dc.description.abstract | ÖZET Bu çalışma Gazi Üniversitesi Tıp Fakültesi Nöroloji Anabilimdalında myastenia gravis (MG) ile myotonik sendrom (MyS) lu hastalarda ardısıra sinir uyanm(ASU) testinde izlenen farklılıkları ortaya koymak amacı ile gerçekleştirildi. On jeneralize MG ve 18 MyS (Myotonik distrofl (MyD): 11, Resesif jeneralize myotoni (RJMy):7) lu hasta inceledi. Sulkus ulnarls hizasından supramaksimal uyan ile abduktor digiti kuinti kasından yûzeyel elektrotlarla istirahat birleşik kas aksiyon potansiyali (iBKAP) ve postegzersiz(pe) BKAP kaydedildi. îki, 5c/s ve yüksek hızda uyarımla sırası ile 5, 2 ve 1 saniye içindeki dekrement yanıt saptandı. Tetanusu takiben 1 saniye sonraki post tetanik(pt) BKAP amplitüdü ve tetanusun 1. potansiyeline oranına bakıldı. iBKAP amplitüdüne göre peBKAP ve ptBKAP amplltüdlerde dekrement yüzdesine bakıldı. Elde edilen veriler MG ile MyS ve MyD ile RJMy arasındaki farklılıklar incelendi. MyS lu grupta; peBKAP, ptBKAP, Posttetanik oran(PT-Oran) anlamlı olarak küçülmüştü ve PE-dekrement anlamlı olarak artmıştı. MG grubunda ise 2 c/s ve 5 c/s uyarımla anlamlı dekrement saptandı. MyD ile RJMy arasında ise; iBKAP ve peBKAP MyD grubunda anlamlı olarak küçülmüştü. RJMy de ise PT-Oran küçülmüş ve PT-dekrement oranı belirgin olarak artmıştı. Hem MG hemde MyS lu hastalarda ASU testinde değişiklikler gözlenmektedir. Bu özellikle MyS lu hastalarda subgurup tayininde yararlı olabilir. 66 | |
dc.description.abstract | SUMMARY Results of repetitive nerve stimulation(RNS) test In myasthenia gravis(MG) and myotonic syndromes(MyS) were analysed. Ten Myastheia gravis (MG) and 18 Myotonic sendrome (MyS) patients were studied. Surface stimulating electrodes were placed on the ulnar sulcus and superficial recording electrodes were placed on abductor digit! quinti muscle. By using supramaximal stimulation, resting(r) and post-exercise(pe) compound muscle action potential (CMAP) were recorded. The nerve was stimulated at 2c/s for 5 seconds, at 5c/s for 2 seconds and at high stimulation rate for 1 second and evoked responses were recorded. One second after tetanic stimulation, a post- tetanic stimulus was given. pt-CMAP amplitude and post-tetanic ratio, as measured by dividing this value by the first potential of high rate stimulus train were obtained. Taking the r-CMAP as reference, the decremental responses of pe-CMAP and pt-CMAP were calculated. We studied the electrophysiologic differences between MG and MyS, as well as MyD and RGMy. In MyS group pe-CMAP, pt-CMAP, post-tetanic ratio(PT-Ratio) were decreased and PE-decremental response was increased significantly. In MG group significant decremental response, at 2c/s and 5 c/s stimulation was observed. The analysis of the MyS group revealed that r-CMAP was significantly decrased in MyD. PT-Ratio was decreased and PT-decremental response was significantly increased in RGMy. RNS yields abnormal results both in MG and MyS. RNS may especially be helpful in the differentiation of subgroups of MyS. 67 | en_US |
