Abstract
Bu in vitro çalışmanın amacı; dört farklı döner eğe sistemi (Reciproc Blue, Twisted File Adaptive, WaveOne Gold ve Xp-Endo Shaper) ile enstrümantasyon sonrası oluşan apikal debris taşmasının ve kanal transportasyonunun karşılaştırılmasıdır. Kanal transportasyon değerlendirmesinde 60 adet S-şekilli kanallara sahip rezin bloklar kullanıldı. Bloklar dört eşit gruba ayrıldı (n=15). Kanalların genişletilmesinden önce ve sonra elde edilen görüntüler bilgisayar ortamında çakıştırıldı. İç ve dış kanal duvarları ile genişletilmiş duvarlar arasındaki mesafe, bu üst üste binmiş görüntüler üzerinden, dijital yazılım kullanılarak ölçüldü. Apikalden taşan debris değerlendirmesi için ise, 60 adet çekilmiş mandibular kesici diş, 4 eşit gruba ayrıldı (n=15). Dişler Eppendorf tüplerine yerleştirildi. Örnekler dört farklı döner eğe sisteminden biri ile prepare edildi. Daha sonra tüpler, kurutma için 5 gün boyunca 70 °C'de inkübe edildi. Apikalden taşan debrisin ağırlığı, hassas terazi kullanılarak, son ağırlıktan başlangıç ağırlığının çıkarılmasıyla belirlendi. Gruplar Kruskal-Wallis ve Dunn çoklu karşılaştırma testleri kullanılarak karşılaştırıldı. Kanal transportasyonunun genel değerlendirmesi açısından, Xp-Endo Shaper-TF Adaptive arasında ve Reciproc Blue-WaveOne Gold arasında anlamlı bir fark yoktu (p>0.05). Ancak, Xp-Endo Shaper ve TF Adaptive, Reciproc Blue ve WaveOne Gold'a kıyasla önemli ölçüde daha az transportasyona sebep oldu (p<0.05). Kanal transportasyonunun apikal üçlü değerlendirmesinde de aynı yapı gözlemlendi. Orta üçlü değerlendirmesinde XP-Endo Shaper diğer tüm sistemlerden istatistiksel olarak anlamlı şekilde daha az kanal transportasyonuna neden oldu (p<0.05). This in-vitro study aimed to compare the canal transportation and apically extruded debris following instrumentation with different rotary systems (Reciproc Blue, Twisted File Adaptive, WaveOne Gold and Xp Endo Shaper). Sixty S-shaped canals in resin blocks were included for canal transportation evaluation. The blocks were divided to four equal groups (n=15). Superimposed pre- and post-instrumentation images were obtained. The distance between inner and outer canal walls to enlarged ones were measured on these superimposed images by using digital software. For apically extruded debris evaluation 60 mandibular incisors were selected, then divided to 4 equal groups (n = 15). The teeth were placed in Eppendorf tubes. The specimens were prepared with one of the four different rotary systems. Subsequently, the tubes were incubated at 70 oC for 5 days for drying. The weight of the extruded debris was determined by subtracting the initial weight from the final weight. The groups were compared using Kruskal-Wallis and Dunn multiple tests. In terms of canal transportation in total, there was no significant difference between Xp Endo Shaper-TF Adaptive and Reciproc Blue-WaveOne Gold (p>0.05). However, Xp Endo Shaper and TF Adaptive resulted in significantly less transportation compared to Reciproc Blue and WaveOne Gold (p<0.05). The same pattern was observed also in the apical third. In the assessment of the middle third XP Endo Shaper caused statistically significantly less canal transportations than all other systems (p<0.05). In terms of the apically extruded debris, Xp Endo Shaper resulted in significantly less extrusion compared to other 3 groups (p<0.05). There was no significant difference between the other groups (p>0.05). XP Endo shaper is advantageous regarding both canal transportation and debris extrusion issues.
