Abstract
Hipertansiyon, oluşturduğu hedef organ hasarı ile dünyada önlenebilir ölüm nedenleri içerisinde ilk sırada yer almaktadır. Esansiyel hipertansiyonun fizyopatolojisinde rol alan Ürotensin-II'nin oksidatif mekanizmalarda da rolleri mevcuttur. Ürotensin-II davranışsal etkileri ile şizofreni, şizoaffektif bozukluk, obsesif kompulsif bozukluk gibi psikiyatrik hastalıklarda araştırılmıştır. Esansiyel hipertansiyona sıklıkla sekonder psikiyatrik hastalıklar eşlik eder. Bunun büyük çoğunluğunu anksiyete ve anksiyete bozuklukları oluşturmaktadır. Hipertansiyonu olan hastalarda anksiyete görülme sıklığı fazla olduğu gibi anksiyete, kan basıncında artışa yol açmaktadır. Protein S100B, serotonerjik nöronların major nörotrofik faktörüdür. Bu proteinin oksidatif stress, inflamatuar yanıt ve endotel disfonksiyonunda rol aldığı bulunmuştur. Bipolar bozukluk, major depresif bozukluk, şizofreni gibi hastalıklarda seviyeleri ve etkileri araştırılmaya çalışılmıştır. Bu çalışmada esansiyel hipertansiyon hastalarının anksiyete düzeyleri araştırılarak, anksiyete şiddetinin bahsedilen S100B ve Ürotensin-II protein seviyeleri üzerindeki etkisinin incelenmesi amaçlandı. Çalışmaya Fırat Üniversitesi Hastanesi Kardiyoloji Polikliniği'ne başvuran ve ayaktan tedavi gören, çalışma ölçütlerine uyan 153 hasta dahil edildi. Hastalara Sosyodemografik ve Klinik Veri Formu, Beck Anksiyete Ölçeği (BAÖ), Beck Depresyon Ölçeği (BDÖ) uygulandı, tansiyon ölçümleri sonrasında venöz kan örnekleri alındı. Enzyme-Linked Immunosorbent Assay (ELISA) yöntemi ile S100B ve Ürotensin-II düzeyleri ölçüldü. Sonuçların değerlendirilmesinde SPSS 22 istatistik programı kullanıldı.Hastaların BAÖ puanları ile Ürotensin-II seviyeleri arasında pozitif yönde anlamlı korelasyon saptanıldı (p=0.043) (r=0.16). Depresyonu olan grupla, olmayan grubun serum Ürotensin-II seviyeleri arasında istatiksel olarak anlamlı fark gözlendi (p=0.017). Hastaların Ürotensin-II seviyeleri ile S100B seviyeleri arasında pozitif yönde anlamlı korelasyon izlendi (p<0.001) (r=0.71). Hastaların sistolik kan basınçları ile BAÖ puanları (p<0.001) (r=0.29) ve BDÖ puanları arasında pozitif yönde anlamlı korelasyon görüldü (p=0.049) (r=0.16). Anksiyete şiddetlerine göre S100B ve Ürotensin-II düzeyleri açısından gruplar arasında anlamlı fark gözlenmezken, şiddetli anksiyete grubunun sistolik kan basınç ortalamaları, diğer normal, hafif, orta anksiyete gruplarına göre istatistiksel olarak anlamlı derecede yüksekti (p=0.001) (p=0.002) (p=0.023). İmmünolojik, hormonal, oksidatif ve antioksidatif sistemlere etkili olan Ürotensin-II ve S100B, hipertansiyon ve anksiyete bozukluklarının etiyolojisine yönelik yaklaşımlara ışık tutabilir. Elde edilen veriler farklı toplumlarda ve yaş gruplarında geniş kapsamlı çalışmalarla desteklenilmeli ve geliştirilmelidir. The Relationship Between Anxiety and Serum Urotensin-II and S100B Levels in Patients with Essential HypertensionHypertension is the leading cause of preventable death based on target organ damage. Urotensin-II, which plays a role in the pathophysiology of essential hypertension, has roles in oxidative mechanisms. Urotensin-II behavioral effects were investigated in psychiatric disorders such as schizophrenia, schizoaffective disorder, and obsessive compulsive disorder. Essential hypertension is often accompanied by secondary psychiatric disorders. Anxiety and anxiety disorders constitute the majority of the diseases that are associated with hypertension. There is high incidence of anxiety in patients with hypertension, as well as anxiety may increase blood pressure. Molecule S100B protein is the major neurotrophic factor of serotonergic neurons. It has been found that this protein is involved in oxidative stress, inflammatory response and endothelial dysfunction. S100B levels and effects have been investigated in some diseases such as bipolar disorder, major depressive disorder and schizophrenia. The aim of study was to investigate the anxiety levels of patients with essential hypertension and to investigate the effects of anxiety intensity on the levels of S100B and Urotensin-II proteins.A total of 153 patients who were admitted to the outpatient clinic of the Fırat University Hospital and who met the study criteria were included in the study. Sociodemographic and Clinical Data Form, Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI) were applied to the patients and venous blood samples were collected after blood pressure measurements. S100B and Urotensin-II levels were measured by Enzyme-Linked Immunosorbent Assay (ELISA) method. The results were analyzed with the IBM SPSS Statistics 22 statistical program. There was a positive correlation between BAI scores and UT-II levels (p=0.043) (r=0.16). A statistically significant difference was observed between the group with depression and the serum UT-II levels (p=0.017). There was a positive correlation between urotensin-II levels and S100B levels (p <0.001) (r=0.71). Systolic blood pressures and BAI scores (p <0.001) (r=0.29) and BDI scores were positively correlated (p=0.049) (r=0.16). While S100B and Urotensin-II levels were not significantly different between the groups according to their anxiety severity, mean systolic blood pressure of severe anxiety group was statistically higher than others which moderate, mild and normal anxiety groups (p=0.001) (p=0.002) (p=0.023). Urotensin-II and S100B, which are effective on immunological, hormonal, oxidative and antioxidative systems, may shed light on approaches to the etiology of hypertension and anxiety disorders. The obtained data should be supported and developed in a wide range of studies in different societies and age groups.
