Abstract
Amaç: Siroz ve portal hipertansiyonda (PHT) hemostazla ilgili bozukluklar sık görülmektedir. Ancak hemoztazdaki değişikliklerdeki net etkinin hemorajik ya da tromboza yatkınlık yönünde olup olmadığı bilinmemektedir. Sirozu olan çocuklarda hastalığın seyri ve oluşabilecek komplikasyonların önceden belirlenmesi, hem etkili tedavilerin verilmesi hem de gereksiz kan ürünü kullanımını önleyecektir. Bu çalışmada sirozu ve/veya portal hipertansiyonu olan hastalarda ADAMTS-13 (A disintegrin and metalloproteinase with thrombospondin motifs 13) ve von willebrand faktör (vWF) düzey ve aktivitelerindeki değişimlerin araştırılması amaçlandı.Hastalar ve Yöntem: Bu çalışma, Ocak 2019- Ekim 2019 tarihleri arasında Atatürk Üniversitesi Tıp Fakültesi Çocuk Gastroenteroloji Bilim Dalı'nda yeni tanı alan ve halen takip ve tedavilerine devam edilen siroz ve/veya PHT tanılı hastalarda yürütüldü. Hastaların demografik özellikleri, fizik muayene ve laboratuvar bulguları kaydedildi. Tüm olgulardan von willebrand faktör antijeni (vWF-Ag), von willebrand faktör ristosetin kofaktör düzeyi (vWF:RCo), ADAMTS-13 aktivitesi ve ADAMTS-13 antijen düzeyleri çalışıldı.Bulgular: Çalışmaya, sirozu olan 28, ekstrahepatik PHT'u olan 16 ve kontrol grubu olarak 20 sağlıklı çocuk dahil edildi. Sirozu olan hastaların vWF antijen (vWF-Ag) ve vWF ristosetin kofaktör (vWF:RCo) düzeyleri kontrol grubuna göre yüksek saptandı (p < 0,01 ve p < 0,01). Ekstrahepatik portal hipertansiyon grubunun vWF-Ag ve vWF:RCo düzeyleri kontrol grubuna göre yüksekti (p <0,01 ve p <0,01). Sirozu olan hastaların ADAMTS-13 antijen ve ADAMTS-13 aktivite düzeyleri kontrol grubuna göre düşük bulundu (p <0,01 ve p =0,02). Ekstrahepatik portal hipertansiyon grubunda ADAMTS-13 antijen ve ADAMTS-13 aktivite düzeyleri kontrol grubuna göre daha düşüktü (P=0,03 ve p=0,02). Von willebrand ristosetin kofaktör otoimmün karaciğer hastalığı olanlarda Wilson hastalığı olanlardan daha yüksek bulundu (p=0,02). Otoimmün karaciğer hastalığı olanlarda ADAMTS-13 antijen ve aktivite düzeyleri Wilson hastalığı olanlara göre daha düşük idi (p=0,01 ve p=0.03). Von willebrand faktör antijeni PELD skoru ≥7 olanlarda PELD skoru < 6 olanlara göre ve MELD skoru ≥ 10 olanlarda MELD skoru < 9 olanlara göre daha yüksek saptandı (p=0,02 ve p=0,03). Child-Pugh skoru (B ve C) olanların vWF antijen düzeyleri Child-Pugh skoru (A) olanlardan daha yüksek saptandı (p< 0.01). Sonuç: Çalışmamızda sirozu ve ekstrahepatik PHT'si olan çocuklarda prokoagülan faktörler olan vWF-Ag ve vWF:RCo düzeyleri artmış ve antikoagülan etkileri olan ADAMTS-13 antijen ve ADAMTS-13 aktivite düzeylerinde azalma yani trombotik değişikliklere neden olabilecek sonuçlar elde edilmiştir. Anahtar Kelimeler: Von willebrand faktör, ADAMTS-13, çocuk Aim: Hemostasis-related disorders are common in cirrhosis and portal hypertension (PHT). However, it is not known whether the net effect of changes in hemostasis in the sense of predisposition to hemorrhagic or thrombotic state. The course of the disease in children with cirrhosis and the predetermination of possible complications will lead to effective treatment and prevent unnecessary use of blood products. The aim of this study was to investigate the changes in ADAMTS-13 (A disintegrin and metalloproteinase with thrombospondin motifs 13) and von willebrand factor (vWF) levels and activity in patients with cirrhosis and/or PHT.Patients and Methods: This study was carried out between January 2019 and October 2019 with new or previously diagnosed patients with cirrhosis and/or PHT in the Department of Pediatric Gastroenterology, Faculty of Medicine, Atatürk University. Patients' demographic characteristics, physical examination and laboratory findings were recorded. Von willebrand factor antigen (vWF-Ag), von willebrand factor ristocetin cofactor levels (vWF: RCo), ADAMTS-13 activity and ADAMTS-13 antigen levels were studied.Results: Twenty-eight children with cirrhosis, 16 patients with extrahepatic PHT and 20 healthy controls were included in the study. The vWF antigen (vWF-Ag) and vWF ristocetin cofactor (vWF: RCo) levels of the patients with cirrhosis were higher than the control group (p <0.01 and p <0.01). The vWF-Ag and vWF: RCo levels of the extrahepatic portal hypertension group were higher than control group (p <0.01 and p <0.01). The ADAMTS-13 antigen and ADAMTS-13 activity levels of the patients with cirrhosis were lower than the control group (p <0.01 and p = 0.02). ADAMTS-13 antigen and ADAMTS-13 activity levels were lower in the extrahepatic portal hypertension group than the control group (P = 0.03 and p = 0.02). Von willebrand ristocetin cofactor was higher in patients with autoimmune liver diseases than patients with Wilson's disease (p = 0.02). The ADAMTS-13 antigen and activity levels were lower in patients with autoimmune liver disease than in patients with Wilson's disease (p = 0.01 and p = 0.03). Von willebrand factor antigen was found to be higher in patients with PELD score ≥7 than those with PELD score <6 and in patients with MELD score ≥ 10 than those with MELD score <9 (p = 0.02 and p = 0.03). The vWF antigen levels of the patients with Child-Pugh score (B and C) were higher than those with Child-Pugh score (A) (p <0.01).Conclusion: In our study, vWF-Ag and vWF: RCo levels, which are procoagulant factors, were increased and ADAMTS-13 antigen and ADAMTS-13 activity levels which have anticoagulant effects were decreased in children with cirrhosis and extrahepatic PHT. So our results suggest presence of thrombotic changes in patients with cirrhosis and extrahepatic PHT. Keywords: Von willebrand factor, ADAMTS-13, children
