of group 1 with the control group (p=0.031). On the 14th day, absence of corneal edema in the first three groups was statistically significant when compared with the control group (p=0.006). In addition, readhesion application was conducted on 4 rabbits in the third group, while no extra applications were conducted in the other groups and this difference was statistically significant (p=0.012). In the comparison of group 1 with other groups, corneal transparency in the recovered perforation area was found to be in favor of group 1 (p=0.027). In terms of histopathological examination, when group 1 was compared with groups 2, 3 and 4, volume of fibrosis and occurrence of less inflammatory cell infiltration were statistically significant (p=0.004 and p=0.012 when compared with group 2; p=0.002 and p=0.007 when compared with group 3; p=0.022 and p=0.001 when compared with group 4).CONCLUSION: The application of Ankaferd in corneal perforations with tissue loss causes fast recovery without a serious side effect on ocular surface and the need for reapplication. `Ankaferd` has been found to be more successful when compared with N-butyl-2-cyanoacrylate, which is commonly used. In addition, a much lower level of stromal fibrosis was seen when compared with other groups. It will be suitable to compare treatment efficiency with other surgical tissue adhesives in corneal tissue defects which are formed experimentally in bigger sizes.KEY WORDS: Ankaferd, N-butyl-2-cyanoacrylate, corneal perforation
