Abstract
Amaç: Bu geriye dönük çalışmada acil servise karın ağrısı şikayeti ile başvuran ve akut apandisit ön tanısı ile ameliyat edilen hastaların, ameliyat mateyallerinin incelenmesi sonucunda belirtilen patolojik tanıları ile hastaların demografik özelliklerini, hemogram parametrelerindeki değişimlerin değerlendirilmesini, acil serviste akut apandisit erken tanısına katkısı olabilecek veriler elde etmeyi ve literature katkıda bulunmayı amaçladık.Gereç ve Yöntem: Beş yıllık çalışma süresinde akut apandisit ön tanısı ile ameliyat edilen 508 hastanın dosyaları, hastane otomasyon sistem bilgileri ve patoloji raporları incelendi. Çalışmamıza katılma kriterlerini karşılayan 417 hasta ameliyat sonrası patoloji raporlarına göre pozitif ve negatif apendektomi gruplarına ayrıldı. Pozitif apendektomi grubu da kendi içerisinde basit ve komplike apandisit gruplarına bölündü. Hastaların yaş, cinsiyet, hemogram parametreleri uygun istatistiksel yöntemler kullanılarak belirlenen gruplar arasında karşılaştırıldı. Bulgular: Çalışmamızdaki 417 hastanın 30'unda (%7.2) negatif apendektomi saptanırken akut apandisitin erkeklerde ve ikinci dekatın sonlarına doğru daha sık olarak görüldüğü tespit edildi. Negatif ve pozitif apandektomi grupları arasında hemogram parametreleri karşılaştırıldığında pozitif apendektomi grubunda beyaz kan hücresi, nötrofil, nötrofil-lenfosit oranı, platelet-lenfosit oranı değerleri anlamlı olarak yüksek bulunurken lenfosit, platelet-nötrofil oranı değerleri anlamlı olarak düşük bulundu. Basit apandisit ve komplike apandisit grupları arasında hemogram parametreleri karşılaştırıldığında komplike apandisit grubunda beyaz kan hücresi, nötrofil, nötrofil-lenfosit oranı, platelet-lenfosit oranı değerleri anlamlı olarak yüksek bulunurken lenfosit, platelet dağılım genişliği, platelet-nötrofil oranı değerleri anlamlı olarak düşük bulundu. Sonuç: Akut apandisit erkeklerde yirmili yaşların sonlarında daha sık olarak görülmektedir. Akut apandisit tanısında, nötrofil, nötrofil-lenfosit oranı değerlerinin yüksek olması, yüksek beyaz kan hücresi değerinden daha anlamlı olmakla birlikte hiçbir hemogram parametresinde tek başına tanı koydurucu yükseklikte duyarlılık ve özgüllük tespit edilememiştir. Aim: In this retrospective study, we aimed to obtain data that could contribute to the early diagnosis of acute appendicitis in the emergency department and to the literature by evaluation of the pathological diagnoses as a result of examination of surgical materials of patients who were admitted to the emergency department with abdominal pain complaints and operated with the acute appendicitis preliminary diagnosis together with the changes in their demographic characteristics and hemogram parameters.Material and Methods: The files belonging to 508 patients, who were operated with the acute appendicitis preliminary diagnosis, have been examined together with the information obtained from the automation system of the hospital and pathology reports during the five-year study period. 417 patients, who met our study's participation criteria, were divided into positive and negative apendectomy groups according to the postoperative pathology reports. The positive appendectomy group was also divided into simple and complicated appendicitis groups. The age, sex and hemogram parameters of the patients were compared in between the groups specified by using the appropriate statistical methods.Results: It is revealed in our study that acute appendicitis was seen more frequently in males towards the end of the second decade and we found negative appendectomy in 30 (7.2%) of our 417 patients. When the hemogram parameters were compared in between the negative and positive appendectomy groups, the values of white blood cell, neutrophil, neutrophil-lymphocyte ratio, platelet-lymphocyte ratio were significantly higher in the positive appendectomy group while lymphocyte, platelet-neutrophil ratio values were found to be significantly lower. When hemogram parameters were compared in between simple and complicated appendicitis groups, white blood cell, neutrophil, neutrophil-lymphocyte ratio and platelet-lymphocyte ratio values were significantly higher in complicated appendicitis group whereas lymphocyte, platelet distribution width and platelet-neutrophil ratio values were found to be significantly lower.Conclusion: Acute appendicitis is seen more frequently in males in the late twenties. Neutrophil, neutrophil-lymphocyte ratio values are more significant than white blood cell values in the diagnosis of acute appendicitis, but a sensitivity and specificity in any hemogram parameters that would be high enough for the diagnose of acute appendicitis alone are not specified in the study.
