dc.description.abstractÖZET Bu çalışmada genel, epidural, genel ile kombine spinal anestezi uygulanarak genel cerrahi girişim yapılan 45 olgunun prolaktin, kortizol, insülin, kan şekeri değerleri çeşitli ev relerde saptanarak cerrahi stres sonucu nöroendokrin ve meta- bolik durumlarının en az etkilendiği anestezi tipi saptanmaya çal ı şı İdi. Verilerimiz, epidural anestezinin ve genel anestezi ile kombine spinal anestezinin en az endokrin ve metabolik değişik liğe yolaçtığını göstermiştir. Bu sonuç, cerrahi strese endok rin ve metabolik cevap gereksizdir; engellenmeli veya en aza indirilmelidir şeklindeki modern görüşe en uygun anestezi tipi nin saptanması bakımından önemlidir. 86 -
dc.description.abstractSUMMARY The aim of this study is to find out the type of anest hesia which effects the neuroendocrine and metabolic balance, the least, in 45 cases who underwent surgery by general surge ons. We monitored the influence of surgical stress on the sys tem by measuring serum prolactin, Cortisol, insulin and blood sugar values in cases who were given different anesthetic met hods namely, general anesthesia, epidural anesthesia and spinal anesthesia combined with general anesthesia. The results of this study are of vital importance becau se it invalidated the hypothesis which states that the endocri ne and metabolic response to surgical stress is essential. In modern concept, endocrine and metabolic response should be avo ided or minimized and our data suggest that the best method in this respect is epidural anesthesia and spinal anesthesia which is combined with general anesthesia. - 87 - TüSse&ö9s®tim Kuruluen_US
