Bipolar hastalarda aile işleyişi ve evliliğin hastalık özellikleri ile ilişkisi

Abstract
Amaç: Bipolar bozukluk, genetik ve çevresel etkenlerle tetiklenen ailesel birhastalıktır. Bu çalışmada; biyolojik bileşeni belirgin bir hastalık olan bipolarbozukluğun görüngüsel özellikleri (başlangıç yaşı, hastalık süresi, hastalık dönemsayıları ve süreleri, psikotik bulgululuk) ve hastalık gidişi ile ilişkili özellikleri(şiddet, hasta geçirilen süre, yatış öyküsü, özkıyım öyküsü, dönemler arası iyileşme,mevsimsellik) ile evlilik ve aile işleyişi gibi psikososyal değişkenler arasındakiilişkinin incelenmesi amaçlanmıştır.Yöntem: DSM ? IV'e göre; Bipolar I (n:53) ve II bozukluk (n:21) tanısı ile izlenenen az 2 yıldır evli ve remisyonda 74 hastanın, hastalık özellikleri; hasta ve yakınlarıile görüşme, poliklinik kayıtları, hastaneye yatış dosyası, izlemiş hekimlerden alınanbilgiler kullanılarak saptanmıştır. Evlilik ve aile işleyişi, sosyal destek sistem veişlevsellik bilgileri, özbildirim ölçek verilerinden (Aile Değerlendirme Ölçeği,Bipolar Bozukluk İşlevsellik Ölçeği, Sosyal Destek Ölçeği ve Çift Uyum Ölçeği)alınmıştır. Araştırma analizinde; evlilik ve aile işleyişine dair değişkenler ile; hastalıkbaşlangıç yaşı, hastalık süresi, hastalık dönemi içinde geçen toplam süre, hastaneyeyatış öyküsü, tanısal alt tip, psikotik bulgulu dönemler, özkıyım girişimi, dönemlerarası iyileşme, mevsimsellik, geçirilen hastalık dönemi sayısı ve şiddeti gibi hastalıközelikleri arasındaki ilişki incelenmiştir.Bulgular: Hastalık özelliklerinden; hastalık süresi, hastalık dönemi içinde geçentoplam süre, dönemler arası iyileşme, hastalık dönem sayısı ve hastalık dönemişiddeti ile evlilik ve aile işleyişine ilişkin değişkenler arasında ilişki saptanmıştır.Depresif dönemde geçen süre, depresif dönem sayısı ve şiddeti ile saptanan ilişkiyoğunluktadır. Hastalık döneminde geçirilen süre ve hastalık dönem şiddeti ile sosyaldestek sistemleri, depresif dönemde geçen süre ile işlevsellik negatif ilişkilidir.Hastalık başlangıç yaşı, hastaneye yatış öyküsü, tanısal alt tip, psikotik bulguludönemler, özkıyım girişimi ve mevsimsellik ile evlilik ve aile işleyişi, sosyal desteksistemler ve işlevsellik arasında ilişki bulunmamıştır.Sonuç: Bipolar bozuklukta, özellikle depresif dönemler olmak üzere hastalıkdönemlerinin sayı, süre ve şiddeti ile evlilik ve aile işleyişi arasında ilişki vardır. Background: Bipolar disorder is familial illness triggered by genetic andenvironmental contributions. This study aims to investigate the association betweenphenomenological (age of onset, duration of illness, duration and sum of episodesand psychotic symptoms) and clinical course features (recovery between episodes,seasonality)of bipolar disorder which has significant biological component andpsychosocial variables such as marital and family functioning.Method: Clinical features of seventyfour patients in remission, married at least fortwo years, with bipolar I (n:53) and bipolar II disorders (n:21) via the StructuredClinical Interview for DSM-IV were evaluated by interview with patients andrelatives, clinic records, hospital admission files and according to informationreceived from physicians who had followed these patients up.Marriage and family functioning, social support systems and functionality of bipolardisorders were assessed by self report scale datas (Family Assessment Scale, BipolarDisorder Functioning Scale, Social Support Scale and the Dyadic Adjustment Scale).Association between marriage and family process variables and clinical features ofillness such as age of onset, duration of illness, sum of duration of episodes, historyof hospitalisation, diagnostic subtype, psychotic symptoms, suicide attemps,recovery between episodes, seasonality, number of mood episodes and severity ofepisodes were analysed.Results: Duration of illness, sum of duration of episodes, recovery betweenepisodes, number of mood episodes and severity of episodes as clinical featureswere determined; significantly correlated to variables about marriage and familyfunctions. Duration of depressive episodes, number and severity of depressiveepisodes were significantly associated with these issues. Duration of illness andseverity of episodes were associated with social support systems and duration ofdepressive episodes with functionality. There was no association between marriageand family process and age of onset, history of hospitalisation, diagnostic subtype,psychotic symptoms, suicide attemps, seasonality.Conclusion: There is significant association between number, duration and severityof episodes and marriage and family functions in bipolar disorder especially indepressive episodes.
