Abstract
ÖZETGiriş: Oral yoldan korozif madde alımı ağız, yemek borusu ve midede ciddi yaralanmalara neden olabilmektedir. Korozif madde alımı çocuk yaş grubunda genellikle kaza sonucu olurken, adelosan ve erişkinlerde kasıtlı ve intihar orjinli olduğu görülmektedir. Çalışmamızda 5 yıllık süre içerisinde hastane acil servisine korozif madde alımı nedeniyle başvuran olgular incelenerek sonuçların literatür bilgileriyle karşılaştırılıp yorumlanması amaçlanmıştır.Gereç ve Yöntem: Acil Servisimize 2013-2017 yılları arasında korozif madde alımı nedeni ile başvuran, yatırılıp takip ve tedavisi yapılan olgular; yaş, cinsiyet, alınan miktar, alım orjini, madde türü, başvuru öncesi yapılan müdahaleler, semptomlar, fizik muayene bulguları, morbidite, mortalite ve adli yönlerden değerlendirildi.Bulgular: Çalışmamızda oral yoldan korozif madde alımı nedeni ile takip ve tedavisi yapılan 261 olgu geriye dönük olarak değerlendirildi. Olguların 141'i erkek (%61,4) ve 120'si kadındı (%38,6). Korozif madde alımı en sık 1-5 yaş aralığındaydı (%57,5). En sık içilen korozif maddeler çamaşır suyu (%32,2), sonra sırasıyla yağ çözücü (%13,4), kireç çözücüydü (%12,3). Olguların %81,6'sı korozif maddeyi kaza ile, %9,6'sı intihar amaçlı almıştı. Olguların %90,8'ine adli rapor düzenlendiği, %9,2'sine ise düzenlenmediği görüldü. En sık başvuru semptomu kusmaydı (%63,2). Hastane öncesi en sık yapılan müdahaleler %4,2 oranla su içme/içirilmeydi. En sık klinik bulgu orofarenkste hiperemiydi (%16,3). Endoskopi yapılan 27 olgunun 11'inde (%4,2) grade 1, 10'unda (%3,8) grade 2A yanık saptandı. Olguların 3'ünde (%4) özofageal darlık, 2'sinde (%3) mide çıkış obstrüksiyonu, 1'inde (%1) gastroözofageal reflü izlendi. Bir olguda sirke ruhu (asetik asit) içimi sonrası ölümün geliştiği görüldü. Sonuç: Acil servise veya herhangi bir sağlık merkezine müracaat eden her türlü korozif madde alımı adli olgu olarak değerlendirilmeli veadli rapor düzenlenmelidir.Adli rapor düzenlenirken, fizik muayenedeçocuk istismarı, ihmali ve cinayet sonucu oluşmuşolabilecek bulgularirdelenmeli ve dosyaya gerekli notlar düşülmelidir. Çünkü korozif madde istismar, ihmal, intihar ve cinayet amaçlı içilmiş veya içirilmiş olabileceği unutulmamalıdır.Anahtar kelimeler: korozif madde, özofageal darlık, adli olgu, uzuv zaafı/tatili ABSTRACTIntroduction: İngestion of corrosive substances can cause serious injuries in mouth, esophagus and stomach. While ingestion of corrosive substances generally occours accidentally in children, it can be seen to occour intentionally and for suicidal purposes in adolescents and adults. The purpose of our study is to examine cases who referred to emergency service due to ingestion of corrosive substances within a period of five years and to compare and interpret the results with the literature.Material and Method: The cases who referred to our emergency service for ingestion of corrosive substances between 2013 and 2017 and who were hospitalized and treated were assessed in terms of gender, age, amount ingested, origin of ingestion, type of substance, interventions before referral to the service, symptoms, physical examination findings, morbidity, mortality and forensics. Results: 261 cases who were treated and followed for corrosive substance ingestion were assessed retrospectively in our study. 141 (61,4%) of the cases were male, while 120 (38,6%) were female. The most frequent corrosive substance ingestion was between 1 and 5 years of age (57,5%). The most frequently ingested corrosive substances were bleach (32,2%), followed by fat-solvent (13,4%) and decalcifier (12,3%). 81,6% of the cases had ingested the corrosive substance accidentally, while 9,6% had ingested for suicidal purposes. 90,8% of the cases had judicial report, 9,2% did not. The most frequent referral symptom was vomiting (63,2%). The most frequent intervention before hospital was drinking water with a rate of 4,2%. The most frequent clinical symptom was oropharyngeal hyperemia (16,3%). Of the 27 cases who underwent endoscopy, grade 1 burn was found in 11 (4,2%), while grade 2A burn was found in 10 (3,8%). It was found that death occurred in 3 (4%) of the cases following esophageal stricture, in 2 (3%) of the cases following gastric outlet obstruction, in 1 (1%) case following gastroesophageal reflux and in 1 (1%) case as a result of spirit of vinegar (acidic acid) ingestion. Conclusion: All kinds of corrosive substance ingestion referring to emergency service or any health center should be assessed as judicial case and judicial report should be complied. While compiling the judicial report, findings that may have occurred as a result of child abuse, neglect and murder should be looked for in physical examination and necessary notes should be taken down in the file because corrosive substance may have been ingested or given for purposes of abuse, neglect, suicide or murder.
