Abstract
1. ÖZET Tüberküloz (TB) halen halk sağlığı için büyük bir tehdittir. TB tedavi başarısızlığının temel nedenleri uyunçsuzluk ve ilaç direncidir. Çalışmamızda TB hastalarının farmasötik bakım gereksinimleri ve ilişkili diğer unsurların belirlenmesinin yanı sıra, sözlü ve yazılı eğitim yöntemleri kullanılarak verilen hasta eğitiminin, uyunç üzerine etkisinin ölçülmesi amaçlanmıştır. TB hastalarının temel farmasötik bakım gereksinimlerinin ağrı kontrolü, beslenme, uygun reçeteleme, solunum ve kan şekeri kontrolü konularında olduğu belirlenmiştir. İlaca duyarlı TB hastaları eczacı tarafından verilen ilaçla ilişkili hasta eğitimi aldıklarında tedaviye uyumları, rutin bakım alan hastalardakinden daha yüksek bulunmuştur. Çalışmanın ikinci kısmında, çok ilaca dirençli tüberküloz (ÇİD-TB) tedavisinde etkinliği, güvenirliği ve immünomodülatör etkisine dayanılarak, ofloksasin'le karşılaştırmalı şekilde levofloksasin'in rolünün incelenmesi amaçlanmıştır. Her iki tedavi kolunda da olumlu sonuçlar gözlenmiş ve bu sonuçlar her iki ilacın da güvenlik profili açısından anlamlı bir ferk bulunmaksızın, ÇİD-TB tedavisinde etkili olduklarını göstermiştir. Her iki grupta da ÇİD-TB tedavisine yanıt olarak pozitif immünolojik değişimler kaydedilmiştir. Periferik kan CD4 T lenfosit düzeyleri artmış, CD8 T lenfosit düzeyleri azalmış ve CD4/8 oram artmıştır. Bu sonuçlar çalışılmış olan ÇİD-TB ilaç rejimlerinin yararlı immünomodülatör etkinliklere sahip olduklarını ortaya koymuştur. Çalışmamızın sonuçlarının klinik eczacının TB hastalığında hasta eğitimi, danışmanlık hizmeti, tedavi izlemi ve akılcı ve etkili ilaç tedavisinin sağlanması gibi eczacılık hizmetlerinin sunulmasında önemli rol oynayabileceğini göstermekte olduğunu düşünüyoruz. 2. SUMMARY Assessment of Pharmaceutical Care Services in Tuberculosis Patients and Investigation of The Role of Levofloxacin in Resistant Cases Tuberculosis (TB) is recognized as a major global threat to public health. The main causes of TB treatment failure are non-compliance and drug resistance. In our study we set out to understand the pharmaceutical care needs issues of TB patients. We aimed to measure the impact of oral and written education tools on compliance to drug therapy. The major pharmaceutical care needs of TB patients were found to be for pain control, nutrition, appropriate prescribing, respiratory and diabetic control. When drug- sensitive TB patients received pharmacist directed drug-related education their adherence to treatment was found to be superior to those receiving routine care. In the second part of our study we examined the role of levofloxacin in the treatment of multiple drug resistant tuberculosis (MDR-TB) focusing on the drug's effectiveness, safety and immunomodulatory activity, compared to ofloxacin. Both treatment arms demonstrated potentially successful outcomes and were shown to be effective in the treatment of MDR-TB with no significant difference in terms of safety profile. Positive immunological changes were noted for both groups in response to MDR-TB treatment. Peripheral blood CD4 T-lymphocytes were increased; CD8 lymphocytes were reduced and the ratio of CD4/CD8 lymphocytes was increased, indicating a beneficial immunomodulatory activity for MDR-TB drug regimens. The outcomes in this study show that the clinical pharmacist can play a key role in the provision of pharmaceutical services such as patient education, monitoring of therapy and ensuring rational and effective drug treatment for the benefit of the patient.
