Abstract
Giriş ve Amaç: Sezaryen oranlarının artması nedeniyle plasenta previa (PP) sıklığı giderek artmaktadır. Çalışmanın amacı plasenta previa tanılı hastalarda alt uterin segment transvers kompresyon sütürü ve bakri balon uygulamalarının başarısını saptayabilmektedir. Gereç ve Yöntem: 2010-2018 yılları arasında Ondokuz Mayıs Üniversitesinde plasenta previa totalis (PPT) nedeniyle sezaryen olan 257 hasta retrospektif olarak taranmıştır. Hastalar medikal tedavi verilen hastalar, bakri balon uygulanan, alt uterin segment transvers kompresyon sütürü uygulanan ve postpartum histerektomi olanlar olarak dört grupta incelenmiştir. Bulgular: Toplamda 27 hastaya kompresyon sütürü uygulanmış 1 hasta buna rağmen histerektomiye gitmiştir. Kompresyon sütürü için başarı oranı %96,3 iken, bakri balon için %91,7 olarak hesaplanmıştır. Toplamda 24 hastaya bakri balon uygulanmış ve 2 hasta buna rağmen histerektomiye gitmiştir. Tüm vakalar incelendiğinde %5,6'sında komplikasyon görülmüştür. Bu komplikasyonların tamamı postpartum histerektomi olan vakalarda izlenmiştir. Sonuç: Plasenta previa totalis tanılı hastalarda bakri balon ve kompresyon sütürü maternal morbiditeyi artırmadan kullanılabilecek etkin tedavi yöntemleridir Anahtar Kelimeler: plasenta previa totalis, Bakri balon, hysterectomy, kompresyon sütürü Aim: The incidence of placenta previa (PP) is increasing due to the increase in cesarean rates. The aim of the study was to determine the success of lower uterine segment transverse compression suture and bacri balloon applications in patients with placenta previa. Material and Method: Between 2010 and 2018, 257 patients with cesarean because of placenta previa totalis (PPT) were screened retrospectively at Ondokuz Mayıs University. Patients were evaluated in four groups as patients undergoing medical treatment, who underwent bacillary balloon injection, underwent lower uterine segment transverse compression suture and who had hysterectomy. Results: A total of 27 patients underwent compression suture and 1 patient had hysterectomy. The success rate for compression suture was calculated as % 96,3 and %91,7 for bakri balloon. A total of 24 patients underwent bakri balloon injection and 2 patients had hysterectomy. Complications were seen in %5,6 of all cases. All of these complications were observed in cases with postpartum hysterectomy. Conclusion: Bakri balloon and compression suture are effective treatment modalities that can be used without increasing maternal morbidity in patients with placenta previa totalis.
