Hipernatremik dehidratasyonlu yenidoğanların uzun dönem nörolojik bulgularının değerlendirilmesi

Abstract
AMAÇ: Sodyum ekstrasellüler sıvının en önemli katyonudur. Serum sodyum düzeyinin 145 mEq/L'nin üzeri olması hipernatreremi olarak tanımlanır. Biz yenidoğan döneminde hipernatremik dehidratasyonun pronozunu araştırmak istedik.MATERYAL METOD: 2003-2006 yılları arasında Atatürk Üniversitesi Tıp Fakültesi Yenidoğan servisinde yatan 62 hipernatremik dehidratasyonlu bebeğin dosyası retrospektif olarak değerlendirildi. Bunlardan 13 tanesi (%19) kaybedilmişti. Geri kalanların 30 tanesine (%48) ulaşıldı ve bu çalışmaya dahil edildi. Ayrıntılı nörolojik değerlendirme ve Denver II psikomotor nörogelişimsel test yapıldı.BULGULAR: Çalışmamıza alınan hastaların en sık başvuru nedeni %63.3 ile emmeme, %16.6 ile huzursuzluktu. Doğum tartısına göre sıvı kaybı %23,8 `di. Hastalar ortalama 4,3 yıl takip edildi. Onüç olgunun Denver II testinde gerilik tespit edildi. Üç hastada aşikar nörolojik anormalite mevcuttu (%10). Denver testi anormal olanlarda ortalama serum sodyum düzeyi 177mEq/L iken, Denver testinin normal bulunduğu olgularda 169,7 mEq/L idi. (p<0.05). Ölen olgularda ortalama sodyum düzeyi 180 mEq/L iken, yaşayan olgularda 173 mEq/L idi (p<0.05). Aşikar nörolojik bulguları olan üç hastanın serum sodyum düzeyleri çok yüksekti. İki olguda (166 mEq/L ve 181 mEq/L) hipernatreminin hızlı düzeltildiği tespit edildi (1 ve 2 gün).Sonuç olarak; yenidoğanlarda hipernatremik dehidratasyon nörolojik morbidite ve mortaliteye yol açabilir. Hipernatreminin hızlı düzeltilmesi de morbiditeye katkıda bulunabilir. Nörolojik bulguların çoğu ancak özel testlerle ortaya çıkarılabilir. Hastaların fizik tedavi ve rehabilitasyon imkanlarından erken faydalanabilmeleri açısından hipernatremik dehidratasyona maruz kalmış çocukların yakın takiplerinin yapılması uygun olacaktır. AİM: Sodium is the most important cation in the extracellular fluid. Hypernatremia is defined as being over serum sodium level of 145 mEq / L. We wanted to investigate the prognosis of hypernatremic dehydration in the newborn period.METHODS: The files of 62 infants with hypernatremic dehydration in neonatal service of Faculty of Medicine of Ataturk University were retrospectively reviewed between 2003-2006. 13 files of them (19%) had been lost. 30 files of the remaining of them (48%) were obtained and included in this study. Detailed neurological examination and Denver II psychomotor neurodevelopmental testing was performed.FİNDİNGS: The most frequent cause of admission is ; sucking problem (63.3%), and the other reason is restlessnes (16.6% ). Fluid loss was 23.8% compared to birth weight. Patients were followed for an average of 4,3. years. Thirteen patients were retarded at the Denver II test. Neurological abnormalities were evident in three patients. Mean values of Serum sodium in patients with abnormal Denver test was 177mEq / L, the mean value of patients with normal Denver test was 169.7 mEq / L. (p <0.05). The average sodium level in patients who died was 180 mEq / L, the average level was 173 mEq/L in the surviving patients (p <0.05). Three patients with obvious neurological findings had high serum sodium levels (166 and 181 mEq/L). In addition, two of them were found to be corrected for hypernatremia quickly (1and 2 days).As a result, hypernatremic dehydration in infants can cause neurological morbidity and mortality. Rapid correction of hypernatremia can contribute to the morbidity. Most of the neurological symptoms may be revealed only with special tests.Patients suffered from hypernatremic dehydration should be followed closely to benefit from early physical therapy and rehabilitation facilities.
