Abstract
Amaç: Literatürde, toplum örneklemlerine ek olarak DEHB tanılı hastalarda da akran zorbalığı oranlarına ilişkin veriler sunan tanımlayıcı çalışmalar bulunmaktadır. Kesitsel olarak tasarlanmış çalışmalarda, DEHB'ye yönelik ilaç alan ve almayan DEHB tanılı hastalar, akran zorbalığı davranışları açısından karşılaştırılmıştır fakat, DEHB tanılı çocuk ve ergenlerde DEHB tedavisi ile akran zorbalığının neden sonuç ilişkisinin ortaya konabilmesi için takip çalışmalarına ihtiyaç duyulmaktadır. Bu çalışmada; DEHB semptom şiddeti ile akran zorbalığı ilişkisininin ve DEHB tedavisinin akran zorbalığına olan etkisininin araştırılması amaçlanmıştır. Yöntem: 8-16 yaşları arasında DEHB tanısı konulup tedavide metilfenidat başlanan ve son birkaç ay içerisinde akran zorbalığı davranışlarına dahil olduğu tespit edilen 50 hasta ile çalışma tamamlanmıştır. Tedavi öncesi ve sonrası, DEHB semptom şiddetlerini ölçmek için Conners Ebeveyn ve Öğretmen Derecelendirme Ölçekleri ile akran zorbalığı durumlarını ölçmek için Olweus Öğrencilerde Akran Zorbalığı Anketi uygulanmıştır. Ardından, 3 ay ara ile elde edilen bu veriler karşılaştırılmış ve DEHB tanılı çocuk ve ergenlerde metilfenidat tedavisinin akran zorbalığı üzerine olan etkisi incelenmeye çalışılmıştır. Bulgular: Tedavi öncesi ve sonrası, Conners Ebeveyn ve Öğretmen Derecelendirme Ölçeklerinin toplam ve tüm alt parametrelerine ait puanlarında istatistiksel anlamda fark tespit edilmiştir (p<0,001). Tedavi öncesinde, CEDÖ-DS (p=0,004), CEDÖ-HD (p=0,016) ve CÖDÖ-DS (p=0,022) alt parametre puanlarının, zorba ve kurbanlar arasında anlamlı farklılık gösterdiği saptanmıştır. Tedavi öncesi ve sonrasına ait Olweus Öğrencilerde Akran Zorbalığı Anketi verilerine göre, zorba (p=0,008) ve zorba/kurban (p=0,039) olma oranlarında istatistiksel anlamda azalma saptanırken kurban olma oranlarında anlamlı farklılık bulunmamıştır.Sonuç: Bu çalışmayla; DEHB tanılı çocuk ve ergenlerde CEDÖ-DS, CEDÖ-HD ve CÖDÖ-DS alt parametre puanlarının zorba olmayı öngörebileceği ve DEHB tanılı zorba ve zorba/kurbanların metilfenidat tedavisi sonucunda zorba olmaktan kurtulabileceği tespit edilmiştir. Objective: In addition to the society samples, descriptive studies presenting evidence concerning the rates of peer bullying among patients with ADHD are available in the literature. In cross-sectional studies, patients taking medication for ADHD or not but diagnosed with ADHD are compared in terms of peer bullying. Yet, there has been no longitudinal study conducted to see the peer bullying in this same group of patients before and after the treatment. Current study aims to investigate the relationship between the severity of ADHD symptoms and peer bullying as well as the effect of ADHD treatment on the peer bullying. Method: 50 patients between the ages of 8-16 who are diagnosed with ADHD, started using metilfenidat for treatment and are observed to have been exposed to or displayed peer bullying behavior over the past few months were included in this study. In order the evaluate the severity of the ADHD symptoms, Conners Parent and Teacher Rating Scales were used while Olweus Bullying Questionnaire for students and parents and teachers peer bullying forms were employed to evaluate peer bullying in pre-treatment and after a three-month treatment period. Then, the results obtained in three months time were compared and the effect of metilfenidat treatment on peer bullying among the child and adolescent patients diagnosed with ADHD was investigated. Results: In pre and post medication, statistically significant differences were found in the total the scores of the CPRS and CTRS as well as the scores related to all sub parameters (p<0,001). In pre-medication, it has been found out that the scores of the sub parameters CPRS-Oppositional (p=0,004), CPRS-Hyperactive Impulsive (p=0,016) and CTRS-Oppositional (0,022) are statistically significant between the bullies and the victims. According to the data driven from the Olweus Bullying Questionnaire for students, there is a statistically significant decrease between the ratios of being a bully (p=0,008) and bully/victim (p=0,039) in pre and post medication but no significant difference the ratios of being a victim. Conclusion: Current study concludes that the scores concerning the sub parameters CPRS-Oppositional, CPRS-Hyperactive Impulsive and CTRS-Oppositional may predict being a bully as well as bullies and bullies/victims diagnosed with ADHD can recover from being a bully following the metilfenidat treatment.
