Abstract
GİRİŞ VE AMAÇ: Çalışan sağlığını korumak amacıyla, işin tehlike derecesine göre, 6331 sayılı İş Sağlığı ve Güvenliği (İSG) Yasası'nda belirtilen aralıklarla çalışanlara periyodik sağlık muayenesinin yapılması zorunlu kılınmıştır. Bu çalışmada Samsun ilindeki bir ortak sağlık ve güvenlik birimi (OSGB) tarafından hizmet verilen Samsun ili Büyükşehir Belediyesi saha çalışanlarının sağlık muayenelerinin kayıt altına alınan sonuçlarının değerlendirilmesi ve işyeri ortamının sağlığı etkileyen faktörleri açısından yapılan işlemlerin uygunluğunun saptanması amaçlanmıştır.GEREÇ VE YÖNTEM: Tanımlayıcı tipteki bu çalışmada araştırmanın evrenini Samsun ili Büyükşehir Belediyesi tüm saha çalışanları oluşturmaktadır. Dosyasında işe giriş / periyodik muayene formunda bir ya da birden fazla eksik bilginin olması ve işe başlama süresinin 3 yılın altında olması dışında kalan toplam 494 saha çalışanının tümü herhangi bir örneklem yapılmadan araştırma kapsamına alınmıştır. OSGB firmasının 01 Ocak- 31 Aralık 2016 tarihleri arasındaki belediye saha çalışanlarının periyodik muayenelerine ait kayıtları çalışmanın verilerini oluşturmuştur. İstatistiksel analizlerde Kolmogrov-Smirnov, Ki Kare, Mann Whitney U Testi ile Kruskal-Wallis Testleri kullanılmıştır.BULGULAR: Çalışma kapsamına alınan toplam 494 çalışanın, 476(%96,4)'sı erkek, 18(%3,6)'i kadın olup, yaş ortancası 38(23-58)dir. Çalışanların iş kollarına göre dağılımı; 263(%53,2)'ü SASKİ (Samsun Su ve Kanalizasyon İdaresi) kurumu, 84'ü (%17) Doğaltaş Kesme, 83'ü (%16,8) Fen İşleri ve 64'ü (%13) Makine İkmal şeklindedir. Çalışanların %46,4'ünün şişman ve %23,3'ünün obez olduğu; en sık el/parmak kesisi, düşme, yanık ve elektrik çarpması sebebiyle iş kazasına maruz kaldıkları; %60,9'unun aktif olarak sigara kullandığı, erkeklerde sigara kullanımının kadınlara göre daha fazla olduğu (p<0,001)saptandı. Çalışanların sistolik kan basıncı (KB) değerleri; erkeklerde kadınlara göre (p<0,001), 50-59 yaş aralığındakilerin diğer çalışanlara göre (p<0,001) ve şişman olan çalışanların zayıf olan çalışanlara göre (p=0,011) daha yüksek saptanıp, bu bulguların istatiksel olarak anlamlı olduğu, % 56,1 çalışana son 5 yıl içerisinde tetanoz; %71,5 çalışana ise hepatit B aşısının yapılmadığı saptandı. Çalışma verilerine göre çalışanların %11,9'unda hafif derecede, %0,4'ünde orta derecede böbrek fonksiyon bozukluğu; %10,3'ünde işitme ile ilgili patoloji olduğu; işitme bozukluklarının en fazla SASKİ kurumu çalışanlarında görüldüğü ancak farklı kurumlarda çalışanlar arasında işitme bozukluğu sıklığında anlamlı bir farklılık göstermediği saptandı (p=0,261). Çalışanların %2,2'sinde akciğer grafilerinde patoloji gösterilirken, solunum fonksiyon testi (SFT) sonuçlarına göre %4,4 çalışanda restriktif veya obstruktif havayolu hastalığı olduğu, sigara içme ile SFT bozukluğu arasında istatiksel olarak anlamlı bir ilişkinin olmadığı saptandı (p=0,688). SONUÇ VE ÖNERİLER: Bu çalışmanın sonucunda, periyodik sağlık muayenelerinde çalışanlardan istenen testlerin seçiminde işyeri ortam faktörleri kadar, hipertansiyon (HT), obezite ve kronik hastalıklar gibi bireysel risklerin de dikkate alınması gerektiği saptanmıştır. Sahada ve vardiyalı işlerde çalışan, düzensiz ve dengesiz beslenen çalışanlara uygun bir diyet programı hazırlanmalıdır. Çalışanların tetanoz aşılarının üzerinden 5 yıl geçmişse tekrar aşılanması, çalışanlara işyeri hekimleri tarafından sigaranın zararlı etkilerinin anlatılması ve çalışanların sigara bırakmaya teşvik edilmesi sağlanmalıdır. Özellikle 50 yaş üstü, vücut kitle indeksi 25'in üzerinde ve erkek çalışanlarda hipertansiyon açısından dikkatli olunmalı; diyabetik çalışanlarda da gelişebilecek ani hipoglisemilere bağlı iş kazalarını engellemek adına vinç, yüksekte çalışma gibi işlerden uzak tutulması sağlanmalıdır. Periyodik sağlık muayenelerinde; çalışanlar, organ fonksiyonlarını gösteren hesaplamalarla değerlendirilmelidir. Saha çalışanlarında odyolojik tetkiklerin; tozlu işyerlerinde çalışanların da yıllık SFT'lerinin yaptırılması sağlanmalıdır.Anahtar Kelimeler: İş Sağlığı ve Güvenliği, Periyodik Muayene, 6331 Sayılı İş Sağlığı ve Güvenliği Yasası, Saha Çalışanları INTRODUCTION AND AIM: In order to protect employee health, it is obligatory to perform periodical health examinations according to the degree of danger of work, at the intervals specified in the Occupational Health and Safety (OHS) Law No. 6331. In this study, it is aimed to evaluate the results of the health examinations of field employees of Samsun Metropolitan Municipality, which is served by a Common Health and Safety Unit (CHSU) in Samsun province and to determine the appropriateness of the transactions made in terms of the factors affecting the health of the workplace environment.MATERIALS AND METHODS: In this descriptive study, the universe of the research is constituted by all the field employees of Samsun Metropolitan Municipality. A total of 494 field employees were included in the survey without any sampling, except that there was one or more incomplete information on the entry / periodic health examination form in the file and the start-up time was less than 3 years. CHSU company's records of periodic examinations of municipal field employees between 01 January and 31 December 2016 formed the data of the study. Kolmogorov-Smirnov, Chi-Square, Mann Whitney U and Kruskal-Wallis Tests were used for statistical analysis. RESULTS: A total of 494 employees, 476(96.4%) were male and 18(3.6%) were females and median age was 38(23-58) year, included the study. Distribution of employees by business line; was 263(53.2%) SAWSA (Samsun Water and Sewage Administration) Institution, 84(17%) Natural Stone Cutting, 83(16,8%) Science Works and 64(13%) Machinery Supply. It was determined that, 46.4% of the employees were overweight and 23.3% were obese; most frequently exposed to work accidents due to hands / finger strikes, falls, burns and electric shocks; 60.9% actively used cigarettes, and cigarette usage was more in men then women (p <0.001). Employees' systolic blood pressure values were higher and found statistically significant at males than females (p <0,001), in the 50-59 age range than the other age ranges (p <0,001) and at overweights than weaks (p = 0,011); it was found that 56.1% of employees did not have tetanus vaccination in the last 5 years and %71.5 of employees didn't have hepatitis B vaccination. According to the study data; it was found that 11.9% of the employees had mild and 0.4% had moderate renal dysfunction; 10.3% had hearing pathology; hearing impairments were mostly observed in the employees of SAWSA Institution; but the frequency of hearing impairment did not show significant difference among the employees in different institutions (p = 0,261). While 2.2% of the employees showed pathology on chest radiographs; according to the results of pulmonary function test (PFT), 4.4% of the employees had restrictive or obstructive airway disease; but there was no statistically significant relationship between smoking and PFT disorder (p=0.688). CONCLUSION AND RECOMMENDATIONS: As a result of this study, it was determined that the individual risks such as hypertension (HT), obesity and chronic diseases should be taken into consideration as much as workplace environmental factors, in the selection of the desired tests from the employees during the periodical health examinations. An appropriate dietary program should be prepared for working at the field and shift jobs employees who has irregular and unbalanced diets. Employees vaccinate again after 5 years of tetanus vaccination; employees should be informed about the harmful effects of the cigarette by the workplace physicians and encouraged employees to quit smoking. Especially in employee who are over the age of 50, whose body mass index is over 25 and male employees should be careful in terms of HT; in order to prevent work accidents due to sudden hypoglycemia that may develop in diabetic employees, it should be ensured that their work is kept away from work such as cranes and elevated work. Employees in periodic health examinations should be assessed with calculations showing organ functions. Audiological audits in field employees and employees in dusty workplaces should also be carried out with annual PFT.Key Words: Occupational Health and Safety; Periodic Examination; Occupational Health and Safety Law No. 6331; Field Employee
