Total tiroidektomi yapılan hastalarda çıkarılan tiroid bezi hacmi ile postoperatif ses bozukluğu arasındaki ilişki

Abstract
Tiroid bezinin hastalıkları toplumda sık görülmektedir. Tiroid bezininbüyümesinde değişik faktörler rol oynar. Tiroidektomi endemik bölgelerde en sıkyapılan ameliyatlardan birisidir. Ameliyatlardan sonra çeşitli komplikasyonlarolabilmektedir. Biz ses bozukluğunun tiroid hacmi ile bir bağlantısı olup olmadığınıaraştırmak için bu çalışmayı planlandık. Mayıs 2008 ve Mayıs 2009 tarihleri arasındaguatr nedeniyle total tiroidektomi ve near total tiroidektomi yapılan 60 hastada çıkarılantiroid bezi hacmi ve postoperatif ses bozukluğu arasındaki ilişkiyi inceledik. Rezekeedilen tiroid bezi hacimleri elipsoid formül ile hesaplandı. Postoperatif ses değişiklikleriindirekt laringoskopi ve subjektif ses değerlendirme anketi ile değerlendirildi. 5 hastadases bozukluğu saptandı. Ses bozukluğu saptanan hastalar ve saptanmayan hastalarortalama tiroid hacimleri yönü ile karşılaştırıldı. Aralarında istatiksel olarak anlamlı farkbulunmadı (P=0.972). Tiroidektomi sonrası geçici hipokalsemi gelişen hastalar tiroidhacimleri yönü ile karşılaştırıldı anlamlı fark bulunmadı (P=0.613). Operasyondakikanama miktarı tiroid hacimleri ile karşılaştırıldı. İstatiksel olarak anlamlı farkbulunmadı (P=0.784).Sonuç olarak tiroidektomi sonrası bazı komplikasyonlar olabilmektedir. Bunlararasında ses bozukluğu da yaşanabilir. Bu komplikasyon riskinin bizim çalışmamızdatiroid bezi hacmine bağlı olmadığı tesbit edildi. Vaka sayısının geniş olduğu serilerdeistatiksel olarak anlamlı sonuçlar alınabileceği kanısındayız. En önemli sonuçkomplikasyonları en aza indirmek için azami tedbirleri almaktır.Anahtar kelimeler: Tiroid, Hacim, Ses, Subjektif Değerlendirme Thyroid gland diseases are seen more frequently in population. Different kindsof factors play role on the enlargement of the thyroid gland. Thyroidectomy is the mostconstant operation made in endemic regions. After this operation, various complicationscan occur. We planned such a study in order to search whether voice distortion isrelated with thyroid volume or not. For this aim, we have examined thyroidectomizedthyroid glands of 60 patients made total thyroidectomy and near total thyroidectomyowing to goiter between May 2008 and May 2009 to understand the relations betweenthyroid gland volume and postoperative voice distortion. Resected thyroid glandvolumes were calculated with the ellipsoid formula. Postoperative voice changes wereassessed with indirect laryngoscopy and subjective voice rating inquiry. Voicedistortion was detected for 5 patients. The patients detected voice distortion or not werecompared in terms of their mean thyroid volumes. It was not found any differencestatistically among them (P=0.972). The patients developing temporary hypocalcaemiaafter the thyroidectomy were compared in the way of their thyroid volumes and notfound any significant difference (P=0.613). The amount of haemorrhage in theoperation was compared with thyroid volumes. There was not found any statisticallysignificant difference (P=0.784).İn conclusion, some complications can be seen after thyroidectomy. Amongthem, voice distortion can also be experienced. It was not found in our study that thiscomplication risk depended on thyroid gland volume. However, we believe that it canbe found statistically significant outcomes in series having more cases. The importantpoint is taking necessary precautions to minimize complications.Keywords: Thyroid, Volume, Voice, Subjective Assessment
