Sertralin`in deneysel olarak indometazinle oluşturulan mide mukozal hasarı üzerine etkisi

Abstract
Giriş:Gastrointestinal sistemde meydana gelen mukozal hasar veya ülserler, halen dünyada ve ülkemizde önemli bir sağlık sorunudur. Koruyucu mekanizmalarla hasar verici mekanizmalar arasındaki dengenin bozulmasıyla oluşan hastalıkta birçok neden suçlanmaktadır. Özellikle antienflamatuar ilaçlar gibi ilaçların kontrolsüz kullanılması, stres, mukozal iskemi, depresyon önemli etyolojik faktörlerdir. Ayrıca serbest oksijen radikalleri, stres ve antienflamatuar ilaçlar ile ilişkili mukozal hasarda patojen faktör olarak bildirilmektedir.Gastrointestinal sistemde mukozal hasar ve ülser, psikolojik olarak kolay etkilenebilenlerde ve depresyona eğilimi olanlarda sık olarak ortaya çıkmaktadır. Bu deneysel çalışmada, sertralin'in indometazin ile oluşturulan mide mukozal hasarı üzerine koruyucu bir etkisinin olup olmadığı ve varsa bu etkinin oksidan-antioksidan parametreler ile ilişkisi araştırıldı.Materyal ve Metod:Bu çalışmada Atatürk Üniversitesi Tıbbi Deneysel Uygulama ve Araştırma Merkezi'inden (ATADEM) temin edilen toplam 36 adet erkek Sprague Dawley türü rat kullanıldı. Ratlar standart şartlar altında beslenip muhafaza edildi ve randomize olarak sertralin, indometazin, famotidin ve sham grubuna ayrıldı. Deneyler Atatürk Üniversitesi Tıp Fakültesi, Farmakoloji AD. laboratuvarında, Biyokimyasal analizler Atatürk Üniversitesi Tıp Fakültesi, Biyokimya AD'ında yapıldı. İndometazin ile oluşturulan gastrik hasar üzerine sertralin'in antiülserojen etkileri makroskopik olarak ölçüldü. Biyokimyasal olarak, mide dokusundan elde edilen numunelerde Malondialdehid (MDA), Glutatyon (tGSH) düzeyleri ve Superoksit Dismutaz (SOD), Glutatyon S-transferaz (GST) ve Glutatyon Peroksidaz (GPx) enzim aktiviteleri spektrofotometrik yöntemle tayin edildi.Bulgular:Makroskobik incelemede, sham grubunda ülser saptanmazken, indometazin almış bütün gruplarda ülser saptandı. Ülser yüzey alanı indometazin grubunda ortalama olarak 33 mm2 idi. Famotidin grubunda antiülser etki % 97 oranında ve en yüksek iken, sertralin grubunda en yüksek antiülser etki 20 mg/kg sertralin verilen ratlarda % 57,6 olarak saptandı.MDA düzeyi Sham grubundaki ratlarda en düşük, indometazin grubunda en yüksek düzeyde saptandı. Sertralin grubunda ise en düşük MDA düzeyi 20 mg/kg Sertralin alan ratlarda saptandı. Bu değerin indometazin grubu değeri ile arasındaki fark istatistiksel olarak anlamlı idi (p<0.001).tGSH düzeyi ile SOD, GST ve GPx enzim aktivitesi ise sham grubunda en yüksek iken, indometazin grubunda en düşük bulundu. Sertralin grubunda tüm antioksidan parametrelerde, 20 mg/kg sertralin alan ratlarda en yüksek değerler saptandı. Bu değerlerin indometazin grubu değerleri ile arasındaki fark istatistiksel olarak anlamlı idi (p<0.001).Sonuç:Sonuç olarak mukozal hasarların tedavisinde, sertralinin 20 mg/kg dozunda faydalı etkilerinin olduğu saptandı. Sertralin bu olumlu etkilerini muhtemelen oksidatif stresi azaltarak ve antioksidan mekanizmaların etkisini artırarak sağlamaktadır. Sertralin'in bu amaçla kullanmasından önce randomize prospektif klinik çalışmalar yapılması gerekmektedir.Anahtar sözcukler: Mukozal hasar, indometazin, sertralin, oksidan, antioksidan ve rat Introduction:Mucosal damage or ulcers occurring in gastrointestinal system is at present an important health problem for both the world and our country. Many reasons are blamed for the disease that results from the imbalances among protective and harmful mechanisms. Taking medicines uncontrolled, particularly anti-inflammatory drugs, stress, mucosal ischemia, depression are important etiologic factors. In addition, free oxygen radicals are reported to be pathogenic factors of mucosal damage related with stress and anti-inflammatory drugs.Mucosal damage and ulcers emerging in gastrointestinal system are observed typically among people being psychologically susceptible and having a tendency to depression. In this experimental study, it was researched that whether sertraline affects gastric mucosal damage formed with indomethacin as a protective or not and if so, the relationship amongst this effect and oxidant-antioxidant parameters was also investigated.Materials and Methods:A total of 36 male Spraque Dawley rats provided from Experimental Medical Application and Research Center (ATADEM) of Atatürk University were used in this study. Rats were nourished and sustained under standard circumstances and were divided randomly in 4 groups including indomethazin, famotidin, sertralin and sham operation, The experiments were performed in the Laboratories of the Department of Pharmacology, and biochemical analyses were performed in the Laboratories of the Department of Biochemistry of Atatürk University Medical School. The anti-ulcerogenic effects of sertaline on indomethacin induced gastric mucosal damage were observed macroscopically. Biochemically, Malondialdehid (MDA) and Glutatyon (tGSH) levels and Superoksit Dismutaz (SOD), Glutatyon S-transferaz (GST) and Glutatyon Peroksidaz (GPx) enzyme activities at the samples obtained from gastric tissue were measured spectrophotometrically.Results:While there were no ulcers detected at macroscopic examination, in the Sham group, all groups received indomethacin had ulcers. The median surface area of ulcer was 33 mm2 in the indomethacin group. The antiulcer effect in the Famotidin group was the highest with a rate of 97% and the highest antiulcer effect in the Sertralin group of was in rats given 20 mg/kg Sertralin with a rate of 57.6%.MDA levels were observed to be lowest in Sham group and highest in indomethacin group. The lowest level of MDA in sertralin group of was observed in the rats given 20 mg/kg sertralin. The difference between this value and the group of indomethacin was statistically significant (p<0.001).It was found that the level of tGSH and the activities of SOD, GST and GPx were the highest in Sham group but were the lowest in indomethacin group. In Sertralin group, the highest values for all antioxidant parameters were determined in the rats given 20 mg/kg sertralin. The differences among these values and the outcomes of the indomethacin group were significant statistically (p<0.001).Conclusion:As summary, for the treatment of mucosal damage, Sertralin at doses 20 mg/kg had beneficial effects. Sertralin provides these positive effects probably by decreasing oxidative stress and increasing the effects of antioxidant mechanisms.
