dc.description.abstract | Bu çalışma T hücreli lenfoma tanılı hastalarda tedavi sonrası yapılan PET- BT görüntülemenin hastalık prognozunu öngörmedeki değerinin araştırılması amacıyla yapılmıştır. Çalışmada 2004-2014 yılları arasında Gazi Üniversitesi Tıp Fakültesi Hastanesi Tıbbi Onkoloji Kliniği'ne başvuran T hücreli lenfoma tanılı 24 hasta retrospektif olarak değerlendirilmiştir. Hastaların tanı anındaki klinik bilgileri, histopatolojik özellikleri, PET-BT görüntüleme sonucu elde edilen sayısal verileri ve primer kemoterapi sonrası PET-BT görüntüleme sonucu elde edilen sayısal verileri incelenmiştir. Tedavi sonrası PET-BT ile hastalıksız sağkalım ve toplam sağkalım arasındaki korelasyon incelenmiştir.Çalışmaya T hücre lenfomalı 24 hasta (median yaş:45.5, aralık 19-84 yaş) dahil edildi. Başlangıç PET-BT sonuçları 23 hastada lenfoma tutulumu için pozitiftir. Progresif hastalık nedeniyle ex olan 4 hasta dışında tüm hastalara PET çekilmiştir. Birinci basamak kemoterapinin tamamlanmasından sonra (%50 CHOP bezeri), 15 hastada PET-BT sonuçları negatif geldi. PET negatifleşen hastaların 8 tanesinde nüks-progres gelişmiştir. PET pozitif hastaların dördünde progres-relaps gelişmiştir. Median takip süresi 26,5 aydır. Bir ve 2 yıllık PFS oranları PET negatif grupta %78,6 ve %50, PET pozitif grupta %40 ve %0 bulunmuştur (p=0,007). İki grup arasında 1 ve 2 yıllık OS oranları açısından istatistiksel olarak anlamlı fark saptanmamıştır (p=0,113). Çoklu değişkenli cox regresyon analizleri sonucunda sadece yaşın OS ve PFS üzerinde etkili olduğu, yaş ilerledikçe progresyon ve ölüm hızının arttığı görülmüştür.Sonuç olarak tedavi sonrası PET-BT sonucunun, T hücreli lenfomalarda PFS ve OS için bağımsız bir prediktör olmadığı görülmüştür. Bununla birlikte hasta sayısının az olması, çalışmanın retrospektif olarak yapılması nedeni ile konu ile ilgili daha geniş hasta gruplu randomize kontrollü çalışmalara ihtiyaç duyulmaktadır. dc.description.abstract | This study was conducted to investigate value of post-treatment PET-CT imaging in predicting disease prognosis in patients with T cell lymphoma. In this study, we retrospectively reviewed 24 patients with T cell lymphoma presented to Medical Oncology Clinic of Gazi University, Medicine School between 2004 and 2014. Clinical findings at diagnosis, histopathological features, numeric data obtained by baseline PET-CT imaging and numeric data obtained by PET-CT imaging performed after first-line chemotherapy were evaluated. Correlations between post-treatment PET-CT imaging and progression-free and overall survivals were assessed. The study included 24 patients with T cell lymphoma (median age: 45.5 years; range: 19-84 years). Lymphoma involvement was positive in 23 patients on baseline PET-CT imaging. All patients other than 4 died due to progressive disease underwent PET-CT imaging after therapy. PET-CT imaging was found to be negative in 15 patients after completion of first-line chemotherapy (50% CHOP). Recurrence-progression was developed in 8 patients with negative post-treatment PET-CT imaging. Progression-relapse was developed in 4 patients with positive post-treatment PET-CT imaging. Median follow-up was 26.5 months. It was found that one- and 2-years PFS rate was 78.6% and 50.0% in the group with negative PET-CT imaging while 40% and 0% in the group with positive PET-CT imaging, respectively (p=0.007). No significant difference was detected in one- and 2-years OS rate between groups. Multivariate Cox regression analysis showed that only age was significantly correlated with OS and PFS, indicating higher progression and mortality rates with advancing age. In conclusion, it was seen that PET-CT imaging result isn't an independent predictor for PFS and OS in T cell lymphoma. However, further randomized, controlled studies with larger sample size are needed, given the limited sample size and retrospective nature of our study.Keywords: T cell lymphoma, PET-CT imaging | en_US |
