Pediatrik cerrahi servisine özgü hasta sınıflandırma ölçeği geliştirilmesi

Abstract

Hasta sınıflandırma ölçeği, hemşirelik bakım gereksinimleri benzer olan hastaları sayısal bir puanlama ile gruplandırarak, hemşire insan gücü planlama sürecinde kullanılmaktadır. Bu çalışma, bir üniversite hastanesinin pediatrik cerrahi servisinde geçerli ve güvenilir bir hasta sınıflandırma ölçeği oluşturmak amacıyla tanımlayıcı ve metodolojik olarak yapılandırılmıştır. Araştırmada Pediatrik Cerrahi Servisi Hasta Sınıflandırma Taslak Ölçeği geliştirilerek, ölçeğin geçerlik ve güvenirlik analizleri incelenmiştir. Araştırma Gazi Üniversitesi Sağlık Araştırma ve Uygulama Merkezi'nde, Pediatrik Cerrahi Servisi'nde yapılmıştır. Araştırmanın örneklemini; Pediatrik Cerrahi Servisi'nde çalışan dört hemşire ile Nisan 2017-Aralık 2018 tarihleri arasında bu serviste tedavi gören hastalar oluşturmuştur. Verilerin toplanmasında hastaların tanıtıcı özelliklerini içeren bölüm ile `Pediatrik Cerrahi Servisi Hasta Sınıflandırma Taslak Ölçeği` kullanılmıştır. Taslak ölçek araştırmacı tarafından, Tez İzleme Komitesi Üyeleri ve servis hemşirelerinin görüşleri doğrultusunda tasarlanmıştır. Hasta sınıflandırma verileri, araştırmacı ve bir servis hemşiresi tarafından toplanmıştır. Araştırmada değerlendiriciler arası uyum 503 veri üzerinden, ölçek geçerlik ve güvenirlik analizi ise 301 veri üzerinden hesaplanmıştır. Ölçeğin güvenirlik analizinde, Cohen'in kappa istatistiği, bağımsız grup t testi, Cronbach alfa güvenirlik katsayısı ve Pearson korelasyon katsayısı kullanılmıştır. Ölçeğin kappa analizi sonucunun 0,683 ile 0,990 arasında değiştiği ve değerlendiriciler arasında `önemli düzeyde iyi uyum` ile `mükemmel düzeyde uyum` olduğu tespit edilmiştir. Bağımsız grup t testinde, değerlendiriciler arası puanlamada fark olmadığı saptanmıştır (t=-0,068; p>0,05). Taslak ölçeğin tüm maddeleri için hesaplanan Cronbach alfa güvenirlik katsayısının 0,765 ve madde-toplam puan korelasyonlarının `eğitim ve rehberlik` alanı dışında (r=0,201, p<0,001), r=0,436 ile r=0,918 arasında olduğu belirlenmiştir. Araştırma sonuçları, geliştirilen ölçeğin pediatrik cerrahi servisi hastalarının bakım gereksinimlerine göre sınıflandırılmasında geçerli ve güvenilir bir ölçüm aracı olduğunu göstermektedir.

The patient classification scale is utilized in the planning process of nursing human power by grouping patients with similar nursing care requirements with numerical scoring. This study was structured descriptively and methodologically to establish a valid and reliable patient classification scale in the pediatric surgery service of a university hospital. In the study, the Pediatric Surgery Service Patient Classification Draft Scale was developed, and the validity and reliability analyses of the scale were conducted. The study was carried out in the Pediatric Surgery Service of Gazi University Health Research and Application Center. The sample of the study consisted of four nurses working in the Pediatric Surgery Service and the patients receiving treatment in this service between April 2017 and November 2018. The study data were collected using the `Pediatric Surgery Service Patient Classification Draft Scale`, which also included a section for obtaining descriptive patient information. The draft scale was designed by the researcher in accordance with the opinions of the Thesis Monitoring Committee members and service nurses. The patient classification data were collected by the researcher and a service nurse. In the study, the inter-rater reliability was calculated over 503 data, and the scale validity and reliability analysis were calculated over 301 data. Cohen's kappa statistics, independent group t-test, Cronbach's alpha reliability coefficient, and Pearson's correlation coefficient were used in the reliability analysis of the scale. The kappa value of the scale ranged between 0,683 and 0,990 and, the inter-rater reliability was determined to be at the level of `substantial agreement` and `almost perfect agreement`. In the independent group t-test, no significant difference was found between the raters' scores (t=-0,068; p>0,05). Cronbach's alpha reliability coefficient calculated for all items of the draft scale was found to be 0,765, and item-total score correlations were found to range from r=0,436 to r=0,918, except for the area of `education and guidance` (r=0,201, p<0,001). The results of the study indicated that the developed scale was a valid and reliable measurement instrument for the classification of pediatric surgical service patients according to their care requirements.
